You react with fear and surprise and are quick to ask, "What does this mean and is my child in danger?" This can be a very frightening situation for new parents because they believe a heart murmur is a sign of a very serious condition. However, about 90 percent of normal children in the first five years of life are noted to have murmurs. A heart murmur is simply an extra sound that the heart makes. A normal beating heart makes two distinct sounds. The first sound, or the "lub", occurs when the valves between the upper (atria) and lower (ventricles) chambers close. These valves close after the ventricles fill with blood and begin to contract. The second sound, or the "dub", occurs when the valves between the ventricles and major arteries close. These valves close after the ventricles have finished pumping. A murmur is a sound made by turbulent blood flow and is heard between the other, normal heart sounds. Murmurs can be innocent, normal sounds or pathologic, abnormal sounds. I usually tell parents to think of murmurs as the sound of water flowing through pipes after the faucet is turned on. The sound may be a normal flow, a leaky faucet or a blocked faucet.

What are the types of murmurs? Types of murmurs include:
  • diastolic murmur - occurs during heart muscle relaxation between beats. systolic murmur - occurs during heart muscle contraction and is graded by intensity (on a scale of one to six, with one being very faint and six being extremely loud). stenotic heart valve - valve cannot open completely, so blood is ejected through a smaller than normal opening. aortic or mitral regurgitation - valve may be unable to close completely, leading to regurgitation of blood back through the valve when it should be closed. innocent heart murmurs - sounds made by blood circulating through the chambers and valves of the heart or through blood vessels near the heart; sometimes referred to as "functional" or "physiologic" murmurs. Innocent murmurs are common in children and usually quite harmless and disappear when a child reaches adulthood, though some adults still have them.

Normal heart sounds come in pairs. The sounds are often described as a constant "lub-dub, lub-dub." The first "lub-dub" is the sound of the mitral and tricuspid valves closing. The second "lub-dub" is the sound of the aortic and pulmonary valves closing soon after. But if there is a problem, a murmur may be added to this normal "lub-dub." By using a stethoscope to listen to your heart, your doctor's trained ear can tell if the abnormal sound indicates turbulence. This is called a heart murmur. Some heart murmurs are a harmless type called innocent heart murmurs. They are common in children and do not require treatment or lifestyle changes. In most cases, innocent murmurs disappear when children reach adulthood. Though some heart murmurs are innocent, others are a sign of a more serious heart problem. In these cases, the sound may indicate that blood is flowing through a damaged or overworked

All murmurs are analyzed for pitch, loudness, and duration. They are also graded according to their intensity (on a scale of one to six, with one being very faint and six being very loud). Types of murmurs include:
  • systolic murmur - occurs during a heart muscle contraction. Systolic murmurs are divided into ejection murmurs (due to blood flow through a narrowed vessel or irregular valve) and regurgitant murmurs. diastolic murmur - occurs during heart muscle relaxation between beats. Diastolic murmurs are due to a narrowing of the mitral or tricuspid valves, or regurgitation of the aortic or pulmonary valves.

Heart murmurs: Which are innocent?
Many children have a heart murmur, especially during febrile illness. Most of these are "innocent", or "functional" murmurs, but some are caused by organic heart defects that are asymptomatic. When a heart murmur is detected during routine physical exam, a careful review of the child's appearance and history often provides clues about the murmur's significance. The classical innocent murmur is the vibratory systolic murmur referred to as Still's murmur. It is soft and of low frequency with a somewhat musical quality, like plucking a string or rubber band. This murmur is of short duration and occurs between the two heart sounds (mid-systole) and is often referred to as "physiologic", or "not significant." Other innocent murmurs are soft, quiet and "blowing" in character. Innocent murmurs often come and go. Also they may disappear if the child sits up or lies down. Once a murmur is discovered, (provided it's not a classical Still's murmur), observation of the child is important, along with noting vital signs. Is the color of mucous membranes and nailbeds pink or dusky (cyanotic)? Is exercise intolerance present? By history, have there been syncopal episodes or palpitations? All these findings suggest an organic murmur.

    You're having a routine checkup when the doctor notices something while listening to your chest through the stethoscope. The doctor says you have a heart murmur, and she would like to check it out further.
    What Are Heart Murmurs?
    A heart murmur is an extra sound made by blood flowing through the heart valves in the different chambers of your heart. Normally, the heartbeat makes a "lubb-dupp" sound. The "lubb" occurs during systole with closure of the valves separating the upper and lower parts of your heart (the atria and the ventricles respectively). The "dupp" is heard during diastole when the valves close between the ventricles and the large blood vessels (aorta and pulmonary artery) that receive blood from the heart. often a "swish" sound occurs somewhere during the "lubb-dupp" of the normal heartbeat. With his or her training and experience in evaluating heart murmurs, by listening carefully to the timing and characteristics of the extra sound, your physician often can tell whether it represents a potentially serious problem.

    ... Women at Risk Understanding Heart Murmurs How Serious Are They? Simply stated, a heart murmur is an abnormal sound in the heart beat. Generally, during a routine office visit, the doctor listens to your heartbeat using a stethoscope an can detect a murmur if present. In most cases, heart murmurs do not affect your health and result in some form of irregular blood flow through the heart. Some, however, are due to underlying defects in the heart or its valves and may require treatment. What are the Underlying Defects? Two common defects that can cause a heart murmur are valvular defects and septal defects. Valvular defects refer to abnormalities of the valves that keep blood from flowing "backwards" into the heart chambers. A defective valve may allow blood to back up in the wrong direction or it may block the passage of blood, thereby interrupting the normal flow. Septal defects are small breaks in the partition that separates the left and right sides of the heart (septum). These breaks can allow blood to flow irregularly between the two sides. Septal defects are more commonly found in children.
