Previous Index Do Menstrual Problems Increase After Tubal Ligation? Tubal ligation is the most common method of contraception used in the United States. It is considered safe and highly effective. However, since the early 1950s, the existence of a "post-tubal ligation syndrome" has been debated. The hallmark of this syndrome is an increased risk of menstrual abnormalities. Moreover, results from one study indicated that women who have this surgery were four to five times more likely to have a subsequent hysterectomy; results from a second study showed that 41 percent of the women who underwent hysterectomy had a previous tubal sterilization. To make further determinations about this issue, Peterson and colleagues reported data from the U.S. Collaborative Review of Sterilization, a large prospective study evaluating the long-term safety and effectiveness of tubal ligation. The U.S. Collaborative Review was started in 1978 and included women between 18 and 44 years of age who were undergoing tubal sterilization at one of several institutions throughout the country. A second group of women whose partners had undergone vasectomy was added to the study later. All women were followed by annual telephone interviews for five to 14 years and were asked to describe their three most recent menstrual periods. Six features of periods that were addressed were duration of bleeding, length of cycle, bleeding or spotting between cycles, cycle irregularity, amount of menstrual pain and amount of bleeding. The findings from the tubal sterilization group were compared with findings from the group whose partners had undergone vasectomy. The primary outcomes of interest were changes in various features of the menstrual cycle.

Also, women have reported passing clots or menstrual blood that is darker than usual. The most common menstrual problem reported by HIV-positive women is the loss of their periods altogether. This condition is called Amenorrhea. Women who miss their periods experience pelvic pain, swollen breasts or hot flashes. Heavy periods can result in excessive blood loss, which could lead to low red blood cells or anemia. Anemia can make you feel tired, weak and short of breath. Since HIV infection itself as well as many of the medications used to treat it can also cause anemia, it is important to carefully look for this problem. Does HIV disease cause menstrual problems?
It is not known for sure. The female reproductive system and the menstrual cycle are very complicated. It is known that female hormones (estrogen and progesterone) can communicate with parts of the immune system. It is also known that the male hormone testosterone may be lower than normal in HIV-positive men. So it is logical to guess that problems in the immune system may cause changes in female hormones that could result in menstrual irregularities. Studies of testosterone levels have been conducted in HIV-positive men. Men with low levels of testosterone often have muscle loss, weight loss, and decreased sexual drive. Testosterone replacement is available in shots or by patch. Men with low testosterone often gain weight, regain sexual drive and feel better with therapy. On the other hand, female hormone function has not been adequately studied in HIV-positive women.

  • Menstruation, or periods, usually begin around puberty, between the ages of 9 and 14. The average menstrual cycle lasts 23-35 days (counting from the first day of one period to the next). Some women may not have a regular cycle (number of days between periods). This is normal, especially in the first 2 years after a woman's first period. Most women have their period for about 6 days. Some may have it fewer than 6 days and some may have it for more than 6 days. This is normal. Some women may have a large amount of blood flow while others have little. Blood flow can vary. It is common to have heavy blood flow at the beginning of a period and lighter blood flow at the end. Blood flow may be heavy one period and light the next.
