Saving Voice Dual Treatment for Larynx Cancer by Gary Logan Oncologist Arlene Forastiere In the early 1990s, advances in the ability of chemotherapy and radiation therapy to shrink and kill tumors had a profound effect on treating larynx cancers. A landmark study showed that the survival rate for patients treated with these new methods was as good as for those who had their larynx removed. Suddenly, about two-thirds of laryngeal cancer patients were able to keep their voice box without shortening their lives. One-third, however, still faced life without their own voice. Stages of larynx cancer
Stage I is the stage where the cancer is in a central location where it started and has not spread. Stage II occurs when the cancer is only in the larynx and has not spread to any other parts of the body. Stage III is when the cancer has not spread but the vocal cords cannot move normally; the cancer has spread to the tissues next to the larynx; or the cancer has spread to one lymph node on the same side of the neck as the cancer.

What is the Larynx? The voice box or the larynx is a tubular, 2-inch long organ in the neck. The larynx is used by humans to talk, breath, or swallow. The larynx sits atop the windpipe or the trachea. It is made of a material called cartilage. Inside Adam's apple, forming a 'V', are two bands of muscle called the vocal cords . During the process of breathing air, the vocal cords inside the larynx are narrowed or relaxed voluntarily and the air passing in between them sets up vibrations, which produces the sound of our voice. The tongue, the teeth, and the lips form the sound into coherent words.

Lubin, Ph.D. Primary lung cancer accounts for about 15 percent of all cancer cases (19 percent in males and 11 percent in females) in the United States; however, because of its high death rate, it accounts for 29 percent of all cancer deathsÑ35 percent in males and 21 percent in females (Boring et al., 1994). The overall 5-year relative survival rate is only 13 percent (Ries et al., 1994). Lung cancer remains the leading cause of cancer death in most countries. Maoris of New Zealand experienced the highest incidence rate in the world among males, with a world standardized rate of 119.1 per 100,000 (Parkin et al., 1992). U.S. black men were near the highest, with an incidence rate of 90.0 per 100,000. Low rates were noted for Israel, India, and Latin American countries. The worldwide incidence of lung cancer is substantially lower in femalesa difference generally attributed to lighter tobacco consumption by women, although other factors may play a role. For example, high levels of indoor and outdoor air pollution have been postulated as an important contributor to the high rates of lung cancer among females in China, whose smoking prevalence is relatively low (Blot and Fraumeni, 1992). In the United States, lung cancer incidence has risen more sharply in females (4.6 percent annual change) than in males (0.5 percent annual change) in recent years, reflecting the growing popularity of cigarette smoking among females over the past several decades (Ries et al., 1994). Currently, more women die each year from lung cancer than breast cancer (Boring et al., 1994).

Larynx Cancer, Laryngeal Cancer Medical Term: None Specified Description: Cancer of the larynx (voice box), and surrounding tissues. . Usually in patients over age 50 years old. Men are nine times more affected than women. Causes: The cause has not been identified, but there is a very strong association with smoking, and also with heavy alcohol use. Prevention: Do not smoke. Limit alcohol use. See your physician if you develop any early symptoms. Persistent hoarseness . Feeling of lump in the throat. Pain or difficulty while swallowing. Hard, enlarged lumps in the neck. Unexplained weight loss. Tenderness in the neck. Chronic cough. Change in voice quality. Risk Factors Smoking. Alcohol abuse. Chronic vocal-cord inflammation from any cause.     What is Larynx Cancer The larynx is the voice box. It is made up of the glottis, which is the vocal cords, the area below (subglottis) and the area above (supraglottis) the vocal cords. Any of the cells lining the larynx can become cancerous. How common is Larynx Cancer In men, the number of new cases of larynx cancer diagnosed in the last 5 years has been decreasing compared to the time before. It is six times more common in men than women. Risks and Causes Smoking is one of the commonest risk factors associated with larynx cancer. Alcohol consumption is also a risk factor. Smokers who also drink alcohol have a much higher risk of developing larynx cancer than persons who only smoke or drink alcohol. Symptoms and Signs of Larynx Cancer A person should seek early medical attention if he has a persistently hoarse voice or a persistent sore throat, a painless lump in the neck, pain on swallowing, difficulty swallowing, or noisy breathing known as stridor. As cancer can spread to other organs in the body, in the very late stages there may be symptoms from the lungs or bone. Diagnostic Tests If larynx cancer is suspected, the doctor will examine for swellings or lumps in the neck. An angled mirror that faces downwards is placed against the back of the throat (the palate) to examine the voice box and surrounding organs. Or a flexible tube, two millimetres in diameter, is passed into one of the nostrils to the back of the throat to look for lumps or ulcers in the voice box and surrounding organs. Small pieces of tissue will be removed (biopsy) during this examination to confirm if there is cancer. This examination is performed under anaesthesia. If there is a neck lump, small pieces of tissue may also be removed using a needle and syringe. These procedures, although uncomfortable, usually last about 10 to 15 minutes.
