Iritis Iritis, as it name suggests, affects the iris or coloured part of the eye. Nobody fully understands what causes it, but this inflammatory condition quite often follow on from inflammation elsewhere, such as arthritis. Generally it will affect only one eye which will become very painful and red, with blurred vision and sensitivity to light. The pupil will probably appear small too. The iris is heavily pigmented - when it becomes inflamed it releases pigment cells. A doctor or optometrist can pick up this sign using a Slit Lamp Microscope. Once diagnosed, it is important that the condition is treated at a hospital eye department as soon as possible. The pigment cells can block the fluid drainage channels inside the eye causing the pressure to go up, resulting in glaucoma. Similarly the inflamed iris will tend to stick to the lens directly behind, once again potentially causing glaucoma. Iritis treatment The condition is treated both with anti-inflammatory drops and drops to enlarge the pupil to prevent it sticking or to release any areas that have become stuck. The drops do cause blurring of vision but are an essential part of the treatment.

Iritis Iritis is a descriptive term for an inflammatory disorder of the colored part of the eye (iris). Sometimes iritis is just one symptom of a disease that affects other organ systems: arthritis and spinal degenerative disease, juvenile rheumatoid arthritis, psoriasis, Lupus, and other collagen vascular diseases, bowel disturbances such as ulcerative colitis and regional enteritis, sarcoid, and certain other very rare infections. Most often, however, iritis appears by itself. The symptoms of iritis include light sensitivity, red eye, blurred vision, tearing, pain, and sometimes floaters. The pupil may appear smaller in the affected eye when compared to the normal pupil. Frequently iritis is a recurrent problem; after a few episodes patients become very astute at early diagnosis. Iritis is sometimes confused with conjunctivitis, a much less serious disorder of the clear outer lining of the eye. The secrets to the successful treatment of iritis are early detection and proper choice of medications. Therapy consists of cortisone and dilating drops. These medicines quell the inflammation and reduce the scarring that can occur. Persistent cases may require more intensive treatment, such as injections of cortisone into the soft tissues around the eye. These injections help to avoid the potential serious systemic side effects of large doses of oral cortisone. Other oral medications are being investigated to better understand their role in treating this disease. Aspirin may prove to be very helpful.

It can be associated with other inflammatory conditions of the body, for example arthritis, but in most cases the cause remains unknown. It is a common condition which tends to recur, and although it usually only affects one eye at any one time, it is important to realise that iritis can affect either eye. Signs and symptoms Aching, painful red eye with small pupil, blurred vision and sensitivity to light. Treatment It is important that this condition is treated at an ophthalmic casualty department, as it can be difficult to diagnose without certain equipment, which is usually not available at your doctor's surgery. The doctor will prescribe anti-inflammatory drops which may be used frequently at first, then gradually tailed off. You must not suddenly stop using the drops, or the iritis may recur. You may also need to use an anti-inflammatory ointment last thing at night. You will be given drops to dilate your pupil. This will cause blurring of vision and difficulty in focusing, and may also increase your sensitivity to light, but it is a vital part of the treatment for iritis. If the pupil is not dilated, the inflamed iris will stick to the lens, which can lead to complications. You may require intensive pupil dilating drops in casualty to ensure your pupil is dilated before you go home.

I am hla-b27 positive with ankylosing spondylitis. The iritis has not responded to 4 weeks treatment with Pred Forte, Voltaren, and cyclogyl. My ophthalmalogist gave me a choice of an injection of steroids into the lower lid of my eye, or oral prednisone, 40 mg per day. I chose the injection, because I am breastfeeding my child. The injection has had very little effect during the last week, and my ophthalmalogist wants to discuss the possibility of the oral prednisone. My question is this; what is considered a safe level of oral steroids for a lactating mother to take, and how long a course of treatment is considered safe? Thanks.

CASE SUMMARIES: Two HIV-positive patients with cytomegalovirus retinitis infections previously controlled with intravenous ganciclovir or foscarnet were treated with intravenous cidofovir. Symptoms of iritis developed after the second or third dose of cidofovir. One patient experienced symptoms unilaterally, while the other patient had bilateral symptoms. In both patients, the iritis resolved with topical ophthalmic therapy, but recurred following subsequent infusions of cidofovir. Therapy with cidofovir was discontinued, and no further recurrences of iritis were noted. One patient had post-inflammatory fixed dilated pupils. CONCLUSIONS: Iritis can uncommonly occur in patients receiving intravenous cidofovir and oral probenecid.
