Information
The fetus floats in the amniotic fluid. During pregnancy the amniotic fluid increases in volume as the fetus grows. Amniotic fluid volume is greatest at approximately 34 weeks of gestation, when it averages 800 ml. Approximately 600 ml of amniotic fluid surrounds the baby at full term (40 weeks gestation ). This fluid is constantly circulated by the baby swallowing and "inhaling" existing fluid and replacing it through "exhalation" and urination. Amniotic fluid accomplishes numerous functions for the fetus, including:
  • Protects from outside injury by cushioning sudden blows or movements Allows for freedom of fetal movement and permits symmetrical musculoskeletal development Maintains a relatively constant temperature for the environment surrounding the fetus, thus protecting the fetus from heat loss

The causes of oligohydramnios are not completely understood. What you needto know oligohydramnios is the term for too little amniotic fluid.
http://www.modimes.org/188_1033.asp
Amniotic Fluid - Oligohydramnios What you need to know:
Oligohydramnios is the term for too little amniotic fluid. This condition affects about 8 percent of pregnancies. It can develop at any time in pregnancy, although it is most common in the last trimester. About 12 percent of women whose pregnancies last two weeks beyond their due date develop oligohydramnios as amniotic fluid levels naturally decline. Oligohydramnios is diagnosed with ultrasound
The most important known cause of oligohydramnios early in pregnancy is birth defects in the baby (often involving the kidneys or other parts of the urinary tract ) and ruptured membranes. The effect of oligohydramnios on the baby depends on the cause, the stage of pregnancy in which the problem occurs, and how little fluid there is.
  • In the first half of pregnancy, too little amniotic fluid is associated with birth defects of the lungs and limbs and increases the risk of miscarriage preterm birth and stillbirth
  • When oligohydramnios occurs in the second half of pregnancy, it is associated with poor fetal growth.
  • Near delivery, it can increase the risk of complications during labor and delivery.

oligohydramnios This is too little amniotic fluid. The normal range is 400-1500 ml. It is defined clinically as the absence of fluid surrounding the fetus. On ultrasound scanning it is defined as a pool depth of amniotic fluid of less than 2 cm - the normal range is from 2 to 10cm.

INTRODUCTION Background: The amniotic fluid that bathes the fetus is necessary for its proper growth and development. It cushions the fetus from physical trauma, permits fetal lung growth, and provides a barrier against infection. Normal amniotic fluid levels vary; however, the average volume increases with gestational age, peaking at 800-1000 mL, which coincides with 36-37 weeks’ gestation. An abnormally high level of amniotic fluid, polyhydramnios, alerts the clinician to possible fetal anomalies. Inadequate levels of amniotic fluid, oligohydramnios, results in poor development of the lung tissue and can lead to fetal death.
In pregnancies affected by polyhydramnios, approximately 20% of the neonates are born with a congenital anomaly of some type; therefore, the delivery of these newborns in a tertiary care setting is preferred. This article presents the causes, outcomes, and treatments of polyhydramnios and oligohydramnios, as well as their effects on the developing fetus and neonate.
Pathophysiology: Rupture of the membranes is the most common cause of oligohydramnios. However, because the amniotic fluid is primarily fetal urine in the latter half of the pregnancy, the absence of fetal urine production or a blockage in fetus’ the urinary tract also can result in oligohydramnios. Fetal swallowing, which occurs physiologically, reduces the amount of fluid, and an absence of swallowing or a blockage of the fetus’ gastrointestinal tract can lead to polyhydramnios.

Disorders of Amniotic Fluid Of all of the modern miracles of science, we still don't know where amniotic fluid really originates. We know that the fluid after a certain point does contain fetal urine, but how can we explain it prior to the baby's ability to make urine? Other amazing facts include that the amniotic fluid continually replaces itself at the rate of every three hours. That said, we've been trying to define what is normal amniotic fluid and what is abnormal. There are four categories of amniotic fluid:
  • Oligohydramnios
  • Pockets seen greater than 1 cm in diameter (normal)
  • Adequate fluid, seen everywhere between the fetus and uterine wall (normal)
  • Polyhydramnios This measurement is commonly taken by using an ultrasound to determine the Amniotic Fluid Index (AFI). The most recent studies say that the

    Description
Amniotic fluid is the liquid that surrounds the developing fetus during pregnancy
    Causes and symptoms
Polyhydramnios, also referred to as hydramnios, can have any one of a number of causes related either to an underlying maternal or fetal condition. Maternal diabetes, which is associated with a macrosomic (enlarged) fetus, is a common cause. The medication lithium, used to treat depression, can also increase amniotic fluid levels. Twin gestations are prone to polyhydramnios. Infections passed from mother to fetus such as rubella , cytomegalovirus, and toxoplasmosis , can also result in damage to the fetus and elevated amniotic fluid levels. Fetal abnormalities, including many that are life-threatening or lead to a significant impairment in the quality of life, are found in up to a quarter of all patients. For this reason, the initial finding of excess amniotic fluid should be followed by thorough diagnostic studies to determine the cause and the prognosis. Because fetal swallowing is a major factor in amniotic fluid removal, fetal abnormalities that prevent fluid uptake should be investigated. These include gastrointestinal obstructions such as

    What is amniotic fluid?
Amniotic fluid is an important part of pregnancy and fetal development. This watery fluid is inside a casing called the amniotic membrane (or sac) and fluid surrounds the fetus throughout pregnancy. Normal amounts may vary, but, generally, women carry about 500 ml of amniotic fluid. Amniotic fluid helps protect and cushion the fetus and plays an important role in the development of many of the fetal organs including the lungs, kidneys, and gastrointestinal tract. Fluid is produced by the fetal lungs and kidneys. It is taken up with fetal swallowing and sent across the placenta to the mother's circulation. Amniotic fluid problems occur in about 7 percent of pregnancies. Too much or too little amniotic fluid is associated with abnormalities in development and pregnancy complications. Differences in the amount of fluid may be the cause or the result of the problem.
