Patellofemoral Syndrome
The patella is one of three bones, along with the tibia (shin bone) and femur (thigh bone), that makes up the knee joint. All of these bones are covered with a layer of articular cartilage at points where their surfaces come into contact. Furthermore, the patella is enveloped by a tendon. This tendon connects the quadriceps muscle of the thigh to the shin bone (tibia) below the knee. The patella is important functionally because it increases the leverage of the knee joint. From a mechanical perspective, the patella allows for an increase of about 30% in strength of extension (kicking) of the leg at the knee joint. There are a few common problems associated with the patella that can cause problems. The most common disorder is known as chondromalacia patellae. Chondromalacia occurs due to a problem of the articular cartilage on the undersurface of the kneecap. Often this problem is due to degeneration of the deepest layers of cartilage, causing ‘blistering’ of the surface cartilage. This pattern of cartilage damage is distinct from the degeneration seen in osteoarthritis, and the damage from chondromalacia is thought to be capable of repair, unlike that seen in osteoarthritis.

Let's define exactly what we mean here: Corrective rehabilitation - Functional disorder only In the ealy stages - before surgery is contemplated - a great deal can be done to improve the situation but it is likely to be a lifelong process, because the body will just go back to the same stance and muscle imbalances if the programme is stopped. However, diligent work may totally put off the spectre of surgery.
  • Firstly, get an evaluation from an orthotist or an osteopath regarding flat feet, leg length, femoral (thighbone) or tibial (shinbone) rotation and pelvic an spinal tilt. If an orthotic is prescribed give it a decent try. Avoid just buying off-the-shelf inserts unless they are very well made of a firm material with good arch support. Avoid high-heeled shoes. If your feet are a size or more different - buy two pairs and throw one shoe from each pair away. Then build up your fitness in a simple way - walking on flat surfaces with good supportive shoes and the inserts if applicable. Take care with your gait while walking.

However, if you have had symptoms for a long time, crutches and medications may be in order. During this 2-4 weeks of "rest", there are some things you can do to speed up your recovery, reduce pain, and help prevent problems in the future. You should consider a Goode Wrap.

Description:
First described by Sir James Paget in 1877, Paget's Disease or Ostitis deformans is a bone disorder commonly seen in the geriatric population, affecting up to 4% of the population over 50 years of age. It is a chronic and slow to progress disease of disorganized bone remodeling, and the process is often asymmetric. As implied by the name Ostitis Deformans, the involved bone is severely deformed and enlarged, suggesting an inflammatory etiology.
Appropriate Care:
Depends on severity of the disease. Not all patients will require treatment. Pain due to associated arthritis can be treated with NSAIDS. Avoid narcotics. Cane or walker may be used for gait stabilization. Indications for treatment include bone pain, high-output cardiac failure, and prevention of pathological fracture.
