Etiology

Caused by an auto-immune response that generates antibodies against desmonglein-3, a protein that is essential for the attachment of skin cells. Affected cells are keratinocytes in the basal and immediate suprabasal layers of the stratified squamous epithelia. The desmoglein-directed antibodies are believed to directly interfere with the adhesion properties of the protein. Top of Page
Pathogenesis
  • Typically starts as sores within the mouth, but may also appear as sores on the skin. Severe blistering of the skin and mucous membrane is fatal without treatment.

  • Affects an estimated 10,000 to 15,000 people in the U.S., Europe, and Japan Higher prevalence in the Ashkenazi Jewish population. Characterised by progressive and severe blistering, primarily on the skin and mucous membranes.

Dear Doctor Richards, I am a new subscriber to your VETINFO, and having just read through many
of your articles, I must tell you that I already enjoy it a lot. I want to tell you about my dog - I am sure all people want to do that, but anyway... Five years ago I bought my two dogs in Thailand when me and my husband comfortable. Due to the huge weight increase I noticed that he started to limp, yep, back on the operating table he went and the vet found out that he also had hip displasia (grade 3-4) so I started to give him "cosequin" - double strength. By then as I said he was quite comfortable with the amount of cortisone (5mg per day) and I put him on Reduced diet, within 3 months by walking him 10 times a day, short trips as his legs weren't that strong, he lost 9 kilos and is now back to his normal weight (31 kilos). He is a really happy dog and I am extremely close to him. The cortisone keeps the pemphigus down, occasionally it will bother him and the skin will fall off, but it will only take about a week, then it is all healed up again. The Cosequin makes his legs stronger, even though they are not that strong, the heartworm treatment that they both get every month keeps away the worms and my vet has been expanding his clinic TWICE since I moved here with doggies!!!!!

Histologic clue: "the naked epidermis", i.e. the superficial cell layers of the stratum corneum are gone (due to superficial acantholysis) and the epidermis appears "nude". Confirmation of the diagnosis by direct immunofluorescence (cf. figure). Differential diagnosis: superficial impetigo contagiosa, which may look very similar histologically but contains an abundance of bacteria, immuno-stainable with anti-mycobacterium bovis BCG, which cross-reacts with many other organisms incl. bacteria. KatieDee Answer Posted By: Derm-M.D.-ASR on Wednesday, May 23, 2001 Your concern in appropraite. Pemphigus is an uncommon and potentially serious disease. I practice in a large city, but because I see it so rarely, I refer cases to a medical center hwere doctors with gretaer expereince than my own can evaluate it. I would suggest a visit to a medical center for evaluation, though perhaps your local dermatologist can provide monitoring so she won't have to make long treks too often. I don't think medications are involved as causes. Pemphigus can certainly be well-controlled nowadays, so don't be too worried by what you've read. Best.

Causes, Incidence, and Risk Factors
Pemphigus involves blistering of the outer (epidermal) layer of the skin and mucous membranes . It is an autoimmune disorder in which the immune system produces antibodies against specific proteins in the skin and mucous membrane. These antibodies produce a reaction that leads to a separation of epidermal cells (acantholysis). The exact cause of the development of antibodies against the body's own tissues (autoantibodies) is unknown. Sometimes pemphigus appears due to reactions to medications, although this is rare. Pemphigus may be a side effect of ACE inhibitors (such as Elanapril) or chelating agents (such as penicillamine).
Pemphigus is uncommon. It occurs almost exclusively in middle-aged or older people, of all races and ethnic groups. About one-half of the cases of pemphigus vulgaris begin with blisters in the mouth, followed by skin blisters. The blisters (

Cutaneous blisters and erosions with the histologic, ultrastructural, and immunofluorescence features of pemphigus occurred in 39 to 55 mice given intraperitoneal injections of IgG from patients with pemphigus and in none of 58 control mice given normal human IgG. IgG fractions with high titers of pemphigus antibodies were most effective in inducing disease, and this effect was dose dependent. Titers of circulating IgG in mouse serum closely correlated with the extent of disease induced (P less than 0.002). This study strongly supports the proposed role of pemphigus autoantibodies in the pathogenesis of pemphigus vulgaris in human beings and demonstrates that pemphigus can be passively transferred to laboratory animals.

Pemphigus Vulgaris Pemphigus is a group of rare autoimmune blistering disorders of the skin in which blisters and sores occur in skin and mucous membrane. It is a disease of immune system. The immune system produces antibodies which normally attack hostile viruses and bacteria in the blood in an effort to keep us healthy. In an affected individual however, the antibodies erroneously perceive the skin and/or mucous membrane tissue as foreign and attacks them. This produces burn-like lesions that will not heal. In some cases, these sores can cover a significant area of the skin. The disease can give a horrifying image and if not managed properly may lead to death. There is no cure for this disease. Steroids are only hope and give dramatic relief.

Actuellement, on distingue le pemphigus vrai et les pemphigoïdes. Le terme de pemphigoïde regroupe l'ensemble des dermatoses se caractérisant par des bulles différentes du pemphigus vrai. C'est le cas des phlyctènes (sortes de vésicules situées sous la peau et remplies de liquide transparent) appelées plus couramment ampoules. La caractéristique des pemphigoïdes bulleux est leur volume généralement plus élevé que celui du pemphigus. Causes
Affection rare, dont on ne connaît pas la cause avec certitude, mais pour laquelle on évoque une réaction auto-immune (l'individu fabrique des anticorps contre ses propres tissus, et dans ce cas, contre sa propre peau).
La prise de médicaments semble être parfois à l'origine de l'apparition du pemphigus. Les molécules incriminées sont les inhibiteurs de l'enzyme de conversion, la D-pénicillamine. Le facteur héréditaire, mis en évidence par la fréquence plus élevée de cette affection dans certaines populations, a également été mis en cause.
