Clinically there is slate gray to browish discoloration of the face. Under microscope examination there is homogenous brownish material in the dermis. This may be denatured collagen or other proteins. This condition is
rather difficult to treat with medication. The best treatment is now by doing laser ablation with either Carbondioxide laser or Erbium :YAG laser. Laser will remove skin down to the dermis follow by regeneration of new healthy skin. There usually is post treatment hyperpigmentation which will eventually fade away. This condition is one of the strong reason why we should not use hydroquinone for long term treatment of melasma.

Die typische Pigmentierung von Ohren und Skleren, das Nachdunkeln des Urins und die lange Anamnese der Polyarthrose mit pathognomischen Zeichen in den konventionellen Röntgenuntersuchungen [1] ergeben die Diagnose einer Alkaptonurie. Diese seltene metabolische Erkrankung, autosomal-rezessiv vererbt, ist gekennzeichnet durch das Fehlen der Homogentisinsäure-Oxidase mit konsekutiver Akkumulation von Homogentisinsäure bzw. Ablagerung, bevorzugt im Bindegewebe, genannt «Ochronose».
In diesem Fall kam es zu einer kardiovaskulären Ochronose [2] mit typischer Ablagerung in den Arterien (inkl. Koronarien) und an der Aortenklappe. Aufgrund der schweren Aortenstenose wurde ein Aortenklappenersatz durchgeführt, intraoperativ zeigt die native Klappe eine dunkelblaue Verfärbung (Abb. 4). Literatur

    Diagnosis: Exogenous ochronosis (from hydroquinone in "Ambi") Also see Case of the Month for August 1997. Histopathology: reveals yellow-brown inclusions in the papillary dermis. These have been said to resemble "bananas". Discussion: Exogenous ochronosis is a well-recognized side effect from the use of topical hydroquinone. It is most commonly seen in darkly pigmented individuals, however, caucasians may also develop the condition. Hydroquinone is readily available in over the counter preparations and from the internet. "Ambi" cream contains the active ingredient of Hydroquinone 2%. Treatment consists of stopping the offending agent; a gradual improvement may occur over time.     INTRODUCTION
    Most patients with arthritis do not know which type they have (Mo. Med. 87: 145, 1990), and, appropriately, most cases of joint disease never get examined by a pathologist. Yet joint problems are the #1 recognized cause of occupational disability, and cause suffering comparable in prevalence and severity (though not in kind) to alcoholism and major mental illness. This unit includes a major systemic disease (rheumatoid arthritis) and several important single-organ problems. You have completed the learning objectives when you know the handout at recall level, and have examined the photos in Big Robbins or your own favorite pathology text. "Joint diseases" involve the diarthrodial (hinge, synovial) joints. (Rule: Disease does not affect non-moving joints like skull sutures or the pubic symphysis). Don't forget the major autoimmune diseases and fibromyaligia syndrome in evaluating musculoskeletal or joint pain. Anatomy: joint capsule (pain-sensitive) lined by synovium (vascular tissue with an inner layer of synovial cells) which out-pouches to form bursae.

    Imaging Findings Findings reveal degenerative changes involving the vertebrae and the intervertebral discs, which show narrowing and calcification. Diagnosis Ochronosis Discussion Ochronosis is the bluish-black discoloration of certain tissues, such as the ear cartilage and ocular tissue, seen with alkaptonuria, a metabolic disorder.
    It was defined by Virchow who described histologically the connective tissue in alkaptonuria, given the cartilage's ochre, or yellow, hue under the microscope. Alkaptonuria is a rare, autosomal recessive, metabolic disorder caused by deficiency of homogentisic acid oxidase, the only enzyme capable of catabolizing homogentisic acid. This deficiency results in accumulation and deposition of homogentisic acid in cartilage, causing the characteristic diffuse bluish-black pigmentation. Exogenous ochronosis, in which bluish-black pigmentation of cartilage is noted iatrogenically by exogenous agents, such as phenol, trinitrophenol, benzene and hydroquinone.
    In alkaptonuria, the accumulation of HGA inhibits collagen cross-linking by affecting a crucial enzyme in collagen synthesis, leading to a diminution of structural collagen integrity. This results in ochronotic arthropathy, which occurs in men aged in their fourth and fifth decades; women develop similar complications in their sixth decade. The larger joints are most affected with early calcification, narrowing and collapse of the intervertebral discs. In addition to joint disease, there are reports suggesting increased incidence of cardiovascular disease due to cartilaginous changes of vessel walls.

Exogenous ochronosis, a little-known side effect of creams containing hydroquinone Two patients with exogenous ochronosis, an unfamiliar side effect of hydroquinone-containing bleaching creams, are presented. This disorder is characterized by progressive darkening of the area to which the cream is applied. Histologically there is deposition of yellow-brown pigment in the dermis, in addition to degeneration of collagen and elastic fibres. The chemical composition of the ochronotic material and the pathogenesis of this paradoxical effect of hydroquinone are unknown. Hydroquinone containing creams should not be used for longer than two years. If there is no improvement within six months, application should be discontinued.
