What is Multiple System Atrophy?
Multiple System Atrophy (MSA) is a neurological disorder caused by degeneration of cells in certain areas of the brain. These control a number of different body systems; hence the name. They include functions of the autonomic nervous system (such as the control of blood pressure, sweating, bladder function) and the motor systems (such as muscle activation, movement and balance). MSA affects both men and women. Symptoms usually start between 40 and 60 years of age. The cause of MSA is not known, but is neither inherited nor contagious. You may also hear it referred to as Shy Drager Syndrome. Symptoms
MSA can manifest in a variety of different ways and progresses in an unpredictable sequence and time scale for each patient. It is important to understand that having a diagnosis of MSA does not mean that you will experience all the symptoms from the range below.
The initial symptoms may resemble Parkinson's Disease, with slowness of movement, stiffness of limbs and occasionally tremor. Some patients may have been given anti-parkinsonian drug treatment for these problems (with mixed results). A poor response to these drugs and the appearance of some of the following symptoms are indicators of MSA:-

What is Shy-Drager Syndrome/Multiple System Atrophy? autonomic nervous system. The second patient subsequently expired and the postmortem examination provided evidence that he had a distinct and unique disease. Drs. Shy and Drager recognized that there was a link between low blood pressure during erect posture (orthostatic hypotension) and disturbances in the central autonomic system. To acknowledge their contribution, this disorder is known as "Shy-Drager Syndrome" (SDS). Today, Shy-Drager Syndrome (also called Multiple System Atrophy ) is a neurological disease resulting from degeneration of certain nerve cells in the brain and spinal cord. Body functions controlled by these areas of the brain and spinal cord function abnormally in patients with this disease. These include the autonomic nervous system or involuntary nervous system (which controls blood pressure, heart rate, and bladder function) and the motor system (control of balance and muscle movement). People in all parts of the world are affected, with onset usually between ages 45 and 60. Approximately 75,000 Americans have this disease. The cause of the disease is not known. It is not inherited and it is not contagious. Depending upon which part of the brain is affected first, the MSA may appear in different ways.

The loss of dopamine in the striatum is the major contributor to the disorder. However, there are several other neurodegenerative disorders involving several different systems in the brain, in which striatal dopamine loss is also found. Among these other neurodegenerative disorders is multiple system atrophy (MSA), in which degeneration in diverse brain regions leads to problems in the control of movement, balance, blood pressure, and sexual and urinary tract function. MSA is often accompanied by some striatal dopamine loss and in certain patients typical parkinsonian symptoms are either the first noted or the most prominent. When MSA begins with non-autonomic features, imbalance is the most common feature. This difficulty in maintaining balance may be due to either cerebellar or Parkinsonian abnormalities. Some patients complain of stiffness, clumsiness, or a change in handwriting at the onset of MSA. The concurrent involvement in MSA of multiple brain systems subserving movement, including the striatum, cerebellum, and cortex, leads to the movement disorder as often being the most profound disability. Hoarseness or even vocal paralysis are relatively common, as are sleep disturbances, including snoring and sleep apnea. The ability to swallow foods and liquids may be impaired.

    Mouse with Iron Disorder Offers Clues to Parkinson's, Similar Diseases Mice engineered to lack a gene involved in iron metabolism may provide important clues for deciphering the nature of a group of brain disorders-similar to Parkinson's Disease-affecting movement, according to a study by researchers at the National Institute of Child Health and Human Development (NICHD). The researchers reported in the February issue of Nature Genetics that mice lacking the gene for iron regulatory protein 2 (IRP2) develop iron deposits in key areas of the brain. The progressive deterioration of the mice's nervous system is like that seen in people with Parkinson's Disease and Multiple System Atrophy (also known as Parkinson's Plus). "Researchers have long debated whether the characteristic iron deposits of these diseases are the cause or the result of the disease process," said Duane Alexander, M.D., Director of the NICHD. "This is a strong clue that iron may play a causative role in Parkinson's and similar disorders." Diseases in which excess brain iron has been observed include Parkinson's Disease, Friedreich's Ataxia and Hallervorden-Spatz disease. However, the brain areas affected differ from those in the IRP 2 deficient mice, and it is unlikely that IRP 2 is involved in these human diseases, said the study's senior investigator, Tracey A. Rouault, M.D., of NICHD's Cell Biology and Metabolism Branch. Still, the NICHD finding suggests that other genes involved in iron metabolism may be good candidates for investigations into the causes of these disorders.

    Causes, incidence, and risk factors: Multiple system atrophy (MSA) is a rare degenerative condition. It is a condition that appears similar to Parkinson's disease in that patients may be slow moving, tremulous and have a shuffling gait but with more widespread neurologic damage and damage to the autonomic nervous system (the portion of the nervous system that controls involuntary functions).
    The cause is unknown. There is progressive damage (degeneration) of the nervous system, with damage to all parts of the nervous system. The disorder develops gradually. It is most often diagnosed in men over 60 years old.
