Doctors generally agree that men with more than 25% body fat and women with more than 30% are obese. Obesity is a known risk factor for chronic diseases including heart disease, diabetes, high blood pressure, stroke and some forms of cancer. Evidence suggests that obesity has more than one cause: genetic, environmental, psychological and other factors may all play a part. The hormone leptin, produced by adipocytes (fat cells), was discovered about three years ago in mice. Subsequently the human Ob gene was mapped to chromosome 7. Leptin is thought to act as a lipostat: as the amount of fat stored in adipocytes rises, leptin is released into the blood and signals to the brain that the body has enough to eat. However, most overweight people have high levels of leptin in their bloodstream, indicating that other molecules also effect feelings of satiety and contribute to the regulation of body weight. The discovery of leptin has initiated a flurry of research into the molecular basis of weight control. A whole network of signals contributes to weight homeostasis, and other key players are being discovered on an ongoing basis. Mice have proved to be an extremely useful model for human obesity, and have helped to begin to unravel the components that contribute to maintaining body weight. Since the market for effective weight-reducing therapies is enormous, drug companies are working alongside basic scientists to find possible drug targets among the tangle of molecules that control body weight.

Over one-half of all Americans (about 97 million) are overweight or obese. If you are overweight or obese, carrying this extra weight puts you at risk for developing many diseases, especially heart disease, stroke, diabetes, and cancer. Losing this weight helps to prevent and control these diseases. Obesity is a condition of excess body fat. It is the most common form of malnutrition in the Western world. It is important to note that no "diet or weight loss formula" works independently of an eating plan based on the Dietary Guidelines given by the mainstream nutrition and health institutes. So if you are obese throw away all those "special formulas", supplements, and pills, stop eating more calories than you need, become more physically active and adopt a healthy eating plan. Severe overweight increases the risk for high blood cholesterol, high blood pressure, and diabetes and, hence, for diseases for which these conditions are risk factors (diabetes, coronary heart disease, high blood pressure, neurologic disorders, cancers, and kidney diseases). Obesity thus contributes to premature mortality. Of all obesity-related diseases, noninsulin dependent diabetes is most clearly and strongly associated with obesity. Obesity is associated with too many adipose cells (hyperplastic obesity), adipose cells that are too large (hypertrophic obesity), or both. While changes in the size of adipose cells may occur at any age, the number of adult cells are fixed and determined by weight gain during certain periods of childhood development; fat cell numbers are established by late adolescence and, once established, does not decline.

Some doctors and dietitians said they were horrified when they saw the public service announcement from the American Heart Association. "I was shocked," said registered dietician Joanne Ikeda of the University of California at Berkeley. "I was very surprised that the AHA would produce a public service announcement like this." Like most things, obesity is a complex phenomenon about which it is dangerous to generalize. What is true for one person is not necessarily true for the next. Nevertheless, we shall try to make sense out of conflicting theories and give answers to people who struggle to maintain self-esteem in a world that seems to be obsessed with youth, thinness, and the perfect body whatever that may be.
  • What is obesity?
A person with anorexia nervosa may define obesity as a weight gain of five pounds, from 89 to 94. A grandmother past menopause may call herself obese because she carries 165 pounds on her large-boned, muscular body. A modeling agency may talk about obesity when one of the women on the payroll puts 135 pounds on her 5'10" body. None of these women is clinically obese. The anorexic and the model are underweight. Men are split in their personal definitions of obesity. Many are just as concerned about overweight as women are, while others, frankly rotund, believe they are just fine, perfectly healthy, and universally attractive to potential romantic partners. Physicians consider a person to obese if s/he weighs more than 20% above expected weight for age, height, and body build. Morbid or malignant obesity is weight in excess of 100 pounds above that expected for age, height, and build.

Kartini Clinic was established in June of 1998 to better serve the children and adolescents (ages 6 - 16) of the Pacific Northwest who suffer from eating disorders such as anorexia nervosa, bulimia nervosa, or binge eating disorders, as well as those whose disordered eating contributes to obesity or wasting conditions. At the Kartini Clinic we take a biopsychosocial approach to eating disorders, with a strong medical component and emphasis on prompt, adequate weight restoration as a prerequisite to normal growth and development. We work closely with other pediatric sub-specialists to provide up-to-date care for these conditions, and require our patients to have a primary care doctor with whom we will work closely to assure continuity.
Eating disorders are complex disorders of unknown etiology. New research in the realm of neurobiology and genetics has taught us that anorexia and bulimia are inherited conditions which are steered by powerful but poorly understood brain chemistry. This understanding of the

    WHAT IS OBESITY? Obesity simply means an accumulation of body fat. In the medical world, it is determined by using a Body Mass Index (BMI), which measures weight in relation to your height. To calculate your Body Mass Index, go to our Body Mass Index Calculator. Obesity is classified in three ways:
    • Obese Class I (Mild) - 20% to 40% overweight
    • Obese Class II (Moderate) - 41% to 100% overweight
    • Obese Class III (Severe) - more than 100% overweight (only occurs in 0.5% of obese people)
    Causes of Obesity in Adulthood Obesity in the United States is on the rise. In the last decade, the number of obese Americans has increased by 33%. Overall, 31% of men and 35% of women are obese.
    • Genetics - Research suggests that on average, genetics influence about 33% of body weight, however it may be more or less in a particular person.

    C auses of obesity in the United States are complex and multifactorial. Increasing evidence suggests that obesity is not a simple problem of will power or self-control but a complex disorder involving appetite regulation and energy metabolism that is associated with a variety of comorbid conditions.
