Hyaluronate for temporomandibular joint disorders (Cochrane Review)
Shi Z, Guo C, Awad M ABSTRACT A substantive amendment to this systematic review was last made on 14 November 2002. Cochrane reviews are regularly checked and updated if necessary. Background: Temporomandibular joint disorders (TMD) refer to a group of heterogeneous pain and dysfunction conditions involving the masticatory system, reducing life quality of the sufferers. Intra-articular injection of hyaluronate for TMD has been used for nearly two decades but the clinical effectiveness of the agent has not been summarized in the form of a systematic review. Objectives: To assess the effectiveness of intra-articular injection of hyaluronate both alone and in combination with other remedies on temporomandibular joint disorders. Search strategy: Intensive electronic and handsearches were carried out. The Oral Health Group's Trials Register (September 2001), The Cochrane Library CENTRAL database (Issue 3, 2001), MEDLINE (1966- May 2001), PubMed ( up to March 2002), EMBASE (1974 - August 2001), SIGLE (1980 - December 2001), CBMdisc (1983 - July 2001, in Chinese) and Chinese Medical Library were searched. All the Chinese professional journals in the oral health field were handsearched and conference proceedings consulted. There was no language restriction. Selection criteria: Randomized or quasi-randomized controlled trials (RCTs), with single or double blind, design testing the effectiveness of hyaluronate for patients with temporomandibular joint disorders.

Background: There has been a long history of using occlusal adjustment in the management of temporomandibular disorders (TMD). It is not clear if occlusal adjustment is effective in treating TMD. Objectives: To assess the effectiveness of occlusal adjustment for treating TMD in adults and preventing TMD. Search strategy: We searched the Cochrane Oral Health Group's Trials Register (April 2002); the Cochrane Central Register of Controlled Trials (CENTRAL) (The Cochrane Library Issue 2, 2002); MEDLINE (1966 to 8th April 2002); EMBASE (1980 to 8th April 2002) and handsearched journals of particular importance to this review. Additional reports were identified from the reference lists of retrieved reports and from review articles of treating TMD. There were no language restrictions. Unpublished reports or abstracts were considered from the SIGLE database.

During the last decade Orthopedic Surgery has made an enormous progress. This is mainly based on clinical research. For a long time the research of our department has concentrated on clinical research concerning fractures, posttraumatic deformity, joint disorders and bone healing. Over the last years the aim of our research programme has been to supply a scientific basis for Orthopedic Surgery. Clinical research is concentrated on prospective randomized trials to improve diagnostic and therapeutic strategies for several joint disorders. Development of instruments and operative techniques are based on anatomic studies and mechanical testing. Mechano-biological processes, and mechanical bone quality and adaptation of bone to endoprotheses are studied. This includes the role of fibrous tissues around implants. A main part of the study is directed towards bone healing. Apart from prospective trials the anchondral ossification is studied in vitro.

temporomandibular joint disorders By Adam Brochert, MD Images (click to view larger image) Temporomandibular joint disorder, which is also known as TMJ, is a painful condition that affects the joint connecting the jawbone to the skull. TMJ may also involve the surrounding muscles and soft tissues. What is going on in the body? Temporomandibular joint disorders are poorly understood. They are thought to involve a disturbance of the joint that connects the back of the jaw to the skull. This joint is located just in front of the ear and can be felt when the jaw opens and closes. What are the signs and symptoms of the condition? Temporomandibular joint disorders may cause a wide variety of complaints, such as: a clicking or snapping noise heard in the joint when the jaw is opened and closed headaches jaw pain joint locking open or shut limited ability to open the mouth pain behind the eye pain that spreads to the shoulders, neck, and back spasm or pain in the muscles used for chewing Certain other symptoms are often seen in people who have temporomandibular joint disorders. These symptoms may be part of the TMJ. Sometimes they may be caused by another condition that is also occurring at the same time. These symptoms include: dizziness ear pain hearing impairment ringing in the ears , also known as tinnitus vertigo , which is a sense of the room spinning around that is worsened by sudden movement or turning visual impairments , such as blurred vision What are the causes and risks of the condition?
