SKELETAL DYSPLASIAS
Osteogenesis imperfecta
  • diffuse cancellous osteopenia (coarse primary trabeculae)
  • diminished tubular bone girth
Osteopoikilosis
  • numerous small, well-defined homogeneous circular foci of increased density (bone islands)
  • cortical-like densities in cancellous bone
  • periarticular, symmetrical, at ends of long bones
  • also found in tarsal bones
Osteopetrosis
  • diffuse bone sclerosis
  • "bone within bone" appearance
  • aka "marble bone disease"
Osteochondromatosis
  • aka multiple cartilaginous exostoses
  • multiple exostoses near diaphyseal side of physeal line (in child)
  • appear in first two decades of life as painless bumps near ends of long bones
  • causes bone deformity and shortening
  • 5% malignant transformation
Melorheostosis
  • radiographic presentation limited to a single limb
  • hyperostosis is seen along bone periphery, extending along length of bone
  • simulates candle wax flowing down side of lit candle
  • wavy, sclerotic bony contour (endosteal usually)

Later, osteosclerotic lesions occur and bone becomes chaotic and thickened, with bowing and fractures Biochemically, there is an elevated serum alkaline phosphatase (from osteoblast membranes) which correlates to the degree of skeletal involvement, as does the 24 hour urinary excretion of pyridinoline cross-links (an index of bone matrix collagen resorption). Hypercalciuria and hypercalcaemia are rare due to tight metabolic coupling of bone resorption and formation

Sclerosing Bone Disorders Sclerosing bone disorders (Osteopathia striata, osteopoikilosis, and meloreostosis, Leri's disease, flowing periosteal hyperostosis)
Osteopathia striata (Voorhoeve's disease) is a benign, usually painless disorder characterized by longitudinal dense striations in affected bones. In meloreostosis, there is thickening of the endosteal and periosteal bone that has been likened to "candle dripping". Multiple punctate or rounded small bone islands characterize Osteopoikilosis. Mixed sclerosing bone disorder refers to patients who have manifestations of two or three of these patterns together.
Meloreostosis, Leri's disease, flowing periosteal hyperostosis

Other rare hereditary and non-hereditary bone disorders marked by sclerosis or longitudinal dense striations in affected bones have been described that may have radiological similarities to the disorders described above.
