Patellofemoral pain syndrome (PFPS) is a general term meaning pain behind the kneecap. Generally, it is an overuse disorder form activities involving running, cycling, jumping and kneeling.
What Happens? Normally, there is a groove at the end of the thigh bone (femur) which allows the kneecap (patella) to slide smoothly across it as the thigh muscles (quadriceps) contracts to straighten the knee. Repetitive bending and straightening of the knee can cause the undersurface of the patella to get irritated and inflamed as it rubs over the end of the femur. If the patella does not align properly in the groove as the quadriceps contracts to extend the leg, wearing of the undersurface of the patella can occur. Predisposing Factors
  • Overpronated or flat feet. "Knock knees" Weak thigh muscles
These conditions predispose PFPS because of the alignment of the knees in relation to the hips and ankles.

What are Range of Motion Limitations? Range of motion (ROM) limitations, or contractures, can result from a variety of orthopedic or neurological conditions. A contracture occurs when connective tissue (ligaments, tendons, joint capsule) becomes scarred or when muscle tissue becomes shortened. This can occur at the knee, elbow, wrist or ankle and result in a lack of full range of motion. For a variety of reasons, some patients do not respond adequately to the traditional exercise and stretching regimens used in the clinic to increase range of motion. However, studies indicate that a low load, prolonged force placed upon the tissue can cause remodeling of the tissue fibers in a lengthened position. Patients who often have difficulty achieving their full range of motion fall into two categories: those having experienced trauma or surgery and those suffering some type of neurological disorder. Some of the more common conditions that result in a reduction of motion are: Trauma
