Bilirubine encephalopathy (kernicterus). Het ontstaan van hypertonie is een slechtvoorteken, aangezien dan meestal later kernicterus blijkt te zijn ontstaan.

Home Syndromen Problemen Algemene linkpagina Bilirubine encephalopathy (kernicterus) Aandoening van de hersenen waarbij als gevolg van hyperbilirubinemie hersenbeschadiging is ontstaan.
De naam bilirubine encephalopathy wordt zowel gebruikt voor: -1- de acute fase met stupor, hypertonie and koorts bij een pasgeborene met hyperbilirubinemie, als voor -2- de chronische fase met neurologische beschadiging als gevolg van (postnatale) hyperbilirubinemia, met athetose, blikparese of strabisme en gehoorverlies. Kenmerken acute bilirubine encephalopathie: De acute bilirubine encephalopathie verloopt volgens drie fasen:
Eerste fase: stupor, hypotonie en voedingsproblemen door een slechte zuigreflex. Deze niet-specifieke symptomen worden na enkele dagen gevolgd door:
Tweede fase: koorts (tot hyperpyrexie met epilepsie) en hypertonie (vooral in de extensoren, waardoor retrocollis en opisthotonus ontstaan. Het ontstaan van hypertonie is een slecht voorteken, aangezien dan meestal later kernicterus blijkt te zijn ontstaan.
Derde fase: In de tweede week treedt een vermindering (of geheel verdwijnen) van de hypertonie op. Dit heeft prognostisch geen betekenis, aangezien uiteindelijk iedereen die in de tweede fase hypertonie ontwikkelde, later kernicterus zal krijgen.

Causas, incidencia y factores de riesgo: Durante el embarazo , los gl³bulos rojos de la circulaci³n fetal se mezclan con la circulaci³n materna. Si la madre es Rh negativo, su organismo no tolerar¡ la presencia de c©lulas Rh positivas. Su sistema inmunol³gico trata a las c©lulas Rh fetales positivas como si fuesen una prote­na o sustancia extra±a y crea anticuerpos contra las c©lulas sangu­neas fetales. Estos anticuerpos anti-Rh positivos se desplazan a trav©s de la placenta hacia el feto y destruyen los gl³bulos rojos circulantes. Por lo general, el primer hijo no se ve afectado (a menos que la madre haya tenido embarazos interrumpidos anteriormente, los cuales podr­an haber sensibilizado su organismo), ya que toma tiempo que la madre desarrolle anticuerpos contra la sangre fetal. Rh incompatibility may cause symptoms ranging from very mild to fatal. In its mildest form, Rh incompatibility causes hemolysis (destruction of the red blood cells) with the release of free hemoglobin bilirubin , which causes an infant to become yellow (jaundiced). The jaundice
Hydrops fetalis is a complication of a severe form of Rh incompatibility in which massive fetal red blood cell destruction (a result of the Rh incompatibility) causes a severe anemia resulting in fetal heart failure , total body swelling respiratory distress (if the infant has been delivered), and circulatory collapse. Hydrops fetalis often results in death of the infant shortly before or after delivery.

    Causes and Risks:
    During pregnancy , red blood cells from the fetal circulation leak into the maternal circulation. If the mother is Rh negative, her system will not tolerate the presence of Rh positive cells. Her immune system treats the Rh positive fetal cells as if they were a foreign protein or substance and makes antibodies against the fetal blood cells. These anti-Rh positive antibodies move through the placenta into the fetus where they destroy the fetus' circulating red blood cells. First-born infants are often not affected (unless the mother has had previous interrupted pregnancies, which could have sensitized her system) as it takes time for the mother to develop antibodies against the fetal blood.
    Rh incompatibility may cause symptoms ranging from very mild to death of the newborn. In its mildest form, Rh incompatibility causes hemolysis (destruction of the red blood cells) with the release of free hemoglobin into the infant's circulation. Hemoglobin is converted into
