Patients of Mayer Rokitansky Kuster-Hauser syndrome suffer from Mullerian agenesis and absent vagina. Several modes of creation of a vagina exist. With increasing laparoscopic skills and as all procedures are being attempted endoscopically, we modified Vecchietti’s method of creating Neo-Vagina Laparoscopically. We did not have exact bead and traction devises used by Vecchietti’s. We, in fact, used our own innovations to get excellent results, according to the infrastructural facilities we have.

Findings:
The uterus and posterior vagina are filled with echogenic fluid and clot. Diagnosis: Hematometrocolpos secondary to imperforate hymen. Discussion: Obstructed ureterovaginal anomalies may present in the neonate, however the majority present with the accumulation of menstrual blood at menarche. These anomalies are classified into three groups:
  • Agenesis of the uterus and most/all of the vagina (lower 1/3 usually present because it is derived from the urogenital sinus), known as Mayer-Rokitansky-Kuster-Hauser Syndrome (MRKHS). Hematometros (blood within the endometrial cavity) or hematometrocolpos (blood within the vagina and endometrial cavity) results if a uterine anlage (Müllerian duct remnant) is present.
  • The descending Müllerian ducts can fail to fuse with the ascending urogenital sinus, resulting in hematometrocolpos from a vaginal septum. Hematometra can occur with an imperforate cervix, cervical agenesis, or vaginal agenesis with a normal uterus. A transverse vaginal septum is most common cause and is usually located at the junction of upper/middle third of the vagina.
  • Disorders of lateral fusion result in duplication anomalies with vaginal obstruction, most commonly occurring with a uterus didelphys. Unilateral obstruction can be difficult diagnosis because there is "normal menses" flow from the unobstructed side. Signs and symptoms include a bulging hymen, an intralabial mass (low vaginal obstruction), an abdominal mass, cyclic abdominal pain, primary amenorrhea, voiding dysfunction, or ambiguous genitalia.

A la exploración se descubre una tumoración en ovario izquierdo que se extirpa. Macroscópicamente se trataba masa tumoral de 9,5 cm de diámetro mayor, sólida, lobulada y de coloración amarillenta. Se tomaron muestras para su estudio histopatológico. Imágenes 892, 893 y 897 H-E. Imágenes 894 a 896 y 898 Grimelius. DISCUSIÓN El caso presentado aquí se trata de un Tumor de células de Sertoli
    Éstos tumores representan el 4% entre los tumores de ovario del grupo de Tumores de células de Sertoli-Estroma. Hallazgos Microscópicos de los TCS : Histológicamente el patrón característico es la identificación de túbulos sólidos, generalmente dispersos, en un estroma fibroso en el que no hay, o se encuentran muy pocas, células de Leydig. Cuando las células de Sertoli tienen un citoplasma abundante lipídico se les puede llamar Tumores de Células de Sertoli Ricos es Lípidos . Ocasionalmente algunas células de Sertoli tienen citoplasma eosinófilo. Generalmente no se observan atipias ni actividad mitósica incrementada, y el pronóstico es excelente.

    The Absent Vagina Being born without a vagina is very distressing, and it is almost always associated with an absence of the uterus. The ovaries are normally present and function as anybody else's ovaries would do, producing eggs and female hormones which keep you healthy. You are of course entirely female. This is known as the Mayer Rokitansky Kuster Hauser Syndrome often shortened to Rokitansky Syndrome. How will I know if I have this problem? You discover that you have no vagina when you fail to start having periods at the age that you would expect. You develop breasts and pubic hair like any other woman, but you will not have a period. This is because your ovaries produce the normal female hormones which make normal development occur, but the absence of the uterus and vagina means there is no period. Most girls report this at the ages of 15 or 16 years. Alternatively, some may have difficulties with sexual intercourse as their first problem. What investigations or tests will I have?
