Equine protozoal myelitis (EPM): EPM is a disease caused by protozoal infection of the central nervous system (CNS). Originally, Sarcocystis neurona was thought to be the only responsible species; however, recent work has suggested that Neospora hughesii , and perhaps other protozoans, may be capable of causing similar syndromes. The disease typically occurs sporadically but epizootics have been reported. The organisms may affect any portion of the CNS, however the spinal cord is most commonly affected. Definitive antemortem diagnosis is difficult due to poor sensitivity and specificity of currently available serological tests. Diagnosis is confirmed at necropsy by demonstration of protozoa associated with characteristic CNS lesions including malacia, discoloration, and hemorrhage. Treatment with trimethoprim sulfa (TMS) or pyrimethamine is effective in many cases.

Question: My aunt has been suffering from numbness in her legs. After a spinal tap and three MRIs, we were told that she has an inflammation of the spinal cord - myelitis. I would appreciate any information. Answer: There are several types of myelitis. Poliomyelitis was common prior to polio vaccine. However, there are several types. Basic types are infectious (usually viral), paraneoplastic (due to tumors), transverse (cause unknown, but thought to be post viral) and inflammatory (usually systemic lupus). The most common is transverse myelitis and many think this is related to multiple sclerosis - but controversial. The disease is manifested by demyelination of the spinal cord leading to dysfunction of the affected nerves. Treatment varies with the probably cause, but is generally steroids or something equivalent. After the damage/inflammation is over there is no effective treatment and the nerve is permanently damaged. Back to Drug InfoNet Home Page.

Transverse Myelitis
Transverse myelitis is a demyelinating (loss of the fatty tissue around the nerves) disorder of the spinal cord. It may occur alone or in combination with demyelination in other parts of the nervous system. Onset of the disorder is sudden. Symptoms may include low back pain, spinal cord dysfunction, muscle spasms, a general feeling of discomfort, headache, loss of appetite, and numbness or tingling in the legs. Transverse myelitis may be caused by viral infections, spinal cord injuries, immune reactions, or insufficient blood flow through the blood vessels in the spinal cord. It may also occur as a complication of such disorders as optic neuromyelitis, multiple sclerosis, smallpox, and measles, or as a complication of chickenpox vaccinations. There is no specific treatment for transverse myelitis. Treatment for the disorder is symptomatic. Generally, prognosis for complete recovery from transverse myelitis is not good. Although recovery usually begins between 2 and 12 weeks after onset and may continue for up to 2 years, most individuals are left with considerable disability. Some individuals may have minor or no deficits, while others may have significant motor, sensory, and sphincter (bowel) deficits. Some individuals show no recovery at all.

Transverse Myelitis is a rare inflammatory disease involving both sides of the spinal cord. The Transverse Myelitis Association was established in 1994 as a not-for-profit organization dedicated to advocacy for those who have the TM diagnosis. Mission The Transverse Myelitis Association facilitates support and networking opportunities amongst TM families; provides educational information; functions as a clearinghouse for articles and research literature about the TM diagnosis; and investigates, advocates for and supports research and innovative treatment efforts. Programs The TMA publishes a newsletter for our members twice a year, which contains articles by physicians and health care providers who treat persons with TM. There are also articles written by people with TM. A membership directory is published and distributed annually. The TMA assists in the development of local support groups and is involved in various fund-raising efforts, such as Reading for Rachel, to raise awareness and funds for TM research. The TMA supports and conducts various symposiums and workshops involving both professionals and patients for the exchange of information regarding TM research and treatment strategies for persons with TM.

It affects one to five persons per million. Description TM affects the entire thickness of the spinal cord, producing both sensory and movement problems. It is believed to be linked to the immune system, which may be prompted to attack the body's own spinal cord. Striking rapidly without warning, its effects can be devastating. Causes and symptoms Transverse myelitis has many different causes, often triggered by a variety of viral and bacterial infections (especially those associated with a rash such as measles or chickenpox). Once the infection subsides, the inflammation in the cord begins. About a third of patients experience a flu-like illness with fever about the time they develop symptoms of TM. Sometimes, there appears to be a direct invasion of, and injury to, the spinal cord by an infectious agent (such as herpes zoster or the AIDS virus). TM can also accompany a variety of diseases that break down tissue that surrounds and insulates the nerves (demyelinating diseases), such as multiple sclerosis (MS).

TM ist ein neurologisches Syndrom das durch Entzündungen im Rückenmark verursacht wird. Die Bezeichnung Myelitis ist eine unspezifische Bezeichnung für Entzündung des Rückenmarks; transverse weist auf eine Miteinbeziehung über eine Höhe des Rückenmarks hin. Ausgangssymptome können Rückenschmerzen, Hautausschlag, Kopfschmerzen, Schwäche, Taubheit und / oder Kribbeln in den Beinen und allgemeines Unbehagen einschließen. Eventuell können auch Lähmungen, Sensoritätsverlust und Darm- und Blasenfunktionsstörungen auftreten. TM kann akut auftreten oder sich langsam entwickeln. Es gibt außerdem auch verschiedene Varianten der transversen Myelitis.
Ursachen der Transversen Myelitis
TM kann allein oder im Zusammenhang mit einer anderen Krankheit auftreten. Wenn es offensichtlich ohne zugrundeliegende Ursachen auftritt, wird angenommen, daß es sich um ein Resultat einer anormalen Aktivierung des Immunsystems gegen das Rückenmark handelt. TM entwickelt sich häufig im Zusammenhang mit einer Virus- und bakteriellen Infektion. Ungefähr jeder dritte Patient mit TM berichtet von einer fiebrigen Erkrankung (grippeähnliche Erkrankung mit Fieber) in engem zeitlichen Zusammenhang zu Beginn der neurologischen Symptome. Es ist weitläufig bekannt daß Impfungen ein Risiko zur Entwicklung der sich akut verbreitenden Enzephalomyelitis (ADEM), die eine akute Entzündung des Gehirns und des Rückenmarks ist, tragen.
