PHYSICIAN ASSISTANT 2002 July;26(7):20-31 Abstract Attention deficit/hyperactivity disorder (ADHD) is the most common psychiatric disorder affecting the younger population. ADHD is frequently accompanied by at least 1 comorbidity: most commonly, oppositional defiant disorder (ODD), conduct disorder (CD), or both. It is difficult to accurately diagnose these disorders and, unfortunately, they often go undiagnosed and untreated. Early detection of ADHD and potential comorbidities in a child’s development enables appropriate treatment to be implemented. These disorders can be successfully managed with nonpharmacologic and pharmacologic interventions. Maximum results are obtained when family, teachers, health care providers, and psychologists work together as a team to treat ADHD, ODD, and CD. CME CATEGORY 1: 1 HOUR

ODD is a psychiatric disorder that is characterized by two different sets of problems. These are aggressiveness and a tendency to purposefully bother and irritate others. It is often the reason that people seek treatment. When ODD is present with ADHD, depression, tourette's, anxiety disorders, or other neuropsychiatric disorders, it makes life with that child far more difficult. For Example, ADHD plus ODD is much worse than ADHD alone, often enough to make people seek treatment. Conduct Disorder refers to a persistent pattern of behaviour in which the basic rights of others or major societal rules, given the particular age of the child, are violated. Conduct disorder is one of a triad of Disruptive Behaviour Disorders often referred to as challenging behaviours. It is the extreme end of the spectrum of difficult and challenging behaviour. Many children present with persistent conduct problems but not all meet the criteria for Disruptive Behaviour Disorder (DSM-IV, Diagnostic and Statistical Manual of Mental Disorders from the American Psychiatric Association). At the primary care level it is more relevant to examine the group of challenging behaviours known as Disruptive Behaviour Disorders, which include: Oppositional Defiant Disorder (ODD). Conduct Disorder (CD). Attention Deficit Hyperactivity Disorder (ADHD). These three labels describe closely related and overlapping patterns of difficult behaviour, which retain some distinctive and unique features.

Oppositional defiant disorder , ODD, is a milder form, and sometimes a precursor to conduct disorder. In contrast to children with conduct disorder, the behavior of children with ODD does not involve serious violations of others' rights. It does, however, impair the child's family, academic and social functioning. Conduct disorder . Children with conduct disorder, CD, are those children who show persistent and serious patterns of misbehavior. Not only may they indulge in frequent temper tantrums like ODD children, but they may violate the rights of others (stealing, vandalism, and aggression). These children are actively aggressive towards people (fighting with siblings and peers, sexually aggressive) and/or animals (engage in animal torture), commit vandalism, lie and steal from persons outside the home, and seriously violate society's moral codes. Children who are diagnosed with CD and who have a higher IQ are easier to treat, but are more imaginative and creative in acting out and evading detection. Some youngsters who have conduct disorder may have a learning disability and lower average verbal skills. These kids have low self-esteem, become impatient easily, and seem reckless and accident-prone. Unfortunately, children and adolescents with CD do not show remorse unless it is to lessen their punishment. In fact, they enjoy telling about what they have done.

One of the most important thing to know about ADHD is that children with ADHD are at increased risk for developing other types of behavior disorders, including Oppositional Defiant Disorder (ODD) and Conduct Disorder (CD). The reason this is so important is that when this occurs, the long-term outcomes for children are likely to be much worse than for a child who has ADHD alone. The information below is intended to provide your with an overview of the diagnostic criteria for these conditions and how they differ from ADHD.
Oppositional Defiant Disorder (ODD)

Conduct Disorder
In some ways, conduct disorder is just a worse version of ODD.  However recent research suggests that there are some differences.  Children with ODD seem to have worse social skills than those with CD.  Children with ODD seem to do better in school.  Conduct disorder is the most serious childhood psychiatric disorder.  Approximately 6-10% of boys and 2-9% of girls have this disorder.  Here is the Definition. A. A repetitive and persistent pattern of behavior in which the basic rights of others or major society rules are violated.  At least three of the following criteria must be present in the last 12 months, and at least one criterion must have been present in the last 6 months. Aggression to people and animals
  • often bullies, threatens, or intimidates others
  • often initiates physical fights
  • has used a weapon that can cause serious physical harm to others (a bat, brick, broken bottle, knife, gun)
  • physically cruel to animals
  • physically cruel to people
  • has stolen while confronting a victim ( mugging, purse snatching, extortion, armed robbery)
