What is TMJ? TMJ is short for Tempromandibular Joint. All people have two TMJ's. Some people have a disorder of one or both TM joint's and this is what people are refering to when they say that "they have TMJ." Actually they are having some sort of "disorder" with one or both TM joints. The TM Joints are the only joints in the body that are directed into position by something other than soft tissues (ligaments, tendons, muscles, etc.) The teeth play the major role in where the joints position upon closing. As there is no resiliancy in the teeth the joints can then be forced into a position that is not favorable. This generally will result in some sort of abnormality in one or both the joints, but not necessarily pain. Sometimes a "click" or "pop" can be heard upon opening or closing the mouth. Although this is not considered normal, many people lead normal healthy lives without discomfort and with the presence of such "joint sounds." Although it does not have to be exactaly perfect, there must be a considerable amount of harmony between the teeth and the TM joints.

Temporomandibular Joint Disorders Dysfunction of this joint can result in pain, limited movement, noises and difficulty chewing and opening the mouth. TMJ disorders can be relatively minor, or they can be extremely painful and debilitating. The most common signs and symptoms in patients with TMJ problems are:
  • Pain in the area of the joint Clicking, popping or crackling sounds in the joint A habit of clenching or grinding the jaws Sore jaw muscles when awakening Difficulty in fully opening the mouth Frequent headaches or neckaches Locking of the joint
The cause of most TM joint problems is related to trauma, systemic disease, or developmental jaw problems. Trauma can be either due to an acute injury (macrotrauma), such as a fall, car accident, or blow to the jaw, or due to a prolonged minor stress to the joints (microtrauma) caused by clenching or grinding the teeth. This frequently results in displacement of the disc of cartilage that cushions the ball-and-socket of the joint, resulting in clicking and pain. Diseases such as osteoarthritis, rheumatoid arthritis, and lupus can also affect the TMJ, causing destruction of the cartilage

A strong commitmet has been made by the Vaderbilt Center or Craniofacial Treatment to the surgical correction of TMJ dysfunction. For many patients surgery and total joint reconstruction can make significant progress to relieve chronic pain. This pain is present in the preauricular region and can radiate to the top of the head, down to the mandible or to the side of the neck. Clinical conferences are being held once in a month to discuss the diagnosis and treatment of patients with TMD who need a multidisciplinary approach. A home page on TMD was open in WWW (World-Wide Web) to display them and to make them accessible from the members of the team as well as other doctors outside the hospital. The contents include chief complaints, clinical history, present status, radiographic, CT and MRI images, treatment history and problems in each year. each case is discussed in amiling list or in conference corner in WWW. So far, 25 cases discussed in 17 conferences were displayed in WWW. The system seems to be quite effective in establishing the treatment of TMD because information on each case is available anytime to anyone involved in the study and treatment of TMD.

What is a TMJ Problem?
When a patient is said to have TMJ, this means they could have any one of a group of conditions that affects the function of the jaw. Another and better collective term is TMD which means Temporomandibular Disorders. Actually the term TMJ is an abbreviation which means Temporomandibular Joint or Jaw Joint. Using this term does not mean the joint itself is the source of the problem (although sometimes it is). If you have been told you have TMJ or TMD, it could mean you have a disk problem in the jaw, arthritis of the jaw joint, jaw muscles that are inflammed and painful, or one of about 15 other conditions which are known to disrupt jaw function. Why should I be concerned?

The case is frequently unrecognized by physicians whose standard training has not equipped them to understand the relationship between the teeth and jawbone and the alignment of the jaws and rest of the body. C: Articular Disc Clicking Jaw. Disc is displaced forward of condyle, clicking on and off of the condyle with opening and closing. TMD-THE LOCKED JAW
If the clicking joint is not treated, it may lead to "locking" of the jaw. In this condition, the disc becomes deformed or wedged between the ball and socket and the sufferer experiences a limitation of jaw movement often with a feeling of "catching or locking" in the joint with movement. Headaches and jaw or facial pain are usually seen with these patients. In addition, many experience various ear symptoms. If treated early, most patients can be treated without surgery but if allowed to progress, degenerative changes and surgery may result.
