Hemolytic anemias generally present with moderate anemia. However they can present with severe aneemia due to megaloblastic crisis or aplastic crisis, thus masking the underlying hemolytic anemia. Here we report two cases of hemolytic anemia with megaloblastic crisis, one a case of auto immune hemolytic anemia in a young boy of 14, and the other thalassemia intermeida in a 48 year old male. CASE REPORT Case 1: A 14 old boy presented to our hospital with history of weakness of 2 month duration, fever of 12 days duration, headache, vomiting and restlessness of 2 days duration. He was provisionally treated for malaria in a local hospital. On examination, he was severely anemic, mildly icteric and had moderate hepatosplenomegaly. He was irritable and restles,s had no signs of meningeal irritation. His Hb% was 3.6G%. Total WBC count was 19,400 cells/cmm. Differential count showed neutrophils 76%, Eosinophils 1% lymphocytes 23%. Peripheral smear showed normocytic picture with few macrocytes, polychromasia and occasional nucleated RBCs. His reticulocyte count was 20%. Bonemarrow aspiration revealed numerous megaloblasts suggestive of megaloblastic anemia.

CAUTION: It is always important to discover the underlying deficiency that is causing a megaloblastic anemia. Higher doses of folic acid (greater than 1 mg per day) may improve or mask the anemia caused by a insufficiency. Unless the B12 deficiency is corrected, permanent nervous system damage may result. In other words, don’t treat a B12 deficiency with high doses of folic acid!
Pernicious Anemia
People with pernicious anemia loose their ability to make intrinsic factor (IF), a substance that enables vitamin B12 to be absorbed from the intestine. The result is vitamin B12 deficiency, which gradually affects sensory and motor nerves, causing neurological, gastrointestinal and cardiovascular problems. The disease can affect all racial groups, but the incidence is higher among people of Scandinavian or Northern European descent.

Megaloblastic (Greek: megalo=giant; blast=germ cell) anemias are those anemias resulting from pathologic slowing, by any cause, of the doubling of the nuclear DNA necessary for bone marrow nucleated cells to divide, with only minor slowing of the synthesis of cytoplasmic RNA. This nuclear-cytoplasmic dissociation becomes greater with each daughter germ cell division, until either the cells die or omit terminal division, surviving in the marrow as end stage giant red cells, white cells, and platelets. These are extruded into the peripheral blood as macroovalocytes, hypersegmented neutrophils, and giant platelets, all with a shortened life span, hence the pancytopenia (anemia, leucopenia, and thrombopenia), characteristic of full-blown megaloblastic anemia.
The most common causes of megaloblastic anemia are deficiencies of vitamin B (cobalamin) and/or folate (folic acid). As with any nutrient deficiency, these two B-vitamin deficiencies may arise from any of 3 inadequacies (inadequate ingestion, absorption, or utilization), or any of 3 excesses (excessive requirement, destruction, or excretion).

Causes and Risks:
Deficiencies of vitamin B-12 and folic acid are the most common causes of megaloblastic anemia. Other causes are leukemia myelofibrosis multiple myeloma , certain hereditary disorders, drugs that affect nucleic acid metabolism such as chemotherapy agents (methotrexate), and other causes. Anemia
The Hct in women who are not pregnant is normally 38 to 45%, but in pregnant women, normal values can be much lower (eg, 34% in a single and 30% in a multiple pregnancy), even when stores of iron, folic acid, and vitamin B are adequate. This lower range represents the physiologic hydremia of pregnancy and does not indicate a decrease in O -carrying capacity or true anemia. Normally, a pregnant patient has erythroid hyperplasia of the marrow and a measurable increase in red cell mass; a disproportionate rise in plasma volume results in hydremia. Unless excessive blood loss occurs at delivery, the Hct generally rises in the immediate postpartum period. Absence of the dilutional effect in late pregnancy suggests inadequate blood volume expansion, which has been associated with growth retardation, pregnancy-induced hypertension, and fetal death. Iron deficiency is responsible for 95% of cases of anemia during pregnancy. The deficiency is usually due to inadequate dietary intake (especially in teenage girls), to a previous pregnancy, or to the normal loss of iron in blood with menses (which approximates the amount normally ingested each month, so that iron stores are never built up).

Background
Risk Factors

Younger women who have moderate to heavy menstrual flows are more likely to develop anemia. Use of prophylactic devices such as the intrauterine device (IUD) may lead to increased loss of blood (and thus iron) during menstruation. During pregnancy, women are at much higher risk of developing anemia since her body's iron supply is compromised and her body must supply enough oxygen for itself and for the fetus. Deficiencies in certain vitamins (B12) may lead to an impaired ability to produce RBCs. Demographically, African-American and other minority women seem to have a much great chance of developing anemia than do their Caucasian counterparts. The reasons for this disparity are unknown. Finally, young women who suffer from the eating disorder anorexia have a significantly great risk of developing anemia than other women.
Women who suffer from mild cases of this condition often do not experience any serious symptoms. More advanced stages of anemia produce fatigue, lightheadedness, paleness, sore tongue, headaches, and even unusual cravings. Weight loss, jaundice, constipation and diarrhea, and abdominal cramps may follow the initial symptoms if the condition is left untreated. In severe cases tachycardia (increased heart rate) and/or hypotension (low blood pressure) may develop.
Diagnosis
When a physician suspects that a woman has developed anemia, the medical practitioner will order a complete blood count (CBC) in order to determine the exact amount of red blood cells (hemacrit level) are in the body. An idea about the shape and appearance the red blood cells also helps in the diagnosis. A complete medical history will also assist in the diagnosis. Tests are also performed to determine the underlying cause of iron deficiency. A low mean corpuscular volume (MCV) obtained from the CBC may suggest iron deficiency (MCV less than 79). An elevated MCV greater than 95-100 may suggest megaloblastic anemia. In cases of megaloblastic anemia it is important to establish whether intrinsic factor is present in the stomach. Without the intrinsic factor B-12 can not be absorbed by the small intestine.
