What Is Pediculosis?
Pediculosis is a common dermatitis caused by infestation by one of several varieties of lice: 1) pediculosis pubis (pubic louse), caused by Pthirus pubis (pubic louse, "crabs"); 2) pediculosis corporis (body louse), and 3) pediculosis capitis (head louse). The infestation can be diagnoses by a number of symptoms such as:
  • Itching with excoriation (deep scratches). Pruritus (itching) is generally intense and very
  • uncomfortable. It is this discomfort that causes the deep scratching (excoriations).
  • Pruritic infected papules are common and may be seen.
  • Nits (eggs) can be found on hair shafts and the lice themselves are often found on the skin or on clothing.
  • Occasionally, sky-blue macule (flat bumps) occur on the inner thighs or lower abdomen signifying pubic louse infestation.
  • The infestation can involve the scalp, trunk or pubic area. ]
  • It usually occurs where people are living in overcrowded situations.
  • It is also common after close intimate contact, shared clothing, sleeping in the same bed or sitting on an infested toilet seat. Head lice are commonly spread by sharing combs, brushes or hats.
The Diagnosis
The diagnosis can be confirmed by the characteristic symptoms and appearance of the excoriations and/or finding the lice themselves or their nits. Absolute confirmation is by scraping off a nit or louse and directly visualizing them under the microscope.

Treatment Failures Failures in the treatment of head lice can be due to four causes.
  • Inadequate application of product
  • Lice are resistant to the insecticide used
  • Failure to retreat to kill nymphs newly emerged from eggs
  • Reinfection By approaching the problem systematically, you can usually work out which aspects are significant in a particular case. Each case is different.
    Cause 1. Inadequate application of product. In this problem the chemical does not come into contact with the lice for long enough at a high enough concentration. This is usually due to failure to cover all hairs adequately with the chemical. Cover from roots to tip. This problem occurs most commonly in treating pediculosis in long hair. One way of ensuring that coverage is adequate is to comb the product in with a normal comb. The end result is a head with every hair coated from root to tip, and lying parallel with its neighbours. The "parallel" aspect ensures that the product has been combed through and makes subsequent use of a fine tooth comb easier. The product should be on the hair for 20 minutes.

    HEAD LICE (PEDICULOSIS) GUIDELINES FOR CONTROL General Information Head lice are tiny insects that live on the human scalp; they feed on human blood. Although head lice are transmissible, the potential for epidemic spread is minimal. Head lice are not known to transmit infectious agents from person-to-person, but a sensitivity or allergic reaction to the saliva of the bites causes itching. Secondary infections can result from excessive scratching. The greatest harm associated with head lice is related to the misguided use of caustic substances to eliminate the lice (Pollack, 2000). Signs/Symptoms Most infestations in school-age children are asympotamic. When infestations are sympotomatic, itching is the primary symptom. Persistent scratching of the head and back of the neck should be cause for evaluation for suspected infestation. Depending on the degree of sensitization, erythematous macules, papules or hive-like reactions may be seen. While uncommon, secondary bacterial infections may occur. Because lice are more active at night, sleeplessness is another potential symptom.

    An infestation of head hair, the hairy parts of the body, or clothing, with adult lice, nymphs, or nits (eggs). Pubic, or "crab" lice, usually infest the pubic area, eyelashes, or armpits; Head lice infest the scalp. Body lice infest the body and can be identified clinging to the seams of clothing. The head louse is a wingless insect 2mm in length. The parasite lives close to the scalp, and is dependent upon human blood to exist. Lice can only survive up to two days, but usually only a few hours, away from the scalp. The main symptom is itching. Transmission occurs by direct contact with a person with a live infestation, or direct contact with their personal belongings that are harboring lice.
    Incubation Period
    Optimally, eggs hatch in a week, and the resultant lice are capable of multiplying in 8-10 days. The typical adult louse lives 20-30 days, laying 4-5 eggs a day.
    Period of Communicability
    As long as live lice remain on an infested person and/or eggs (nits) in hair are within a ¼" from the scalp.

    What is pediculosis?
    Pediculosis is an infestation of lice. Body lice infestations occur primarily on clothing, especially along the seams of inner surfaces and the adjacent area of the skin. Pubic/crab lice infest hairy parts of the body including the groin, eyebrows, eyelashes and facial hair. Both body lice and pubic/crab lice feed on human blood and can cause severe local itching. Who gets pediculosis? Anyone can become infested with body lice or pubic/crab lice, regardless of age, sex, race, or standards of personal hygiene. Body lice infestation are usually found in people living in crowded, unsanitary conditions where clothing is infrequently changed or laundered. Pubic/crab lice infestations are mainly found among sexually active individuals. How are body lice and pubic/crab lice spread? Body lice are spread from person-to-person through close direct contact with a lice infested area of a person or through sharing of unlaundered clothes or bedding that had contact with an infested person. While other means are possible, pubic/crab lice are usually spread through sexual contact. Since nits (louse eggs) must be laid by adult lice and need warm temperatures to hatch, the chances of nits being spread from person-to-person are minimal.
