  • Effect of maternal hydration on oligohydramnios: A comparison of three volume expansion methods:
    S Doi, H Osada, K Seki et al (Chiba Univ, Japan) Obstet Gynecol 92:525-529,1998.
    The effect of maternal hydration with IV isotonic fluid, IV hypotonic fluid and oral water on the amniotic fluid index in women with oligohydramnios were reported.
    Eighty-four patients with low AFI gestational age older than 35 weeks and without maternal complications were assigned randomly to 1 of the 3 treatment groups or a control group. The mean increase in AFI after hydration was significantly greater in the IV hypotonic and oral water groups compared with control group. Hydration with IV hypotonic or oral water increases amniotic fluid index in women with oligohydramnios. Maternal osomotic change had a greater direct effect on increasing amniotic fluid volume with short-term acute hydration than maternal volume expansion.
    The approach offers the possibility of avoiding labour induction solely on grounds of a reduced AFI at or past term through use of oral water administration.

    J Matern Fetal Med 7: 197-200.
    183 women under-going labour induction for isolated oligohydramnios at between 37 and 42 weeks gestation were studied. It was found that isolated oligohydramnios in otherwise normal term pregnancies does not necessarily indicate fetal compromise. Thus in most women with isolated oligohydramnios, labour induction may not be warranted.

What is amniotic fluid?
Amniotic fluid is an important part of pregnancy and fetal development. This watery fluid is inside a casing called the amniotic membrane (or sac) and fluid surrounds the fetus throughout pregnancy. Normal amounts may vary, but, generally, women carry about 500 ml of amniotic fluid. Amniotic fluid helps protect and cushion the fetus and plays an important role in the development of many of the fetal organs including the lungs, kidneys, and gastrointestinal tract. Fluid is produced by the fetal lungs and kidneys. It is taken up with fetal swallowing and sent across the placenta to the mother's circulation. Amniotic fluid problems occur in about 7 percent of pregnancies. Too much or too little amniotic fluid is associated with abnormalities in development and pregnancy complications. Differences in the amount of fluid may be the cause or the result of the problem. What is hydramnios?

Low amniotic fluid (oligohydramnios)

What is amniotic fluid? Amniotic fluid is the stuff that surrounds your baby as he grows in the amniotic sac in your uterus during pregnancy, and it serves some important purposes. It cushions your baby against trauma (if you fall down, for example) and keeps the umbilical cord from being compressed (which can reduce your baby's oxygen supply). It protects both the baby and the uterus from infection, provides a zero gravity environment for the baby to move around in, and helps the baby's digestive, musculoskeletal, and breathing systems develop properly. I am 22 weeks pregnant. About an hour after I had sex a few
weeks ago, I noticed a gush of liquid. It wet my underwear
leftover sperm that was still in there, eventough I had
"cleaned up" afterward. But I was reading about
amniotic leakage today, and am a little scared. I did have

With hospital bed rest and IV fluids, the level rose above seven. Ultrasounds did not show any obvious disorder. The doctor said that the level of fluid was mostly controlled by the baby, not my wife's fluid intake. If so, why did the IVs raise the fluid levels? Why does bed rest help with the levels? Measurement of the volume of amniotic fluid is extremely difficult and subjective assessment is still used by many. Measurement of the depth of pools of amniotic fluid gives a semiobjective method of assessment either in the form of amniotic fluid index or just as maximum pool depth. The commonest causes of oligohydramnios are premature rupture of membranes, intrauterine growth retardation renal tract abnormalities and major syndrome complexes. Fetal ultrasound usually demonstrates the renal tracts with some accuracy although oligohydramnios does prevent high quality demonstration of the kidneys. It will usually exclude obstructive renal disorder. Renal agenesis can be diagnosed but the level of certainty of the diagnosis is hampered by the lack of amniotic fluid.

There are four categories of amniotic fluid:
  • Oligohydramnios . Too little amniotic fluid.
  • Pockets seen greater than 1 cm in diameter (normal).
  • Adequate fluid, seen everywhere between the foetus and uterine wall (normal).
  • Polyhydramnios . Too much amniotic fluid. Measurement is commonly taken by using an ultrasound to determine the Amniotic Fluid Index (AFI). Oligohydramnios When a woman is said to have too little amniotic fluid she has oligohydramnios. This is defined as having less than 200 ml of amniotic fluid at term or an AFI of less than 5 cm. This means that during an ultrasound the largest pocket of fluid found did not measure 1 cm or greater at its largest diameter. It is clinically very hard to prove prior to delivery. After the birth, examining the placenta for the presence of amnion nodosum on the placenta is highly correlated with oligohydramnios. Depending on when the woman is diagnosed with oligohydramnios, there are different complications to look for, although the majority of women diagnosed will not have problems.

    Polyhydramnios is a high level and oligohydramnios is low level of amniotic fluid. Description Amniotic fluid is the liquid that surrounds the developing fetus during pregnancy Causes and symptoms Polyhydramnios, also referred to as hydramnios, can have any one of a number of causes related either to an underlying maternal or fetal condition. Maternal diabetes, which is associated with a macrosomic (enlarged) fetus, is a common cause. The medication lithium, used to treat depression, can also increase amniotic fluid levels. Twin gestations are prone to polyhydramnios. Infections passed from mother to fetus such as rubella , cytomegalovirus, and toxoplasmosis , can also result in damage to the fetus and elevated amniotic fluid levels. Fetal abnormalities, including many that are life-threatening or lead to a significant impairment in the quality of life, are found in up to a quarter of all patients. For this reason, the initial finding of excess amniotic fluid should be followed by thorough diagnostic studies to determine the cause and the prognosis. Because fetal swallowing is a major factor in amniotic fluid removal, fetal abnormalities that prevent fluid uptake should be investigated. These include gastrointestinal obstructions such as

    Oligohydramnios What is oligohydramnios Oligohydramnios refers to a condition where there is significantly less amniotic fluid around the foetus. This disorder indicates some abnormality in the pregnancy. An abnormality that affects the production of urine in the foetus may be linked to the development of oligohydramnios.
    How is this condition diagnosed and treated The size of the abdomen could indicate the existence of this condition. A suspiciously small abdomen could indicate oligohydramnios. The diagnosis can be confirmed by doing an ultrasound. If a woman has oligohydramnios, depending on the woman’s health and pregnancy records, the doctor may recommend one or all of the following:
    • Change in the mother’s diet (increase in fluids, nutritive foods, etc); Complete bed rest (if intrauterine growth retardation is suspected); Additional tests to determine if there are any genetic deformities.

    How is it harmful If the term of the pregnancy exceeds forty-one weeks, oligohydramnios is a risk factor and will influence the decision to induce labour.
