Peer Review Status: Internally Peer Reviewed INTRODUCTION DIAGNOSIS AND CLINICAL PRESENTATION The psychiatric co-morbidity associated with OCD is high. The primary Axis I diagnoses often associated with OCD are Major Depressive Disorder, Anxiety Disorders (e.g., Specific Phobia, Panic Disorder, Social Phobia), Eating Disorders, and Obsessive-Compulsive Personality Disorder or those personality disorders in the Cluster C category (Black et al 1993). In the study by Rasmussen and Eisen (1992), two thirds of the patients examined had experienced a Major Depression sometime in their lives. Eight-five percent of these patients had a secondary depression due to the OCD and 15% had a concurrent depression. It is estimated that approximately 35-50% of patients with Tourette's Syndrome also suffer from OCD and 5-7% of OCD patients also have Tourette's, although between 20-30% of these patients have reported current or past tics (American Psychiatric Association 1994). PATHOPHYSIOLOGY The true pathophysiology of OCD is unknown at this time, but many theories have surfaced which are based on what we know has been effective to treat this disease and our knowledge of how these medications work. The role of serotonin (5HT) has been highly questioned and investigated. Since medications that are serotonin reuptake inhibitors have been shown to be effective in OCD, it is believed that changes in 5HT function are critical for the treatment of this disease, and perhaps also play a role in the pathophysiology of the disease. The role of serotonin in OCD has been demonstrated in studies using serotonin agonists and antagonists. M-CPP is a 5-HT 1C receptor agonist that also bind to 5-HT2, 5-HT1A, and 5-HT1B receptors. Administering this compound causes acute exacerbations of OCD symptoms, while fenfluramine, which is a 5-HT presynaptic agonist, does not exacerbate symptoms (Chouinard 1992).

Begins in early adulthood and features a combination of the following; Fixation with details, rules, order, control, structure, and losing the forest for the trees. Difficulty completing tasks because it is impossible to meet their own strict standards for perfection. Ignores leisure and friendships, because of obsession with work. Overambitious, honest, moral, and fixated about values. Behaves like a pack rat, never able to throw out items, even without any sentimental value. Cannot delegate because of fear work will not be done to perfection. Miserly and stingy. Stubborn and rigid.
These individuals are rigid and inflexible, and have difficult interpersonal dealings. Treatments for the disorder which do not fit the individual's cognitive rigidity will be quickly rejected. Learning and evaluation of self is difficult. Change is difficult. Convincing this type of patient is difficult and impossible without their sign-off. Individuals with this problem seek help when life becomes overwhelming and where functioning deteriorates considerably. Psychotherapy generally directs itself at symptom relief and development of coping mechanism. Long term solutions are uncertain, expensive, and limited in scope. Change is extremely difficult due to the very nature of the disorder.

PROFILE:
Obsessive Compulsive Disorder (OCD) is a neurobiological disorder. OCD causes repetitive thoughts and behaviors that are distressing, disruptive and senseless. It has been estimated that OCD will affect over 5 million Americans during their adult lives. We are recruiting individuals (children and adults) with OCD for our family studies.
GOALS:
Our goal is to identify the genetic factors that contribute to OCD. We will use this information to: define the causes of OCD improve the clinical diagnosis of OCD improve interventions and treatment of OCD
ELIGIBILITY:
Children with a diagnosis of OCD Adults with a diagnosis of OCD that have another family member that also has been diagnosed with OCD
STUDY:
Participating in the study involves: Brief interview with study coordinator to determine if you are eligible and discussing family and medical history. Signing an informed consent. Giving a small sample of blood (this can be done by your local lab or physician at no cost to you).

What Is Obsessive-Compulsive Disorder?

However, when they become so excessive such as hours of hand washing or make no sense at all such as driving around and around the block to check that an accident didn't occur then a diagnosis of OCD is made. In OCD, it is as though the brain gets stuck on a particular thought or urge and just can't let go. People with OCD often say the symptoms feel like a case of mental hiccups that won't go away. OCD is a medical brain disorder that causes problems in information processing. It is not your fault or the result of a "weak" or unstable personality. Before the arrival of modern medications and cognitive behavior therapy, OCD was generally thought to be untreatable. The obsessive thoughts range from the idea of losing control, to themes surrounding religion or keeping things or parts of one's body clean all the time. Compulsions are behaviors that help reduce the anxiety surrounding the obsessions. Most people (90%) who have OCD have both obsessions and compulsions. The thoughts and behaviors a person with OCD has are senseless, repetitive, distressing, and sometimes harmful, but they are also difficult to overcome. OCD is more common than schizophrenia, bipolar disorder, or panic disorder, according to the National Institute of Mental Health. Yet, it is still commonly overlooked by mental health professionals, mental health advocacy groups, and people who themselves have the problem.
