The muscles and nerves of the urinary system work together to hold urine in the bladder and then release it at the appropriate time. Nerves carry messages from the bladder to the brain and from the brain to the muscles of the bladder telling them either to tighten or release. In a neurogenic bladder, the nerves that are supposed to carry these messages do not work properly. Complications of a neurogenic bladder:

Causes and symptoms Diagnosis ... Resources Definition Neurogenic bladder is a dysfunction that results from interference with the normal nerve pathways associated with urination. Description Normal bladder function is dependent on the nerves that sense the fullness of the bladder (sensory nerves) and on those that trigger the muscle movements that either empty it or retain urine (motor nerves). The reflex to urinate is triggered when the bladder fills to 300-500 ml. The bladder is then emptied when the contraction of the bladder wall muscles forces urine out through the urethra. The bladder, internal sphincters, and external sphincters may all be affected by nerve disorders that create abnormalities in bladder function. There are two categories of neurogenic bladder dysfunction: overactive (spastic or hyper-reflexive) and underactive (flaccid or hypotonic). An overactive neurogenic bladder is characterized by uncontrolled, frequent expulsion of urine from the bladder. There is reduced bladder capacity and incomplete emptying of urine. An underactive neurogenic bladder has a capacity that is extremely large (up to 2000 ml). Due to a loss of the sensation of bladder filling, the bladder does not contract forcefully, and small amounts of urine dribble from the urethra as the bladder pressure reaches a breakthrough point.

Description
Neurogenic bladder is impaired bladder function resulting from damage to the nerves that govern the urinary tract. Various nerves converge in the area of the bladder and serve to control the muscles of the urinary tract, which includes the sphincter muscles that normally form a tight ring around the urethra to hold urine back until it is voluntarily released. A variety of factors can damage these nerves and cause urinary incontinence. In some cases, spontaneous nerve impulses to the bladder trigger spastic unexpected bladder contractions, resulting in accidental voiding of sometimes large amounts of urine. In other types of neurogenic bladder conditions, the bladder may become flaccid and distended and cease to contract fully, resulting in only partial emptying and continual dribbling of small amounts of urine. Rashes may erupt in areas of the skin irritated by urine. Stagnant urine in the bladder also increases the risks of bladder stone formation and urinary tract infections. Such infections, when severe, can lead to life-threatening kidney failure. In some patients, there is a partial loss of anal sphincter control as well.

Management of the neurogenic bladder in children Another area of clinical investigation is the neuropathic bladder associated with spina bifida. Our doctors assist in the care of a large population of children with this condition through the Texas Scottish Rite Hospital, located a short distance from Children's Medical Center. We provide comprehensive urodynamic evaluation, medical management and surgical reconstruction, and are currently creating a database to identify important health influences. For example, we are now reviewing our experience with various conduits as an alternative route to the urethra for intermittent catheterization, having found this procedure greatly facilitates self-care by children who depend upon intermittent catheterization to empty the bladder.

Spinal Cord Injury - Neurogenic Bladder Definition and causes of a neurogenic bladder
The term neurogenic bladder refers to the condition, caused by spinal cord injury or other neurological disease, in which patients lose normal control over voiding. Multiple sclerosis, Parkinson’s disease, stroke, and other syndromes cause interruptions in the normal neurological pathways which control bladder and sphincter function. Depending upon the severity, this may lead to severe problems with urinary incontinence, recurrent urinary tract infections, and renal deterioration. Accurate diagnosis, using modern urodynamic techniques, allows specific therapy to be instituted. The use of medications, intermittent catheterization and surgical reconstruction allows restoration of continence and dramatically improved quality of life. What are the key procedures used to diagnose a neurogenic bladder Urodynamics Urodynamics refers to the study of bladder and urethral sphincter function; by measurement of bladder capacity and contractility, an accurate diagnosis can be made. For those patients who have bladder control problems because of spinal cord injury, multiple sclerosis, or other neurological condition, urodynamics is an essential tool to direct management. For men with voiding difficulties because of BPH (benign prostatic hyperplasia), urodynamics are also quite helpful in choosing the most appropriate therapy. Urodynamic studies are generally performed by placing a small catheter into the bladder and then filling the bladder with either water or radiopaque contrast. The pressure within the bladder is measured, usually electronically, and the activity of the external urethral sphincter can be measured as well. When these studies are done with Xray capability, the configuration of the bladder can be assessed, as well as any evidence of vesico-ureteral reflux. Finally, during urination, the presence of absence of obstruction can be determined by measurement of the flow rate and voiding pressure.
