Transcranial magnetic stimulation (TMS) is a potential new treatment for depression. It is currently being tested as a potential antidepressant in a large clinical trial in the South East London area. Information on the trial and this promising new therapy in psychiatry is given below. hat does it involve? A single treatment with TMS (sometimes called rTMS) takes about 20 minutes. However, to receive a full course patients have to attend every weekday for 2 to 3 weeks (i.e. 10 to 15 treatments). The procedure itself involves a magnetic coil being held over a specific part of the front part of the head. The picture below shows a typical situation. The person being treated is wearing a cloth cap that has marks on it so that the doctor knows exactly where to hold the magnetic coil. here is the trial being run? Treatments are given at an out patient clinic at the Maudsley hospital on Denmark Hill, London. The trials are being coordinated by the following people: Dr Declan McLoughlin, Dr Andrew Mogg, Dr Savitha Eranti, Dr Graham Pluck and Mrs Lee Wilding all of whom work at the Institute of Psychiatry, De Crespigny Park, London. s this an alternative to medication?

    Recent research on depression provides invaluable insight into who is and who is not receiving treatment. Consistently, more than 80 percent of the people who score positive for depression are not in treatment at the time of the screening and nearly 60 percent have never been in treatment. Additional study results reveal that five particular symptoms appear more frequently in people who screen positive for depression. These symptoms are psychological in nature and are not symptoms that would necessarily lead someone to seek professional help. This may be one key explanation for the vast discrepancy between depression's prevalence and the number of people who actually receive the care they need.

    fifield@nso1.uchc.edu Fatigue Linked To History Of Mood Disorders In Rheumatoid Arthritis Patients Fatigue, a common and debilitating symptom of rheumatoid arthritis, may be up to 10 percent higher in patients who also have a history of depression and anxiety disorders, according to a new study. "Much research has been done tying fatigue to concurrent symptoms in rheumatoid arthritis patients, such as the accompanying pain, sleep interruptions and inflammation," said lead author Judith Fifield, PhD, of the University of Connecticut School of Medicine. "Our study suggests episodes of depression and other mood disorders, which occurred as much as three decades earlier, contribute and could be a predictor of fatigue levels in these patients." Fifield and her research team conducted yearly phone interviews between 1989 and 1995 with 415 patients diagnosed with rheumatoid arthritis to assess their levels of fatigue and distress, as well as record their histories of major depression and anxiety disorder. Participants were predominantly upper middle age (average 58 years), married women. The results of the study are published in the February 2001 issue of

    Neuroimaging in Child and Adolescent Mood Disorders
    During the last decade, neuroimaging has enhanced our understanding of psychopathology in adults with a psychiatric illness. Yet such studies in children have lagged behind those of adults because of concern about harming children by exposing them to unnecessary health hazards. In recent years, however, increased clinical confidence and a desperate need to better understand mood disorders in children and adolescents have led researchers to begin using neuroimaging in children. Magnetic resonance imaging (MRI) and spectroscopy are now being used to better delineate the structures of the brain and to identify functional and chemical defects of mood disorders in children and adolescents. In a symposium at the fall 1999 meeting of the American Academy of Child and Adolescent Psychiatry, five research centers reported on exploring the neurobiology of depression and mania in children and adolescents. Many of the studies discussed had a small number of subjects and were preliminary in nature, but their results may still help researchers identify biological factors that contribute to the manifestation of mood disorders in children. There is growing evidence that bipolar disorder in children and adolescents is similar to that in adults, and biological findings may help make the case that these children have a medical disorder that should be covered by third-party payers.

    Plants have growing and dormant seasons. Bears hibernate. Many birds fly thousands of miles between summer nesting sites and winter feeding areas. Likewise, the change in season can cause a change in mood in some people that is called a Seasonal Affective Disorders (SAD).
    Research has shown that in very depressed people there is a shortage of serotonin, along with dopamine and norepinephrine, three neurotransmitters used by the brain. So the serotonin and melatonin link along with their dependence on the body clock may explain the depression experienced by people suffering from SAD. Currently treatment of SAD and other rhythm disorders involves exposure to full spectrum visible light, which appears to help renormalize the circadian rhythms or waves of hormones, like melatonin within the larger seasonal waves. Other well-known methods of resetting, and entraining, the circadian rhythm in an organism include exercise, exposure to heat and cold, regularity of meals, and the organization of daily activities so that they are repeated at the same time each day.

    Treating Mood Disorders in Drug Abuse Patients Yields Improvement in Both Conditions
    By Patrick Zickler , NIDA NOTES Staff Writer For many drug abuse patients, mood disorders are a constant companion. Among cocaine abusers, for example, depressive disorders are the most commonly diagnosed coexisting, or comorbid, conditions. The relationship between mood disorders and drug abuse in these patients is often complex and interconnected: Drug abuse patients may develop depression as a result of the physical and psychological suffering associated with their drug use, and patients suffering from mood disorders may become drug dependent in attempts to self-medicate. For patients suffering from both drug abuse and mood disorders, the conditions once seemed impossible to untangle, but recent NIDA research suggests that treatment for the mood disorder alone also can have a positive effect on drug abuse treatment. One NIDA-supported study found that drug use declined among teenage drug dependence patients being treated with a medication for bipolar disorder, which is characterized by alternating periods of depression and mania. In a related study, chronic opiate-dependent adults reported less drug abuse when they were treated with the antidepressant imipramine for comorbid depression. Reducing Substance Dependence in Bipolar Teens Lithium has proven effective in treating adult bipolar disorder patients and research with adult patients suggests that lithium also may be an effective treatment for comorbid substance dependence. However, no double-blind, placebo-controlled studies have tested the therapy in adolescents. Now, at Washington University in St. Louis, Dr. Barbara Geller and her colleagues have found that, in adolescent patients, lithium treatment for the manic phase of bipolar disorder also reduces drug and alcohol dependence.
