PURULENT PERICARDITIS Primary pericardial disease is a rare problem in pediatrics. Inflammation of the pericardium may result from a number of non- bacterial microorganisms or may occur with a variety of noninfectious illnesses. Purulent pericarditis generally refers to bacterial infection of the pericardium. Regardless of the cause of pericarditis, the responses of the pericardium are limited to (1) acute inflammation, (2) effusion with or without tamponade, and (3) fibrosis with or without constriction. Pericardial disease of all etiologies occur approximately once per 850 hospital admissions. Acute purulent pericarditis is rare with a predominance of cases under 2 year of age. Male predominance have been noted in few studies; however, one review of 50 infants revealed equal sex distribution. Purulent pericarditis is associated most often with infection from another site, with hematogenous or direct spread to the pericardium. The most common concomitant site involved is the lung, especially for Staph aureus H. influenzae

Answer Pericarditis is a term which describes any inflammation of the pericardium. The pericardium is the membrane which surrounds the heart. There are two portions, an outer, fibrous sac which completely encloses the heart, and an inner membrane, which is called the serous pericardium. This is attached closely to the muscle of the wall of the heart. There is a very small amount of fluid in the sac, which prevents friction as the two surfaces slide over each other when the heart beats. Pericarditis can be caused by infections, bacterial or viral, or as a result of infection in the lungs, or trauma to the heart itself. In some cases the cause is unkown, in which case it is called idiopathic pericarditis.

    What is pericarditis?
    Pericarditis is inflammation of the pericardium, the thin sac (membrane) that surrounds the heart and the roots of the great vessels. There is a small amount of fluid between the inner and outer layers of the pericardium. When the pericardium becomes inflamed, the amount of fluid between its two layers increases, compressing the heart and interfering with its ability to function properly. Symptoms of pericarditis:
    • chest pain that:
      • can especially be felt behind the breastbone, sometimes spreading to the neck and left shoulder
      • is a sharp, piercing pain over the center or left side of the chest that increases if the person takes a deep breath a low-grade fever
      • pain when swallowing
      What causes pericarditis?
      Usually, the cause of pericarditis is unknown, but may include:
      • a viral, bacterial or fungal infection

    By DrRich What is pericarditis? The pericardium is a protective lining that surrounds the heart. Pericarditis is an inflammation of the pericardium. What causes pericarditis? Pericarditis can be caused by infection, heart attack, autoimmune disorders, chest trauma, cancer, kidney failure, or drugs. Infections that can cause pericarditis include viral infections, bacterial infections, tuberculosis, and fungal infections. Patients with AIDS frequently develop infections that produce pericarditis. Autoimmune disorders that can cause pericarditis include rheumatoid arthritis, lupus, and scleroderma. Pericarditis occurs in up to 15% of patients who have acute myocardial infarctions (heart attacks).

ETIOLOGIA La pericarditis que raramente representa un proceso que afecta al pericardio en forma exclusiva, puede originarse por diversos padecimientos de origen infeccioso o no infeccioso:
  • Causas infecciosas. Los virus son en la causa más frecuente de pericarditis infecciosa, entre ellos los enterovirus, especialmente los coxsackievirus B y los echo. En la actualidad se detecta pericarditis en los pacientes con SIDA causada por mycobacterias ( e.g Mycobacterium avium) . El diagnóstico que se hace casi siempre es el de pericarditis idiopática o no específica, aunque con frecuencia se asocia con una virosis. Otras causas infecciosas son las micóticas y en menor importancia las bacterianas. Causas no infecciosas. Tumores metastásicos o linfoma, la uremia, el trauma, el infarto del miocardio y las enfermedades del tejido conectivo. A menudo aparece después de cirugía a corazón abierto y posterior a la radiación. Es posible que algunos fármacos la causen, en particular la procainamida, la hidralazina y la difenilhidantoina.
