    MILLER-FISHER SYNDROME
    DEFINITION:
    A benign variant of Guillain-Barre syndrome characterized by ataxia, areflexia, and ophthalmoplegia.
    EPIDEMIOLOGY:
    • incidence: rare
    • age of onset:
      • young children
    • risk factors:
      • viral infections
        • viral prodrome 5-10 days prior to onset of acute ataxia in 50% of cases
        CLINICAL FEATURES:*
        1. Neurological Manifestations
        • truncal ataxia (limbs may also be affected)
        • areflexia
        • motor weakness
          • limbs
          • unilateral or bilateral facial weakness
          2. Ocular Manifestations
        • 1. Ophthalmoplegia
          • horizontal gaze is unaffected
          • dissociated nystagmus - most marked in abducting eye
          • ptosis
        • *Note: While Guillain-Barre syndrome affects only the peripheral nervous system (PNS), Miller-Fisher syndrome is thought to affect both the PNS and CNS.
          INVESTIGATIONS:

            MANAGEMENT:
            1. Supportive

Miller Fisher syndrome is a rare, acute polyneuropathy characterized by ataxia (abnormal muscle coordination), ophthalmoplegia (paralysis of the eye muscles), and areflexia (absence of the reflexes). The disorder is a variant of Guillain-Barre syndrome. Symptoms may be preceded by a viral illness and include generalized muscle weakness and respiratory failure as in Guillain-Barre syndrome. The majority of individuals with Miller Fisher syndrome have a unique antibody that characterizes the disorder. Is there any treatment?

    Associated persons: Charles Miller Fisher Description: A disturbance thought to be a variant of the Guillain-Barré syndrome usually characterised by total external ophthalmoplegia, ataxia, and loss of tendon reflexes. Early symptoms include fever, headache, and pneumonia. They are followed by facial paralysis, diplopia, external ophthalmoplegia, and paresthesia of the arms and trunk. Reported in male patients between 38 and 65 years of age. Complete recovery is usual. Some authors have named the Bickerstaff’s syndrome as a synonym for Miller Fisher syndrome, but that is a misnomer. Bickerstaff’s syndrome is a brain stem encephalitis. See under E. R.     Guillain-Barre Syndrome And The Miller Fisher Variant
    Christopher P. Sobczak, M.D., Safwan Jaradeh, M.D. Medical College of Wisconsin, Milwaukee, WI Discussion: Miller Fisher syndrome is a variant of Guillain-Barre Syndrome involving the triad: ataxia, ophthalmoplegia, and areflexia. The pathophysiology of the syndrome can be explained through GQ1b ganglioside autoantibodies, which are seen in 96 percent of cases of Miller Fisher Syndrome. These autoantibodies recognize epitopes expressed specifically in the nodal regions of oculomotor nerves and cerebellar neurons. Organisms such as Mycoplasma Pneumoniae and Campylobacter jejuni contain cell walls that are capable of inducing the production of these antoantibodies. Treatment is thus involved in immune modulation, through the use of IVIg, plasma exchange, and steroids. Our patient required intubation and wan treated with all three modalities over a three-month hospitalization. He eventually recovered to be discharged to long term rehabilitation.     Miller Fisher Syndrome (MFS) Miller Fisher Syndrome is a variant form of Guillain Barre Syndrome (GBS). GBS is more common than MFS but is still relatively unusual but becoming more common than it used to be. They are both diseases in which the bodies central nervous system is attacked by the bodies own auto-immune system, which suddenly decides that the myelin sheath that acts both as a nerve insulator and fast impulse carrier is a foreign body and attacks it, causing inflammation and destruction. The difference between MFS and GBS is that different nerve groups are affected such that paralysis in GBS tends to start in the legs and move up, where in MFS it starts in the head, affecting eye muscles, balance and slowly descends to the neck arms etc. MFS does not generally have the life threatening aspects of GBS but can be very difficult to live through with double vision, nausea, weakness, difficulty walking, light sensitivity and pain which can go on for months. The cause is still unknown but about two-thirds of cases occur after a viral infection, either one you know about such as a sore throat, diarrhea, flu or cold, or one that is subsequently confirmed by blood tests. The viral infection may be one of the herpes viruses, such as the Epstein-Barr virus. But GBS may also follow a bacterial infection. Guillain-Barré is also sometimes associated with medical procedures. Five percent to 10 percent of cases occur after an operation. Age is not a factor, people of any age can get it and it can reoccur in a small percentage of cases.

    Orthoptists are often confronted with patients with diffuse ophthalmoplegia of apparent supranuclear origin who have normal neuroimaging studies. Disorders like myasthenia gravis, vasculopathic, inflammatory and traumatic cranial nerve palsies are frequently encountered. A rare disorder for which the orthoptist should be familiar is called the Miller Fisher syndrome. In 1956 C. Miller Fisher described three patients with diffuse ophthalmoplegia, ataxia and loss of tendon reflexes. He theorized that these patients had an unusual variant of the Guillian Barre syndrome. In order to fully appreciate the Miller Fisher syndrome, a review of Guillian Barre syndrome is in order. © 2001 The Board of Regents of the University of Wisconsin System

    ABSTRACT The Miller Fisher syndrome consist of the following signs: arreflexia, ataxia and ophtalmoplegia and it is included among the "demiakinating" syndromes. Initially it was thought to be a variant of the Guillain Barré syndrome. In 1956, it was considered a different entity that involves the CNS. We describe a patient that was admitted to the ABC Hospital after developing an acute bilateral eye lid ptosis, blurred speach and ubable to swallow solid foodd; within the next 24 hours he developed quadriplegia and required assisted ventilation. The patient was treated with plasmapheresis and his respiratory status did not deteriorate. We include a pertinent review of the literature. Key words: The Miller Fisher syndrome, differential diagnostic, plasmapheresis treatment, evolution.

    Summary : Miller fisher syndrome (MFS) is a variant of Guillain-Barre syndrome characterized by the triad of ophthalmoplegia, ataxia and areflexia. Recurrences are exceptional with MFS. A case with two episodes of MFS within four years is reported. He presented with findings of ophthalmoplegia, ataxia, areflexia, and oropharyngeal weakness and mild distal sensory impairment during both episodes. Electrophysiological findings showed reduced compound muscle action potentials and sensory nerve action potentials with no evidence of conduction blocks. Nerve biopsy showed segmental demyelination. MRI of brain was normal. He responded well to immunoglobulins during both episodes suggesting that immunomodulating drugs have a role in the treatment of MFS.
