Transverse myelitis
Definition
Transverse myelitis (TM) is an uncommon neurological syndrome caused by inflammation (a protective response which includes swelling, pain , heat, and redness) of the spinal cord, characterized by weakness, back pain, and bowel and bladder problems. It affects one to five persons per million.
Description
TM affects the entire thickness of the spinal cord, producing both sensory and movement problems. It is believed to be linked to the immune system, which may be prompted to attack the body's own spinal cord. Striking rapidly without warning, its effects can be devastating.
Transverse myelitis has many different causes, often triggered by a variety of viral and bacterial infections (especially those associated with a rash such as measles or chickenpox). Once the infection subsides, the inflammation in the cord begins. About a third of patients experience a flu-like illness with fever about the time they develop symptoms of TM. Sometimes, there appears to be a direct invasion of, and injury to, the spinal cord by an infectious agent (such as herpes zoster or the

The rapid clinical deterioration and imaging findings of edema within the spinal cord are similar to those seen in transverse myelitis of other etiologies. Outcome varies, with about half of SLE cases of transverse myelitis reported to have complete recovery, 30% with partial recovery, and 20% with no recovery (4). A. B. C. Figure 6. Myelitis in SLE. A woman in her late 30s present with acute onset paraplegia 6 years after the diagnosis of SLE. She was treated with cyclophosphamide and corticosteroids but failed to regain any significant lower extremity motor function. Sagittal T1-weighted image (A) is unremarkable. After intravenous gadolinium administration, sagittal T1-weighted image (B) shows enhancement in the lower thoracic spinal cord (arrow). Corresponding high signal intensity is seen on the sagittal T2-weighted image (C). Focal White Matter Lesions Small areas of T2 prolongation in cerebral white matter are common in SLE. The correlation with clinical signs and symptoms of CNS disease is unclear (5). Most likely these are related to chronic changes in the brain rather than an acute event. However, in the patient with an acute presentation, diffusion-weighted imaging can be very helpful in distinguishing small, acute infarcts from chronic changes. The pathologic basis of these white matter lesions is also unclear. These are not usually due to an underlying vasculitis. Despite SLE often being included in lists of cerebral vasculitides, few SLE patients have true vasculitis. Rather, SLE is often accompanied by a bland, small vessel vasculopathy. In our experience, small areas of T2 hyperintensity are very common in SLE patients and are most often seen in subcortical white matter.

Transverse Myelitis (TM)
Transverse Myelitis is a neurological syndrome caused by inflammation of the spinal cord. The term myelitis is a nonspecific term for inflammation of the spinal cord; transverse refers to involvement across one level of the spinal cord. Initial symptoms may include back pain, rash, headache, weakness, numbness and/or tingling of the legs and generalized discomfort. Eventual paralysis, sensory loss, and bowel and bladder dysfunction may occur. Transverse Myelitis can be acute or slow developing. There are several variations of the TM diagnosis, as well. TM may occur in isolation or in the setting of another illness. When it occurs without apparent underlying cause, it is assumed to be a result of abnormal activation of the immune system against the spinal cord. TM often develops in the setting of viral and bacterial infections. Approximately one third of patients with TM report a febrile illness (flu-like illness with fever) in close temporal relationship to the onset of neurological symptoms. Vaccination is well known to carry a risk of the development of acute disseminated encephalomyelitis (ADEM) which is an acute inflammation of the brain and spinal cord.

One of Sir Hugh’s brothers was Sir Thomas Myddelton (c. 155o—163f), lord mayor of London, and another was William Myddelton (c. 1556—1621), poet and seaman, who died at Antwerp on the 27th of ~‘Iarch 1621. Sir Thomas was a member of parliament under Queen Elizabeth arid was chosen lord mayor on the 29th of September 1613, the day fixed for the opening of the New River. Tinder James I. and Charles I. he represented the city of London in parliament, and he helped Rowland Heylyn to publish the first popular edition of the Bible in Welsh. He died on the 12th of August 1631. Sir Thomas’s son and heir, Sir Thomas Myddelton 1586—1666), was a member of the Long Parliament, being an adherent of the poptilar palty. After the outbreak of the Civil War he served in Shropshire and in north Wales, gaining a signal sticcess over the royalists at Oswestry in July 1644, and another at Montgomery in the following September. In 1659, however, he joined the rising of the royalists under Sir George Booth, and in August of this year he was forced to surrender his residence, Chirk Castle. His eldest son, Thomas (d. 1663), was made a baronet in 1660, a dignity which became extinct when William the 4th baronet died in 1718. In early May 1984, CDC received reports that 15 (83%) of 18 American students participating in a travel/study program in Kenya had acquired Schistosoma mansoni infections. Two of these students developed flaccid paraplegia. Although data are incomplete on all 18 students, no unusual attributes could be identified in these two students that might explain why their infections were associated with severe neurologic disease. General background information and the case histories of these students follow. All 18 students arrived in Kenya on February 13, 1984. From March 5 to March 25, they shared housing in the Machakos district. To provide a place for bathing, the students dammed a small stream; two of the infected students recalled experiencing an itchy rash shortly after bathing at this site. Subsequently, the group separated, as the students took individual assignments in various regions of the country. Between April 26 and May 12, 14 of the 15 infected individuals became acutely ill with fever, diarrhea, malaise, and weight loss. On evaluation in the United States, the student had no rash, fever, lymphadenopathy, hepatosplenomegaly, or point tenderness on palpation of the spinal column. Neurologic examination revealed a flaccid paraplegia at and below the level of T-10. There was marked sensory loss, including loss of vibratory sensation.
