Infection with the ulcer-causing bacterium Helicobacter pylori may increase a person's risk of developing larynx cancer. Approximately two thirds of the world's population is infected with H. pylori. In the United States, H. pylori is more prevalent among older adults, African Americans, Hispanics and people of lower socioeconomic status, according to the Centers for Disease Control and Prevention (CDC). In the study researchers checked for H. pylori infection in 26 patients diagnosed with larynx cancer and 32 cancer-free patients. About 73% of patients with larynx cancer had H. pylori infection, the investigators found, while 41% of those without cancer were infected with the bacterium. This study suggests that H. pylori may be an initiator or promoter organism of larynx cancer. H. pylori could make the cells lining the larynx more susceptible to cancerous changes when they are exposed to substances known to promote cancer, such as tobacco and alcohol. According to the CDC, recent studies have shown an association between long-term H. pylori infection and the development of gastric, or stomach, cancer.

Variations of polychemotherapy (PCT) and hyperfractionated irradiation for (HFI) locally advanced larynx cancer (T3N0M0)
Department of Upper Respiratory and Digestive Tract Tumours, Blohin's Cancer Research Center, RAMS, Moscow, Russia
Purpose: To study the expediency of variations of the PCT and HFI for larynx cancer T3N0M0.
Methods: 72 patients are included in research with locally advanced larynx cancer T3N0M0. There were 3 groups:
  • 2) 29 pts: two cycle PCT Fu+Blm+DDP, 1 cycle before HFI, 2 nd - after 1 stage of irradiation.
  • 3) 22 pts: two cycles of PCT - 5-Fu, DDP, leucovorin 200 mg i.v. in 2 hours continuous infusion followed HFI.

Acute mycositis grade III, leucopenia and diarrhea were in all groups.
Results: PR 84% (1 st group); 86% (2 nd group); 92% (3 rd group). CR 80% (1gr), 89% (2gr), 100% (3gr). Total five years survival rate: 76.4% (1gr), 84% (2gr), 90% (3gr). Conclusion: We confirm the expediency of PCT-HFI larynx cancer as organ-preservation treatment. The received data allow to conclude that the realisation of two cycles of chemotherapy is necessary. Two cycles of PCT with DDP, 5-Fu and leucovorin to effort citostatic effect of 5-Fu on the first stage HFI is the most perspective.

Screening and Prevention
Cigarette smoking is the most significant risk factor for many caners of the head and neck. The best way to reduce your risk of these cancers is not to smoke cigarettes. If you don't smoke now, don't ever start. If you do smoke, NYU offers smoking cessation programs to help you to stop. Tim Heidler received the laryngeal graft in 1998. Surgeons at the Cleveland Clinic Foundation used a mosaic of the larynx, pharynx and trachea from a man who had died of a ruptured blood vessel in his brain. Heidler's voice box and pharynx were crushed in a motorcycle accident in the late 1970s, leaving him unable to speak. He was 40 at the time of the transplant. Three days after the operation, Heidler uttered his first word, "Hello," followed a short time later with a "Hi, Mom." Three years after the operation his voice has a nearly normal quality, allowing him to go back to work as a motivational speaker.     Somerset is located in south central Kentucky, in the hills of Appalachia. Since my practice started in Somerset, I have evaluated 61 patients with invasive cancer (squamous cell carcinoma) of the voice box (larynx). 59 patients smoked. 58 patients had over a 20 pack year history of smoking (smoking 1/2 pack per day for 40 years). Two patients had severe GE Reflux and in one patient the cause of the cancer was unknown. The average age was 64 years. The average length of smoking was 58 pack years. (A pack year is one pack per day smoked in one year, or one-half pack per day smoked over two years.) 14 patients had quit smoking. Four patients had quit more than 25 years prior to developing the cancer. Exposure to the carcinogens in tobacco can cause permanent genetic damage in the cells and tumors can develop years later. Fifteen of these patients were treated with a total laryngectomy, or had to have their voice box removed.

Smoking dramatically increases the risk of throat cancer. FIND IT EARLY AND BE CURED! Cancer of the head and neck is curable if caught early. Fortunately, most head and neck cancers produce early symptoms. You should know the warning signs so you can discuss your symptoms with your doctor as soon as possible. LOOK FOR THE FOLLOWING:
    • A LUMP IN THE NECK... Cancers that begin in the head or neck usually spread to lymph nodes in the neck before they spread elsewhere. If you have a lump in your neck lasting more than two weeks, you should see your physician as soon as possible.
    • A CHANGE IN THE VOICE... Most cancer in the larynx (voice box) causes some change in your voice. Any hoarseness or other voice change lasting more than two weeks should alert you to see your physician.
    • A GROWTH IN THE MOUTH... Most cancers of the mouth or tongue cause a sore or swelling that doesn't go away. These may be painless, unless they become infected. Bleeding may occur, but often not until late in the disease. If an ulcer or swelling is accompanied by lumps in the neck, be very concerned.
