Before surgery 16 months after surgery This is a 17 year old male with a history of gradual growth of his breasts over the past four years. He was bothered by the breasts to the point of not wearing T-shirts and keeping himself covered at the beach. He states he walked with his hands covering his chest so that they were not seen. Gym activities embarrassed him. He did not have a hormonal workup before coming to my office. His breast examination revealed mild to moderate breast enlargement with primarily glandular tissue. His testicles were small. His pediatrician assisted with a work up that revealed Klinefelter's syndrome. This is a common cause of gynecomastia and genetic problem seen in males (1:500). Hypogonadism is a problem with small testicles and low hormone output. Some patients do not develop gynecomastia until after puberty.

Boys are made of special things and sometimes some Xtra special things! Allow me to begin by telling you that many doctors and researchers do not refer to Klinefelter Syndrome when referring to a child because a child can not and does not have the symptoms of what Harry Klinefelter researched and not all babies and boys will go on to develop the symptoms associated with Klinefelters Syndrome and until they do, they do not have a syndrome. There is the blessing of the early diagnosis. SO much can be changed but just having that advantage. Use it to yours!! So you have just been told that your baby has an extraX.......There is so much more to know than just that! Please take the time to educate yourself. The unknown is a scary place to be and usually the FACTS are not as bad as they first may seem. Please read on!
I was in your shoes just a short time ago when I was told that my two month old baby had this disorder. I was terrified....but I also love my child with all of my heart so I took the time to learn. I will share with you what I felt, heard, read and learned and hopefully provide you with something I wished I had had available to me the day I was told.

My father kept saying that these would be the best years of my life: The years of junior and senior high were going to be the best that life could be. Such concepts gives Stephen King the power to inhibit the dreams of cheerleaders everywhere, and for me, not much better, for I was fat. Had I simply been fat, it would have been tolerable. But I went beyond fat. I had breasts, and everyone I knew seemed to know it. Everyone, that is, except the doctors I went to see. Year after year, from when I was 12 until I was 19, every new doctor said the same thing. "You're Fat. Go on a diet." I could almost read the minds of each one as I walked in their office for the first time. 'Hello Tubby' was written on every scowl and grimace. And year after year, the chants at school of "You need a bra" took me lower and lower down the path of self-defeat. It was a long first year in junior college, and now I was going to go see a new doctor. Here we go again. The doctor looked at me with a concentrated expression. "You've got breasts" were his first words. I shall never forget his next line. "Those are SYMPTOMS." Over the course of the next hour, he called my mom and got permission to have a rare and costly test done. He then ordered that they do a karotype (chromosome count) on me, as well as a hormone level test, and a sperm count. The test took a week, and the conclusion was not obesity. It was something totally alien to me, and apparently to all the other doctors I had seen.

About 47XXY/Klinefelter Syndrome 47/XXY is estimated to occur in one out of 500 males, making it the most common sex chromosome variation. Rather than the normal male chromosome pattern, 46XY, there is an extra X chromosome. The result is the 47/XXY arrangement. This variation happens when paired chromosomes fail to separate in the first or second stage of meiosis. For many years, this condition has been called Klinefelter Syndrome, named for Dr. Harry Klinefelter, who published a paper in 1942 about nine men with enlarged breasts, sparse facial and body hair, small testes, and an inability to produce sperm. However, because the manifestations of 47XXY vary from person to person, some individuals may not have all of the characteristics described. Thus, medical researchers prefer the term XXY when describing this condition. Some variations of this condition include 46XY/47XXY mosaicism; 48XXXY 48XXYY; and 46XX males, which is termed sex-reversal syndrome.

WHAT IS KLINEFELTER SYNDROME? In 1942, Dr. Harry Klinefelter and his coworkers at the Massachusetts General Hospital in Boston published a report about nine men who had enlarged breasts, sparse facial and body hair, small testes, and an inability to produce sperm. By the late 1950s, researchers discovered that men with Klinefelter syndrome, as this group of symptoms came to be called, had an extra sex chromosome, XXY instead of the usual male arrangement, XY. (For a more complete explanation of the role this extra chromosome plays, see the accompanying section, "Chromosomes and Klinefelter syndrome." In the early 1970s, researchers around the world sought to identify males having the extra chromosome by screening large numbers of newborn babies. One of the largest of these studies, sponsored by the National Institute of Child Health and Human Development (NICHD), checked the chromosomes of more than 40,000 infants. Based on these studies, the XXY chromosome arrangement appears to be one of the most common genetic abnormalities known, occurring as frequently as 1 in 500 to 1 in 1,000 male births. Although the syndrome's cause, an extra sex chromosome, is widespread, the syndrome itself-the set of symptoms and characteristics that may result from having the extra chromosome-is uncommon. Many men live out their lives without ever even suspecting that they have an additional chromosome.

An infant appears normal at birth, but the defect usually becomes apparent in puberty when secondary sexual characteristics fail or are late to develop, and testicular changes occur that eventually result in infertility in the majority of those affected. Some mild cases may go undetected with no abnormalities present except infertility. Increased maternal age is a risk factor.
