Osteoporosis
Osteoporosis is a disorder where the bones become weakened by loss of substance (osteopenia), leading to an increased risk of broken bones (fractures) with minimal trauma.
Symptoms
The thinning of the bones does not, in itself, cause much in the way of symptoms. It does, however lead to a risk of broken bones without much of an injury. In fact the bones of the spine can sometimes collapse without any obvious cause. When the bones are significantly thinned (low in bone mass) people who fall are more likely to break their wrist, hip, or other bones. A cough or a sneeze is more likely to cause a fracture of a rib or the partial collapse of one of the bones of the spine (vertebra). Any bone is more at risk with osteoporosis. Osteoporosis affects people in a number of ways:
  • Pain . The broken bones (fractures), which can happen without any obvious cause, can lead to severe pain, which lasts for quite a number of weeks.
  • Financial . People (especially the elderly) who develop a fracture become more dependent on others to look after them. It may result in an independent person needing long term support at home, or even having to go into a nursing home.
  • Mortality . With a 20% increase in mortality in the first year after a broken hip, the mortality is greater than that of cancer of the neck of the womb (Cancer of the cervix).

Osteoporosis is the progressive loss of bone density. It is often called the "silent killer" because the condition can often go undetected until it is severe and fractures begin to occur.
Sadly, if undetected, the degree of bone loss can leave a person disabled and in a great deal of pain. Bone density is not a subject that many people think about on a daily basis, and so this "silent thief" can steal much of the calcium and mineral strength from your bones.
Most commonly a loss in bone density is associated with changes women experience during menopause as the decreased hormonal levels affect the calcium balance in the blood and bone mass is lost as a result (Marchigiano, Sept./Oct. 1999).
Hormone replacement therapy is used to treat both menopausal symptoms and the osteoporotic changes and associated bone loss. This can happen too if other endocrine disorders are present in either male or female patients, since the endocrine system controls the levels of the hormone calcitonin which, in turn, affects the level of calcium in the body.

edicine fascinates me because it constantly teaches me something new. Back in the 1850's, the German philosopher Goethe said, "what you know about you see." Once you know about something you start seeing it everywhere, and this is certainly true about medicine and nutrition. I want to increase your understanding of diet and nutrition so you start seeing things differently, especially when it comes time to order your next meal in a restaurant or when you are pushing your cart down the supermarket aisles trying to decide what to bring home for dinner. Nutritional Awakening It began when I was on the cardiovascular anesthesia service (a service that deals with people's hearts and blood vessels). One evening I was making a round to see a patient who was scheduled for a four-vessel coronary by-pass in the next few days. The man had been out on a day-pass with his family before surgery, quite a common practice and had returned to the hospital rather late. The nurse notified me that there were several blood tests - missing from his chart, so we promptly performed another blood draw and -brought it to the nurse's station. I let it sit there while I got busy with some work in the ward. An hour and a half later I went back to the nurse's station to take the blood tube down to the laboratory. What I saw in the blood tube gave me a rude awakening as to the condition of this patient's coronary health.

Web posted at: 11:52 a.m. EDT (1552 GMT) WASHINGTON (CNN) - The National Osteoporosis Foundation, in collaboration with a broad, multi-disciplinary coalition of clinical experts, is expected to announce Thursday the first official guidelines to help prevent, diagnose, and treat osteoporosis in post-menopausal women. The guidelines focus on diet, exercise and drug treatments. They are intended for physicians and offer concise recommendations in regards to prevention, risk assessment, diagnosis and treatment. They recommend counseling all post-menopausal women at high risk of developing the disease and suggests using bone mineral density as the standard form of testing to determine a woman's risk and severity of the disease. The guidelines recommend patients get at least 1200 milligrams of calcium and 400-800 IU (international units) of Vitamin D daily. They recommend women do regular weight-bearing or strength-training exercises to reduce the risk of falls and fractures and avoid drinking alcohol and smoking.

Often we speak of healthy bones and strong teeth, and most of us remember our parents' admonition to “drink your milk.” Though we may have resisted then, as adults we are now more aware of the need for calcium, and perhaps calcium supplements.
An adequate intake of calcium can help to prevent the bone loss associated with osteoporosis, a condition where the structure, or matrix, of bone is no longer as solid and strong as it should be. The bone matrix is composed of both a mineral component that includes both calcium salt and phosphate salt , and an organic component that is largely collagen fibers. The calcium and phosphate salts combine to form hydroxyapatite crystals and give bone its amazing tensile strength.
Bone matrix resembles a sponge when viewed through a microscope, with “stronger” tissue densest on the external surface of all bones, and with “softer” or less dense tissue towards the middle of some bones.
The mineral component makes up 60 to 70 percent of mature bone matrix, and the remainder is basically collagen and water. The mineral component provides strength and the organic component provides flexibility to bone (Alvioli, 1993).
Calcium Deficiency
The mineral component of the bone matrix is intimately related to the levels of the minerals in the circulating blood. The regulation of mineral levels is such that if a person experiences a serum calcium deficiency, the bones surrender calcium to maintain the amounts needed in the blood for other body functions. Osteoporosis or "softening of the bones" is a major health problem - affecting over 20 million Americans. This condition, which, results in loss of bone density, afflicts half of all women over the age of fifty and one - third of men over the age of seventy - five . The risk of breaking a bone increases and , indeed, people with osteoporosis sustain approximately 1.2 million fractures annually in this country. The cost to the nation in medical bills and lost wages is in the billions. of dollars. An understanding of osteoporosis requires knowledge of body metabolism, calcium regulation, hormones and vitamins , and skeletal structure. This can be very complicated and scientific, but I will try to simplify this as much as possible with easy to understand analogies - at the risk of over - simplication. Calcium and Bone We all know that we get calcium from milk and that calcium goes into the bones to make the skeketon strong. In fact, an orthopedist would probably believe that calcium ,the mineral, exists solely to build bones. An endocrinologist or a cardiologist, on the other hand,would perhaps consider the skeleton to exist only to provide calcium to be used for various important bodily functions. Premature bone initially is made up of soft tissue called collagen. Collagen contains cells that form bone - OSTEOBLASTS. When immature bone is laid down by osteoblasts, calcium and another mineral,phosphate, combine to form a crystal - like substance . These calcium crystals are incorporated into the tissue - eventually forming hard , mature bone. Bone formation begins in utero and continues for nearly two decades. Calcium intake is very important during these years of development. Bone mass increases in childhood and peak bone mass is obtained by one's mid - twenties. Peak bone mass refers to the point when a bone will be at its maximum density. Peak bone mass is directly influenced by adequate intake of dietary calcium. Women who take in inadequate dietary calcium will have a lower peak bone mass and thus, a higher risk of deveolping osteoporosis later in life.

Bone strength reflects the integration of two main features: bone density and bone quality. Osteoporosis is the most common of the bone diseases that affect Americans. Although it is the underlying cause of most fractures in older people, the condition is silent and undetected in many cases until a fracture occurs. Osteoporosis is a major health risk for 28 million Americans. In the United States today, 10 million individuals already have osteoporosis and 18 million more have low bone mass, placing them at increased risk for this disease. American women are four times more likely to develop osteoporosis than men. One out of every two women and one in eight men over 50 will have an osteoporosis-related fracture in her or his lifetime. Osteoporosis may be attributed to three factors: (1) accelerated bone loss at menopause in women or as men and women age; (2) suboptimal bone growth during childhood and adolescence resulting in failure to reach peak bone mass; and (3) bone loss secondary to disease conditions, eating disorders, or certain medications and medical treatments.
