Intermediate Hemolytic Disease (Kernicterus)
Infants with intermediate hemolytic disease are generally born at or near term (40 weeks) and appear to be in good condition. If, however, these infants are not carefully treated at birth, they can develop anemia and increased levels of bilirubin (hyperbilirubinemia) as a result of the increase red blood cell destruction. When red blood cells are destroyed, one of the byproducts is bilirubin. High levels of bilirubin can cause a condition called kernicterus which results in nerve damage in the brain. Prevention of kernicterus includes careful monitoring of neonatal bilirubin levels and putting the baby under special lights (phototherapy) to reduce the level of bilirubin in the body. In many cases, an exchange transfusion is needed. This involves removing much of the baby's blood and replacing it with Rh-negative blood. The new blood, which is carefully screened to be compatible with the baby and to be free from diseases (including HIV and Hepatitis), helps to eliminate the anemia by bringing in healthy red blood cells and also removes much of the excess bilirubin. Kernicterus is quite rare with advanced techniques in neonatal management. Experts in the field report knowing of few, if any, cases in recent history. Those babies who do develop kernicterus and survive may suffer from deafness, cerebral palsy and/or other conditions caused by the nerve damage.

Definición:
La incompatibilidad Rh se desarrolla cuando existe una diferencia en el tipo de sangre Rh de la mujer embarazada (Rh negativo) y el del feto (Rh positivo). Como consecuencia, se forman anticuerpos anti-Rh positivos. Ver también incompatibilidad ABO
Causas, incidencia y factores de riesgo:
Durante el embarazo , los glóbulos rojos de la circulación fetal se mezclan con la circulación materna. Si la madre es Rh negativo, su sistema no tolerará la presencia de células Rh positivas. Su sistema inmunológico trata a las células Rh fetales positivas como si fuesen una proteína o sustancia extraña y crea anticuerpos contra las células sanguíneas fetales. Estos anticuerpos anti-Rh positivos se desplazan a través de la placenta hacia el feto y destruyen los glóbulos rojos circulantes. Por lo general, el primer hijo no se ve afectado (a menos que la madre haya tenido embarazos interrumpidos anteriormente, los cuales podrían haber sensibilizado su sistema) ya que toma tiempo para que la madre desarrolle anticuerpos contra la sangre fetal.

Unconjugated bilirubin is lipid soluble and travels in the circulation largely bound to albumin. However, if the levels of unconjugated bilirubin are high a small fraction of free bilirubin escapes from the vascular compartment and enters the cellular lipid fraction of brain cells, causing a brain damage that may or may not be permanent, i.e. kernicterus.

Any baby with untreated jaundice is at risk for kernicterus. This does not mean that every baby with yellow skin will have brain damage. Most babies with jaundice get better by themselves. (Summary) We report a 2-year-old-girl with a clinical record of neonatal jaundice due to AB0 incompatibility.
She attended the outpatient department with dystonia and slow psychomotor development.
MRI showed symmetrical bipalidal lesion with low signal in T1-IR weighted images and high signal in T2-weighted images.
The diagnosis was kernicterus.

The yellow staining in the brain of a neonate is known as kernicterus. There is a coronal section of medulla on the left and cerebral hemisphere on the right demonstrating kernicterus in deep grey matter of hemisphere and brain stem. Increased unconjugated bilirubin, which accounts for the kernicterus, is toxic to the brain tissue. Kernicterus is more likely to occur with prematurity, low birth weight, and increased bilirubin levels.

(bilirubine encefalopathie) Kernicterus is een aandoening van de hersenen waarbij als gevolg van een teveel aan bilirubine in het bloed (zichtbaar als geelzucht = 'icterus') een beschadiging is ontstaan van bepaalde delen van de hersenen, met name van de zogeheten basale kernen (vandaar de naam 'kern-icterus'). Kenmerken eerste jaar:
  • Bij of kort na de geboorte:
  • geelzucht, sufheid en spierslapte en slecht drinken
  • vervolgens ontstaan na enkele dagen koorts en stijfheid van de spieren
  • afname koorts en stijfheid vanaf tweede week
  • epilepsie
  • Eerste jaar:
  • slecht drinken
  • slappe spieren
  • vertraagde ontwikkeling
Kenmerken later:
  • doofheid of ernstige slechthorendheid
  • scheelzien
  • schokkering, onbedoelde, bewegingen van de ledematen
  • verstandelijke handicap (licht tot ernstig)
  • spraakproblemen
  • eet- en slikproblemen
