But I went beyond fat. I had breasts, and everyone I knew seemed to know it. Everyone, that is, except the doctors I went to see. Year after year, from when I was 12 until I was 19, every new doctor said the same thing. "You're Fat. Go on a diet." I could almost read the minds of each one as I walked in their office for the first time. 'Hello Tubby' was written on every scowl and grimace. And year after year, the chants at school of "You need a bra" took me lower and lower down the path of self-defeat. It was a long first year in junior college, and now I was going to go see a new doctor. Here we go again. The doctor looked at me with a concentrated expression. Over the course of the next hour, he called my mom and got permission to have a rare and costly test done. He then ordered that they do a karotype (chromosome count) on me, as well as a hormone level test, and a sperm count. The test took a week, and the conclusion was not obesity. It was something totally alien to me, and apparently to all the other doctors I had seen.

Klinefelter's syndrome is found in approximately 1 of 700 men, i.e. there are approximately 3600 boys and men with Klinefelter's syndrome in Denmark with 5 million inhabitants. Men usually have but one X and one Y chromosome, i.e. the chromosome constitution 46,XY. Men with Klinefelter's syndrome have more than one X chromosome, usually two X chromosomes, i.e. the chromosome constitution 47,XXY. At birth the testicles of Klinefelter boys are of normal size. When the testicles grow quickly in boys with normal chromosomes at the age of 11-12 years, the testicles of Klinefelter boys stay very small, as a rule only 2 cms. from pole to pole. Usually only few sperms are developed in the testicles, and men with Klinefelter's syndrome are as a rule infertile. Boys with Klinefelter's syndrome should be treated with testosterone, usually from the 11 or 12 years of age, best with Restandol tablets (Testosteroneundecanoat), which does not affect the liver, or with injections of other testosterone preparations. Klinefelter boys more often than other boys have delayed motor function, speech, and maturation development, which does not make them patients demanding special treatment. It is important that these boys at an early age go to a good day-care institution, and that there is a close cooperation between parents and day-care institution staff. If the speech development is delayed, it is important to get help from a speech-therapist for a period. Regarding the motor function development, the participation of parent/child in sports activities and group activities of any kind is of great value. The same is the case with regard to stimulating the delayed development in maturation. In this connection it must be mentioned, that it is also important to stimulate these boys to independence, and not in any way to overprotect them.

I was born with Klinefelter's syndrome (XXY). I get a shot of testosterone every 3 weeks but it does not seem to do much for my libido or anything else. I'm 65, have diabetes mellitus, and am on blood thinners (to prevent clotting in my legs) and I have poor circulation. I guess I should 'hang up my sexual hat', but I didn't find out about the XXY until I was 53, and I went through puberty at 54! Answer Even though you found out that you have Klinefelter's syndrome 11 years ago I'm wondering just how much your physician has explained to you. Here are a few remarks on it. There are 23 pairs of chromosomes found in each cell of the body, and each contain genes that determine our coloring, our features, and our sex. Women inherit two X chromosomes - one from each parent, written XX. Men inherit an X chromosome from their mothers and a Y chromosome from their fathers, written XY. The X chromosome determines whether a child is going to be a male or a female. X-chromosome mutations cause various human genetic disorders, such as Turner's syndrome and Klinefelter's syndrome.

KLINEFELTER'S SYNDROME Dear Alice, Dear Reader, Men who are born with two X chromosomes [or a genotype (genetic information) of XXY instead of the more typical XY] usually develop a condition called Klinefelter's syndrome. Klinefelter's is the most common sex chromosome disorder, affecting approximately 1 in 500 males across all ethnic and racial groups. Because the "extra" chromosome is a sex chromosome, most of the physical effects of Klinefelter's syndrome become apparent during puberty, if at all. Men with Klinefelter's tend to be tall and have decreased facial hair and muscle mass. They are also more likely to have breast development and very small testicles. Some of these physical differences are thought to be the result of decreased testosterone levels. Injections of synthetic testosterone have been found to increase muscle development and bone density, giving men with Klinefelter's syndrome a more traditionally masculine appearance. However, this replenishment of testosterone cannot correct the infertility that most with Klinefelter's have as a result of the condition. Some research suggests that early (recommended by ages 11 - 13 years) testosterone replacement therapy may decrease the incidence and severity of learning disabilities that can be related to having Klinefelter's, potentially improving overall quality of life. Research on the cognitive problems associated with Klinefelter's are still in the early stages though.

Klinefelter's Syndrome There is a wide variety of symptoms of which any number may be expressed with varying severity. They include lack of secondary sexual characteristics, development of female characteristics, infertility, language impairment, and mental retardation. Mental retardation can be caused by Klinefelter Syndrome, but not in the usual 47, XXY karyotype. Instead, it results from multiple aneuploidies such as 48, XXXY; 48, XXYY; and 49, XXXXY (Robinson et. al., 1994). These individuals generally demonstrate more cognitive and behavioral problems, and therefore, can not be considered simultaneously with the more common 47, XXYcases (Utah Department of Health, 1993). Another possible variant is a mosaic individual which means that he possesses two or more different cell lines differing in the number of chromosomes (Robinson et. al., 1994). A mosaic Klinefelter male, which is much less common than the other types, would have a 46, XY/47, XXY karyotype. Klinefelter syndrome in these individuals can be more or less severe, depending on which cell line is used for the symptomatic traits. The prognosis for Klinefelter patients is bright in some areas and unclear in others. To deal with the lack of secondary sexual features and other problems caused by low quantities of testosterone, there is testosterone therapy. Testosterone should be administered starting at eleven to twelve years of age, when puberty normally begins. The benefit is a more normal appearance including weight gain, increased muscle mass, increased hair distribution, acne, and less feminization (Slywka, 1994). If the syndrome is diagnosed later in life, testosterone treatmentsshould still be helpful and effective and usually increase sex drive. These treatments are available in oral, injectable, and patch therapy form. If gynecomastia is already prevalent, breast reduction surgery is a feasible option.
