Pectus excavatum Pectus excavatum is the most common congenital chest wall deformity, also known as "funnel chest" or "sunken chest." It involves a depression of the anterior chest wall and the sternum. It is often present at birth and is usually noted within the first year of life. Pectus excavatum tends to run in the family and occurs more frequently in boys than girls. The cause is still unknown. There is an association of pectus excavatum with other musculoskeletal abnormalities such as Marfan's syndrome or scoliosis. The extent of deformity and symptoms from pectus excavatum range from mild to severe. A pectus is usually well tolerated in infancy or childhood. An operation to correct the deformity may be indicated if there are severe symptoms related to lung or heart functionparticularly if diagnostic tests document these limitations. Cosmetic and psycho-social considerations may also lead to a decision to operate on a child with pectus. The decision to perform a corrective procedure for pectum excavatum must be made carefully, allowing time for consideration of the risks and benefits by both parents and children. The operation The operative management of pectum excavatum has evolved over time. Since the 1940s, pectus excavatum has been treated by removal of the cartilage connecting some of the ribs to the sternum along with cutting through part of the sternum to allow it to be pulled forward. Over the last decade a less invasive approach to pectus excavatum has been developed.

Indications:
Repair of pectus excavatum may be recommended for: improved appearance (cosmetic repair), improved breathing (respiratory function), or to stop compression of the heart. Description of the operation: The repair of pectus excavatum has changed over the past several years. We now use the minimally invasive Nuss procedure. The old procedure required a large incision on the front of the chest and removal of 6-8 sets of cartilage. The much less invasive Nuss procedure is described here: While the child is deep asleep and pain-free (using general anesthesia), 2 small incisions are made over the sides of the chest and a curved titanium bar is passed behind the breastbone (sternum). The deformed sternum is elevated when the curved bar is turned over. The placement of the bar may be aided by the use of a small scope placed into the chest to monitor the bars progress. Once the bar is in place it is sutured under the muscle so it will not move. Rarely a chest tube may be placed to reexpand the lung if the lining of the lung is entered. Expectations after surgery: The child will have some pain after the procedure but this will be controlled with either an epidural catheter or a PCA (patient controlled analgesia) pump. The child is usually in the hospital 3 to 4 days after the surgery. The metal bar is removed 2-3 years later through a small skin incision. This removal is usually done as an outpatient. After the repair the child will be on restricted activity for a time period and will gradually return to full activity.

After reading an article in the local paper about a new surgical procedure, Ryan began researching it on the Internet. The topic isn't exactly what you'd expect a 14-year-old boy to be interested in, but Ryan had quit playing baseball, was short of breath and tiring easily. Maybe his symptoms were caused by the condition this surgery was intended to correct. Ryan was born with pectus excavatum, or "sunken chest," a congenital defect that can cause shortness of breath and possibly more serious heart and lung problems. Last fall, Ryan had an EKG and X-rays taken. They showed Ryan's heart was displaced. "All we could think of was permanent damage," Ryan's mother, Laurie, recalls. "We thought Ryan wouldn't be able to play baseball or run or any of those things." Up until that point, neither Laurie nor Ryan had considered surgery to correct the problem. Traditional surgery takes four to six hours and involves cutting the cartilage joining the ribs to the sternum and reshaping the breastbone. Patients can lose some blood, and recovery can be long and painful.

Definition
Pectus excavatum is a chest wall deformity in which the central portion of the chest, or sternum, is pushed toward the spine.
How often is it seen?
Funnel chest is the most common deformity of the chest and is seen 3 to 10 times more often in boys than girls. It is usually diagnosed in the first year of life and rarely improves without therapy.
Why is it a problem?
There are two orders of suffering involved associated to PE: the physiological and the social.
Physiological or medical problems:
The sternum or breastbone is so far displaced in severe PE as to push the heart to one side or compress it while the lungs are compressed in a smaller chest cavity.
Social problems:
These affect each individual differently, in severe cases the deformity will significantly alter the child's life. Self-esteem issues are very prominent and you may notice that as they come closer to adolescence many activities will be abandoned if they involve removing their tops. What is the therapy? Mild PE can be improved with exercise and posture programs but severe cases require surgery for correction.

Considerations: Pectus excavatum is a congenital abnormality that can be mild or fairly marked. The child typically has a depression in the center of the chest over the sternum, and this may appear quite deep. It is caused by excessive growth of the costosternal cartilage (the connective tissue joining the ribs to the breastbone), with resulting inward deformity of the sternum.
If pectus excavatum is severe, it may affect the underlying organs (heart and lungs) leading to exercise intolerance or other problems. In addition, the cosmetic appearance of a pectus excavatum may pose a psychological difficulty.
Pectus excavatum may occur as the only abnormality, or in association with other syndromes.
Common Causes:
  • Isolated defect (i.e., not associated with other problems)

Pectus Excavatum:
Pectus excavatum is the medical label for the condition sometimes called "funnel chest", a depressed or sunken breastbone (sternum). It is one of the most common human bone deformities doctors suggest around 1 of every 400 people have some degree of PE. The deformation ranges from a very mild, centered "dent" in mid-chest, with few symptoms to an extremely deep depression forcing the heart over to one side and impairing breathing and heart function and making any exertion painful. Pectus carinatum is a related condition in which the breastbone and ribs protrude more than normal. As with PE, there's a wide range of degrees of severity with PC. tendency to develop PE or PC seems to "run in families". Many people with pectus discover that a parent, often their father, had a mild case. It is also a feature in congenital conditions like Marfan Syndrome and Ehlers-Danlos Syndrome. Pectus may also occur as "isolated" that is, not part of a system-wide syndrome but something that's happened on its own. Both PE and PC reflect an underlying problem with connective tissue formation.

Description While the child is deep asleep and pain-free (using general anesthesia), an incision is made over the breastbone (sternum). The deformed cartilages are removed and the rib lining is left in place to allow regrowth of the cartilages. An incision is made in the sternum and it is repositioned. A rib or metal strut may be used to stabilize the sternum in normal position until healing occurs in 3 to 6 months. A temporary chest tube may be placed to reexpand the lung if the lining of the lung is entered.
Metal struts are removed 6 months later through a small skin incision under the arm. This procedures is usually done as an outpatient. Most repairs are done between 18 months and 5 years of age. Indications Repair of pectus excavatum may be recommended for:
  • improved appearance (cosmetic repair)
  • improved posture
  • improved breathing ( respiratory function)
