Diagnosis:
1) Isaacs' Syndrome
2) Syncope Patient #2 is a 39 year old male who was referred to our Center with a history of difficulty gripping things with his hands, "jumping muscles", slurred speech, and weight loss. The referring physicians were concerned about the possibility of Amyotrophic Lateral Sclerosis and the patient had been referred to evaluate this possibility. On clinical examination, the patient had good proximal strength with slight distal weakness. He did not appear to have the characteristic random muscle twitches which jump from place to place which would be characteristic of fasciculations. The movements were undulating in character like "worms moving under the skin", which is characteristic of myokymia. The EMG confirmed the absence of fasciculations and showed the presence of doublets, widespread irregular grouped motor unit discharges, and complex repetitive discharges with mixed widespread myotonic discharges which was consistent also with a diagnosis of Isaacs' syndrome or neuromyotonia. The patient's father also had a history of cramps, but the patient probably did not have the familial form of the disease because of the good response he had to treatment with plasma exchange, as outlined later.

A case of postpolio syndrome with myokymia and abnormal F response
Postpolio syndrome refers to new symptoms that may occur years after recovery from acute paralitic poliomyelitis. Diagnosis of PPS is based on clinical grounds. Conventional electrophysiological evaluation do not distinguish between residual paralitic poliomyelitis and PPS. We reported a case of PPS with myokymia and abnormal F response that is related with actual motor neuron degeneration

Regulation of Ca channels in nerve and muscle cells Voltage-gated (or voltage-dependent) Ca channels are composed of at least 5 (T, L, N, P/Q, R) types as determined by electrophysiological and pharmacological properties. In neurons, N- and P/Q-type channels are located in presynaptic terminals and trigger release of neurotransmitters. In cell bodies and dendrites of neurons, almost all the types of Ca channels are found, but their composition seems to be different in different types of neurons. Although their physiological roles are not fully understood, the channels are thought to play roles in plasticity of neural circuits and regulation of enzymes and gene expression. In the last decade, several classes of genes coding different types of Ca channels have been cloned and sequenced. It is interesting to investigate how expression and subcellular localization of each type of the channel are regulated. Ca channels are playing important roles also in muscle cells. In the skeletal and cardiac muscles, a plenty number of the channels are located in the transvers tubular system and transduce electrical signal to an intracellular signal that triggers release of Ca from sarcoplasmic reticulum. In addition, the channels contribute to the generation of action potential in the pacemaker cells, that is important for the automaticity of the heart. Activity of Ca channels is regulated by transmitters and hormones. For example, L-type Ca channel is up-regulated by beta-adrenergic stimulation through cAMP-protein kinase A (PKA) system. It has been shown that PKA phophorylates alpha and/or beta subunit of the channel protein. However, an exact molecular mechanism underlying regulation of the channel function is not fully understood. We have recently found that unidentified factors in cytoplasm of cardiac myocytes significantly affect activity of the Ca channel. We are now exploring to isolate the factors to elucidate the mechanism of action of the factors.

According to a news article, Robert Lord, age 10, fell from a tree and fractured his spine. He underwent surgery at Children's Hospital and the fall threatened to paralyze Robert for life. His father, Stephen Lord, concerned for his stricken son, went for medical help on the World Wide Web. Using search engines Robert's dad discovered a study that used the experimental drug GM-1 gamglioside. The study results indicated that of the 34 patients with injuries similar to Robert's, those given ganglioside within 72 hours of the injury had significantly better improvement than patients given a placebo. His father was able to get the U.S. Food and Drug Administration to grant the boy a special "compassionate use" permission to take the drug and with the help of the manufacturer the drug arrived within hours of the deadline. Now, Robert is walking again with the help of a walker and may be on his way to full recovery. Limited Information If you have a serious medical condition, it is in your best interest to learn as much as you can about your specific problem. Doctors cannot provide other than limited information about your problem because of constraints on their time. And, although doctors and pharmacists advise you of conflicts in your medications, thousands of seniors die each year because of taking too many different drugs or as not recommended. So you need to learn of the interactions of the different medicines, including those you purchase over-the-counter and their side-effects.

Review by Ellen Isaacs Rating: +2 In 1943 in Mississippi, Jewel Hilburn gives birth to a child with Down's Syndrome, changing the course of her and her family's life. Jewel, who spent part of her childhood in an orphanage, is determined not to put Brenda Kay in an institution, even though she and the doctors know relatively little about how to take care of her. The cost is that she has little energy left for her husband Leston and her five other children. Eventually, she sees an article in Reader's Digest of a school in Los Angeles for children like Brenda Kay, and she is determined to overcome Leston's objections to moving to a place so far and different from their home. When they finally move, Jewel becomes involved in the school, while the rest of the family grows up, building new lives in this very different world. By the end, Jewel and Leston reconcile their choices and how their lives have been affected by Brenda Kay. This book could have been one of those formula "triumph over adversity" stories that are frequently made into Hollywood movies. Instead, I found this book to be a rich character study that gave a realistic portrait of how people make do when faced with hardship. I'm no expert on Down's Syndrome, but I did spend a summer as a counselor for kids with Down's, and the characterization of Brenda Kay's behavior rang true to me. So did the way Jewel gradually adjusted her expectations to match Brenda Kay's abilities, so that she could take great joy in each small step, just as you would with any child. I found it especially interesting to see how Jewel coped at a time when there was less support for people with this syndrome, at a time when the syndrome was actually called Mongoloid Idiocy. Lott's lovely writing style and insightful but understated tone enhance the story.

History A 22-year-old woman with a 4-year history of insulin dependent diabetes mellitus and hypothyroidism presents to the electrodiagnostic laboratory with a chief complaint of involuntary muscle tightening and spasms. She has not experienced pain in association with the spasms, and she has no muscle cramps. She denies weakness. Her symptoms began 8 years ago while she was in high school, and the muscle tightening and spasms have been slowly progressive in severity and duration. The episodic tightening or twitching of her muscles involves her abdominal, hand, and lower limb muscles bilaterally. These paroxysmal episodes last for approximately 5 seconds and occur many times during the day. On occasion, she notices that objects fall from her hands. She believes that cold may induce her symptoms. Initially, she was not concerned about the abdominal spasms since they helped maintain her muscle tone.
  • Prior to continuing, please develop a differential diagnosis and list each possible diagnosis in order of likelihood.
