"Microsporidiosis a Major Cause of HIV-Related Diarrhea in Germany" "Microsporidiosis a Major Cause of HIV-Related Diarrhea in Germany"
Reuters Health Information Services (02/23/98) According to a report by Dr. David A. Schwartz of Grady Memorial Hospital in Atlanta and colleagues, microsporidosis constitutes a major cause of diarrhea in HIV-infected patients in Germany. The study, which appeared in the February issue of Clinical Infectious Diseases, examined 97 German HIV-positive patients with diarrhea. The data showed Germany to have one of the highest reported rates for intestinal microsporidosis in the world. Microsporidia were the most common enteropathogens detected in 36 percent of 50 individuals with diarrhea and some 4 percent of 47 patients without diarrhea.

M: MAC through Myopathy
MAC (Mycobacterium Avium Complex)
A serious opportunistic infection caused by two similar bacteria (Mycobacterium avium and Mycobacterium intercellulare) found in the soil and dust particles. In AIDS, MAC can spread through the bloodstream to infect lymph nodes, bone marrow, liver, spleen, spinal fluid, lungs and intestinal tract. Typical symptoms of MAC include night sweats, weight loss, fever, fatigue, diarrhea and enlarged spleen. MAC is usually found in people with CD4 counts below 100. Clarithromycin, azithromycin, ethambutal, rifampin, clofazimine and rifabutin are some of the antibiotics commonly used in MAC prevention and treatment. (Treatment of active infection usually involves combination therapy.) MAC is also called MAI.
Macrophage
A large scavenger cell that ingests degenerated cells and foreign organisms. Macrophages exist in large numbers throughout the body and contribute to the development of acquired immunity by acting as antigen presenting cells . They also ingest and destroy foreign matter coated with antibody. Macrophages can be infected by HIV.
Macula
The pigmented central area or "yellow spot" of the retina that is adjacent to the optic nerve. It is the most sensitive area of the retina and contains the fovea, a depressed region that is responsible for detailed central vision.

AIDS - Parasitic Infections A person suffering from AIDS is susceptible to parasitic infections. Some of the common infections are listed as follows.
Cryptosporidiosis
Cryptosporidiosis (crypto) is a disease of the intestine (gut) which is caused by a parasite. Many people are exposed to crypto, but people with healthy immune systems usually get rid of the parasite without getting any symptoms or illness. Your doctor can give you a test to find out if you have this disease by examining your feces (shit). There is another illness that people with AIDS get called cryptoccocal meningitis that is completely different from cryptosporidiosis.
Symptoms
People with cryptosporidiosis usually have watery diarrhea more than 4 to 6 times a day. Weight loss, nausea, vomiting, cramping, stomach pain, and fever are also common symptoms. These symptoms can develop slowly over time. If you've had diarrhea for a long time, you may be losing important vitamins, minerals, and fluids which you need to live. If you have chronic diarrhea or any of the other signs, you should see your doctor right away.

AIDS 1997 Nov;11(13):1589-94 OBJECTIVE : To determine the prevalence of microsporidiosis in HIV-infected patients with and without diarrhoea and to characterize alterations in mucosal architecture and brush border enzyme activities in patients with microsporidiosis. PATIENTS: A total of 259 HIV-infected patients undergoing oesophago-gastroduodenoscopy because of diarrhoea (n = 123) or other symptoms (n = 136) were studied. METHODS : Patients were evaluated for the presence of microsporidia by electron microscopy of duodenal biopsies. Brush border enzyme activities were measured by histochemistry and mucosal architecture was determined by three-dimensional morphometry in biopsies from patients with microsporidiosis and compared with biopsies from a subgroup of HIV-infected patients with or without other enteropathogens.

A. Maryland Department of Health and Mental Hygiene, Epidemiology and Disease Control Program, Carmela Groves: . Two outbreaks were reported between Apr. 19 - Apr. 25, 1996. These include:
  • Two separate outbreaks of presumed viral gastroenteritis in nursing homes.
Influenza Update: No cases added since last report.
B. The Johns Hopkins Hospital (Information provided by Dr. Kim M. Ruska, Pathology Resident) Clinical Information
This patient was a 37 year-old man who was found to be HIV-positive during blood donation in 1985. He has been on several different antiviral regimens since 1992, most recently AZT/3TC, but his CD4 count continued to decline, dropping below 200 in May 1994. He has remained relatively asymptomatic, with the exception of some dermatologic problems, thought to be related to his obesity, and one episode of prolonged diarrhea. A biopsy of a rectal nodule at colonoscopy in 1994 revealed a lymphoid nodule, and upper endoscopy showed mild gastritis. The patient presented to the Moore Clinic on 3/25/96 with complaints of the recent onset of diarrhea. White blood cell count on that day was 4300 with a differential of 31% PMNs, 55% lymphs, 12% monos and 2% eosinophils. CD4 count was 123. A stool specimen was positive for
