Background: No treatment is currently available for intestinal microsporidiosis due to Enterocytozoon bieneusi , a cause of chronic diarrhea, malabsorption, and wasting in immunocompromised patients. Methods: We conducted a randomized, double-blind, placebo-controlled trial of fumagillin (60 mg/day orally for 2 weeks) in patients with chronic E. bieneusi infection. Efficacy was assessed primarily by the clearance of microsporidia from stools. Patients who did not clear microsporidia from stools received 2 weeks of open-labeled fumagillin. Follow-up stool examinations were performed monthly in patients with parasitic clearance to detect a relapse. Results: 12 patients were enrolled in this study, 10 had acquired immunodeficiency syndrome (AIDS), and 2 were organ transplant recipients. Clearance of microsporidia from stools was obtained in 6 of 6 patients in the fumagillin group vs of 6 in the placebo group (p=0.002). Treatment with fumagillin was also associated with significant increases in D-xylosemia (p=0.003) and Karnofsky scores (p

Fumagillin Effective for Microsporidiosis Treatment
The fungal antibiotic fumagillin can resolve intractable intestinal infections in HIV patients and others, according to researchers in France. "Intestinal microsporidiosis due to Enterocytozoon bieneusi is a cause of chronic diarrhea, malabsorption, and wasting in immunocompromised patients," explained Jean-Michel Molina and colleagues at Saint-Louis Hospital, Public Assistance Hospitals of Paris, and the Pitie- Salpetriere Hospital Group in Paris. "Currently, there is no effective treatment." Fumagillin is the first effective therapy for patients suffering from chronic intestinal E. bieneusi infection, Molina and coauthors found. The researchers assessed the performance of the fungal antibiotic in a group of 10 AIDS patients and two organ transplant recipients. All six patients treated with fumagillin achieved E. bieneusi clearance, which was not seen in any of the six placebo-treated patients, they said. Patients who received fumagillin therapy enjoyed significant improvements in Karnofsky performance status and intestinal nutrient absorption, and were able to reduce their intake of the antidiarrehea drug loperamide, study data showed. None of these benefits was seen in control patients until they were switched to open-label fumagillin treatment.

Round Table 6 - Protozoology MICROSPORIDIA AND HUMAN MICROSPORIDIOSIS and Prevention, Atlanta, Ga., USA Microsporidia is the term used to refer to emerging unicellular eukaryotic parasites that are obligatorily intracellular and belong to the order Microsporida of the phylum Microspora. Mcrosporidia infect practically all groups of animais, including humans and other protozoans. More than 100 genera with over 1,000 species of núcrosporidia have been described. To date, seven species, classified to six genera (Enc~tozoon, Enterocytozoon, Pleistophora, Trachipleistophora , Nosema and Vittaforma) have been identified as human pathogens (l6). Nficrosporidia are increasingly recognized as important opportunistic pathogens predominantly, but not exclusively, in persons with AIDS and cause gastrointestinal (GI) as well as disseminated microsporidiosis (DM). GI disease accounts for 30% of diarrhea in patients with AIDS. Of the several microsporidia that infect humans only Enterocytozoon bieneusi is known to cause strictly GI disease, whereas

Fumagillin, a drug once used to treat malaria, is an effective treatment for intestinal microsporidiosis caused by Enterocytozoon bieneusi, according to a report published in the June 20th issue of The New England Journal of Medicine
The intestinal disorder is known to cause malabsorption, chronic diarrhea, and weight loss in immunocompromised patients. While the diagnosis is easily made with stool analysis, no treatments that effectively eradicate E. bieneusi had been identified.
Ten patients had AIDS and two had undergone organ transplantation. The patients were randomized to receive fumagillin 60 mg or placebo daily for 2 weeks.
After the randomized phase, fumagillin was offered to patients in the placebo group. All of these patients cleared E. bieneusi from their stool.
While three patients in the fumagillin group demonstrated laboratory abnormalities, none required hospital admission. In contrast, one patient in the placebo group required hospitalization for severe diarrhea and weight loss. Several minor adverse events occurred, but it was unclear whether these were due to the fumagillin therapy or to underlying conditions.
