Material and Methods
Study Population The study population consisted of consecutive patients with SCD and those with chronic anemia (other than SCD) who were either admitted to King Fahd Hospital of the University or attending the outpatient department. Control subjects were volunteers from hospital employees, blood donors and medical students. They all had their heights and weights recorded. Informed consent was obtained from all subjects who participated in the study after approval from the research committee of the College of Medicine and Medical Science, King Faisal University. Hematological Investigations All subjects had a complete blood count (CBC), hemoglobin electrophoresis (including quantitation of HbF and HbA2), determination of serum iron, serum ferritin and total iron binding capacity.

Mitral insufficiency is a leaking heart valve
Anatomy of the heart of a dog Many dogs slowly develop degenerative thickening and progressive deformity of one or more heart valves as they age. In time, these changes cause the valve to leak. The mitral valve is most commonly affected. This valve separates the blood collecting chamber (left atrium) from the pumping chamber (left ventricle) leading to the body. Some dogs also develop these changes in the tricuspid valve, which separates the collecting (right atrium) and pumping (right ventricle) chamber leading to the lungs. Certain breeds have an inherited predisposition to this disease. Degenerative valvular disease is slowly progressive over years and is non-reversible. The volume of blood that leaks back into the atrium with each heartbeat tends to increase slowly over time. However, many dogs with this disease never develop signs of congestive heart failure even though they may have a loud murmur. Dogs with mitral insufficiency may not show signs or may develop mild to severe heart failure Reduced exercise ability may be the first sign of heart failure.

Causes and Risks Mitral valve prolapse (MVP) is one cause of mitral regurgitation (leakage of blood from insufficient valve closure). It occurs in about .3% to 5% of the adult US population; women are affected twice as commonly as males.
Mitral valve prolapse is the most common cause of mitral regurgitation requiring surgical treatment in the US. Some forms of MVP seem to be hereditary. MVP has been associated with Marfan's syndrome characteristics.
Most individuals with mitral valve prolapse are thin women who may have minor chest wall deformities, scoliosis , or other disorders. There may also be an

Heart Valve Disease: Keeping Blood on Course
Back Next Last spring, Sudbury pediatrician Michael Robinson, MD, noticed he was getting short of breath when he exercised. For Robinson, 62, who swims laps six mornings a week to keep fit, that was unusual. But he wasn't concerned until he fainted after sprinting up the stairs in his house. At that point Robinson knew something was definitely awry. "I'd known for years that I was born with a defect in one of my heart valves, and was aware it might someday cause problems," he says. "So I made an appointment right away with a cardiologist to get it checked out." Robinson went to William H. Gaasch, MD, a widely recognized expert in heart valve disease and interim chair of cardiology at Lahey Clinic Medical Center. Following a physical examination and diagnostic tests, Gaasch determined that Robinson had a condition called aortic stenosis. Stenosis is said to be present when a heart valve - in this case the aortic valve - becomes narrowed, or "stenotic," due to abnormalities present since birth or degeneration caused by aging. Less commonly it occurs as a result of rheumatic fever.

Malfunction of one of the four valves within the heart can cause problems that affect the entire circulatory system. A leaky valve does not close all the way, allowing some blood to flow backward as the heart contracts. This backward flow decreases the amount of oxygen the heart can deliver to the tissues with each beat. Symptoms of heart valve disease include shortness of breath, swelling in the legs, chest pain and heart palpitations. Phen Phen Background During the period from March to August 1997, the Mayo Clinic in Rochester, Minnesota observed and reported an association between the use of fenfluramine and/or dexfenfluramine and valvular heart disease ("VHD"). On September 15, 1997, AHP and the FDA announced that there would be no further sales of Pondimin and Redux in the United States. Since the withdrawal. epidemiological studies have established a causal relationship between fenfluramine and dexfenfluramine and VHD. Epidemiological studies have also established that fenfluramine and dexfenfluramine cause a fatal disease known as primary pulmonary hypertension ("PPH").

A convenient way to analyze cardiac pressure and volume changes is by using ventricular pressure-volume loops . The following describes pressure-volume changes that occur during: Mitral stenosis Aortic stenosis Mitral regurgitation Aortic regurgitation Mitral Stenosis Mitral stenosis impairs left ventricular filling so that there is a decrease in end-diastolic volume ( preload ). This leads to a decrease in stroke volume by the Frank-Starling mechanism and a fall in cardiac output and aortic pressure. This reduction in afterload (particularly aortic diastolic pressure) enables the end-systolic volume to decrease slightly, but not enough to overcome the decline in end-diastolic volume. These changes just described do not include cardiac and systemic compensatory mechanisms that attempt to maintain cardiac output and arterial pressure. These compensatory responses include, but are not limited to systemic vasoconstriction, increased blood volume, and increased heart rate and inotropy.

HEART VALVE DISEASE
DEFINITION The heart's 4 valves help direct the flow of blood through its chambers (see figure 16.5 ). All of these valves are composed of thin leaflets that when closed prevent a backflow of blood and when open permit the blood to move forward to its next destination. When a valve fails to close properly, as is the case in a common disorder called mitral valve prolapse, there is a regurgitation or backflow of blood. A valve that fails to open properly—a condition called valvular stenosis—impairs the forward flow of blood to the body. In either case, the heart has to work harder to pump enough blood to the body, eventually leading to heart muscle damage. Congestive heart failure, syncope (fainting), and arrhythmias are common signs of valve disease.
CAUSE A number of conditions can lead to heart valve disease. Congenital defects and infections, such as rheumatic fever, are among the most common. Rheumatic heart disease, although greatly diminished since the advent of antibiotics to treat streptococcal infections, still affects more than 1 million Americans and causes about 6,000 deaths per year.
DIAGNOSIS In many cases, people can have a diseased heart valve for many years without suffering any symptoms or even being aware of the problem. Diseased valves can be detected by murmurs or other unusual sounds heard through a stethoscope. Ultrasound examination of the heart, also called echocardiography, in which sound waves are used to map internal structures, is also helpful. The most precise diagnosis is made by cardiac catheterization and angiocardiography.

Clinical presentation:
Adult male with a long history of breathlessness, on exertion. There is a bulky heart. Its contour is not normal. The pleural silhouette of the aortopulmonary reflection extends down to a bulge on the left heart margin. This has a smaller radius of curvature than the left heart margin. There is a double contour to the right heart margin. The upper margin is the ascending aorta, which has a different radius of curvature from the lower. The centre of radius of the lower curve is behind the heart. The tracheal bifurcation is not well shown, but the left hilum is a little depressed. Its dense portion is normally above the right because of the presence of the ligamentum arteriosum. There is an ill-defined density behind the left heart. The silhouette of the left lower lobe vessels is lost. Clinical presentation:
40 year old woman with breathlessness, on exertion. She had rheumatic fever at the age of 9 years.
