The Child Psychiatry Branch of the National Institute of Mental Health is conducting a study of the effects of sex chromosomes on brain development. This study hopes to determine whether brain-imaging studies of children with sex chromosome variations will help uncover core biological features of these chromosomal conditions. Male volunteers aged 5-25 with any sex chromosome variation (XXY, XYY, XXYY, XXXY, XXXXY) are needed to participate in this study. Participation involves coming to the National Institutes of Health (NIH) in Bethesda, Maryland (just outside of Washington D.C.) for a one-day visit, which will include:
  • an interview a brief physical exam cognitive testing a blood draw for genotyping and a brain magnetic resonance imaging (MRI) scan (the MRI scan provides detailed images of the brain without the use of radiation)
We also ask that the child and parents return 2 years after the first visit for a follow-up MRI.

Klinefelter Syndrome
Dateline: 06/26/99 Doctors are trying to get away from calling this a "syndrome". Klinefelter Syndrome or XXY Male is a common genetic abnormality. There are few clinical manifestations and a person may live for-years-or-forever without anyone knowing he has it. In this condition the patient looks male but the sex chromosome make up is XXY instead of XY. (There are also other, less common, chromosomal variations.) Klinefelters is occasionally diagnosed prenatally when cells are analyzed for chromosome abnormalities during pregnancy (through amniocentesis or chorionic villus sampling). Sometimes the diagnosis is made during puberty because of the noticeable and potentially embarrassing physical signs which include enlarged breasts, little body hair and extra height. Testes are small and there is a tendency toward androgen deficiency. Affected males are usually sterile. The benefit of knowing early that a baby or child has Klinefelters is that parents can be alert for signals that a child may need medical or developmental intervention. Most symptoms are treatable and early intervention can prevent developmental and educational difficulties.

Description Klinefelter syndrome is a condition where one or more extra X-chromosomes are present in a male. Boys with this condition appear normal at birth. They enter puberty normally, but by mid puberty have low levels of testosterone causing small testicles and the inability to make sperm. Affected males may also have learning disabilities and behavior problems such as shyness and immaturity and at an increased risk for certain other health problems. Klinefelter syndrome is one of the most common chromosomal abnormalities. About 1 in every 500 to 800 males is born with this disorder. Approximately 3% of the infertile male population have Klinefelter syndrome. Causes and symptoms Chromosomes are found in the cells in the body. Chromosomes contain genes, structures that tell the body how to grow and develop. Chromosomes are responsible for passing on hereditary traits from parents to child. Chromosomes also determine whether the child will be male or female. Normally, a person has a total of 46 chromosomes in each cell, two of which are responsible for determining that individual's sex. These two sex chromosomes are called X and Y. The combination of these two types of chromosomes determines the sex of a child. Females have two X chromosomes (the XX combination); males have one X and one Y chromosome (the XY combination).

Klinefelter syndrome is caused by an extra X chromosome and affects only males. An infant appears normal at birth, but the defect usually becomes apparent in puberty when secondary sexual characteristics fail or are late to develop, and testicular changes occur that eventually result in infertility in the majority of those affected. Some mild cases may go undetected with no abnormalities present except infertility. Increased maternal age is a risk factor. It is usually a sporadic event, not inherited, that leads to the extra X chromosome.

Klinefelter Syndrome
Klinefelter Syndrome occurs in 1 in 500 to 1 in 1000 live births. People with this disorder develop as males with subtle characteristics that become apparent during puberty. They are often tall and usually do not develop secondary sex characteristics such as facial hair, or underarm and pubic hair. Men and boys with Klinefelter Syndrome have a Y chromosome and 2 X chromosomes. This is an example of trisomy Where does the extra chromosome come from? In about half of Kleinfelter cases, the extra X chromosome is from the mother's egg, while in the other half of cases the extra chromosome is from the father's sperm.

WHAT IS KLINEFELTER SYNDROME?
Klinefelter syndrome is the name given to a condition in which there is a characteristic group of symptoms including the tendency to develop breasts, sparse facial hair, and an inability to produce sperm. The condition only affects boys. An additional X chromosome is found in the cells of these affected boys, giving them two X chromosomes instead of the usual one.
The syndrome was first described in 1942 by Dr Harry Klinefelter and it was later given his name. Klinefelter syndrome is one of the more common genetic conditions and occurs in between 1 in 500 and 1 in 1000 males.
WHAT CAUSES KLINEFELTER SYNDROME?
Klinefelter syndrome is the result of a mistake in cell division either at the time of conception or soon after. Males usually have one X chromosome which they receive from their mother and a Y chromosome which they receive from their father. These are called the sex chromosomes. They also have 22 pairs of autosomes (chromosomes which are not sex chromosomes), one of each pair from mother and the other from father. Therefore, there are 46 chromosomes altogether in every cell of the body and this is written as 46,XY.

At least two of the chromosomes must be X chromosomes, and one must be a Y chromosome. The condition causes males to have some female-like physical features. What is going on in the body?
