The P roblem of I mmediate G ratification T o the extent that we can understand the biological, psychological and social determinants of behavior, we can, if committed enough, intentionally influence our life's course. Each of us is dealt a unique biology, psychological history and current social circumstance, and so there is a limitation on how relevent an externally developed treatment program can be to any particular individual. Unlike Newton who shared the same universe as his predesessors and so could "stand on the shoulders of giants," each of us must develop the ability to intentionally influence our actions during our own lifetime. The purpose of this web site is to describe the important cause and effect relationships pertaining to addiction - here defined as dependence on external incentives - and self-determination. The first cause of addiction is the Problem of Immediate Gratification [the PIG ]: A small immediate reward has a greater influence on behavior than a much larger, but delayed reward. Consequently, incentives that are especially good at producing immediate gratification are especially corrupting.

Depressive symptoms, memory problems, insomnia and hot flashes occur commonly in women entering the menopause; however, some women develop more significant symptoms of major depression. The symptoms of perimenopausal depression include depressed or sad mood, tearfulness, loss of interest in usual activities, fatigue, sleep disturbance and difficulty concentrating. Women at greatest risk are those with a history of depression, particularly if depression has occurred in the past after childbirth or in association with the menstrual cycle. Services offered include psychiatric evaluation and consultation, medication management, and psychotherapy.     Everyone has moods. Moods are a normal part of everyday life. Daily life events, sleep, weather, physiological cycles, etc. can all alter one's moods. Mood disorders are distinct from normal moods in their depth and persistence, and in their interference in one's ability to function. More on Depression Adjustment Disorder with Depressed Mood "I felt shattered when she broke up with me. I knew that things weren't going that well lately, but I felt like I was punched in the gut when she told me she had met someone else whom she was interested in. Since then I've felt really awful. I'm okay wen I'm with friends, but when I'm alone, or when something reminds me of her, I feel really down. I've even thought of suicide at times. I haven't been able to concentrate well over the past while. I've been feeling quite worn out as well. Often I just can't seem to fall asleep. I just lie awake thinking about how alone I am." One thing you can be sure of in life, is that at some point you will have an adjustment disorder with depressed mood. The symptoms may be relatively mild, and may go away simply with support from family and friends. At times the symptoms may be quite severe and professional help may be necessary. Usually the symptoms gradually get better with time. Overwhelming feelings of hopelessness and despair one week, may diminish to tolerable sadness the next week. When treatment is necessary, counseling or short-term psychotherapy will usually quickly resolve the symptoms and unhappy feelings. When sleep is considerably disturbed, attention to normal sleep hygiene, or at times mild medication, will often leave a person better able to cope with the events causing their distress.

    Office Location The Department of Child and Adolescent Psychiatry provides an array of child and adolescent psychiatric services both here at Connecticut Children's Medical Center and at the Institute of Living On the Connecticut Children's campus, consultations are provided to inpatients and outpatients with special medical needs. Children in crisis are seen either in our ambulatory service or in the emergency room. On the campus of the Institute of Living there are inpatient psychiatric services, partial hospitalization programs, a child guidance clinic and a special education school. Throughout the Hartford Hospital/IOL spectrum of care, children and adolescents are seen with Attention-Deficit Hyperactivity Disorder, Oppositional Defiant Disorder, Conduct Disorders, Anxiety Disorders, Mood Disorders including Depression, Developmental Disorders such as Autism, Pervasive Developmental Disorders and Asperger's Syndrome, and Somatoform Disorders such as Conversion and Somatization, and Psychological Factors Affecting Medical Illness. The Department will triage referrals to the appropriate clinician based on his/her area of expertise and availability. There is also a special Pediatric Psychiatric hotline particularly devised for referring community pediatricians at 860.545.7493 and a 24-hour emergency service available at 860.545.7200.     Depression
    Bipolar Disorder (Manic Depression) Depression Feeling blue or sad, especially around the time of a difficult life experience, is normal. When sad feelings continue a long time, appear for no reason, or begin to affect multiple aspects of life - eating, sleeping, working, relationships - you may be experiencing a major depression.
    • What Is Depression?
      Depression is a biological illness which involves body, mind and emotions. Because depression involves a change in chemicals in the brain, people with depression cannot "just snap out of it." However, depression can usually be successfully treated with a combination of medications and counseling.
      What Causes Depression?
      There seem to be a combination of causes, different for each person. A vulnerability to become depressed appears to run in families, although not everyone at risk for depression gets depressed. Sometimes a depression can be triggered by a traumatic event or life change. In other cases, depression may be the result of a medical illness, such as stroke or heart attack, or a side effect of medication taken for a medical illness.

    Bipolar disorder, often called manic depression, is one of the major mood disorders and is distinguished by behavior that fluctuates from extreme highs (mania) to serious lows (depression) interspersed with periods of "normal" mood. Simply put, a person who suffers from the condition is unable to keep a sense of calm about his or her life.
    Mania is a term used a lot these days (like "basketball mania"), but in medicine, mania refers to behavior that includes profuse and rapidly changing ideas, exaggerated sexuality, extreme gaiety, intense irritability, and decreased sleep. Depression is used frequently to describe a feeling of sadness. With bipolar disorder, however, the lows of depression are characterized by extreme hopelessness and a feeling of worthlessness accompanied by thoughts of suicide.
