Pituitary structure and function and relationships between the hypothalamus and the pituitary gland are discussed in Ch. 6. Patients with hypothalamic-pituitary disorders present with some combination of (1) symptoms or signs of a mass lesion (eg, headaches, visual field defects) or (2) hypersecretion or hyposecretion of one or more pituitary hormones. Other hypothalamic functions also may be affected. Symptoms of hypo- or hyperpituitary secretion are the most common complaints of patients presenting with pituitary or hypothalamic neoplasms, but the symptoms may have other causes. A mass involving the hypothalamus or pituitary also should be suspected if the sella turcica is enlarged on skull x-ray or if neurologic symptoms and signs suggest compression of the optic chiasm (especially bilateral hemianopia). The enlarged sella may represent the empty sella syndrome

PITUITARY DISORDERS The pituitary gland has been called the "master" gland. It manages long-term growth, day-to-day functions, and reproductive processes. The gland produces eight hormones. It hangs from the base of the brain. When the gland does not work right, it may produce either too much or too little of one or more of the hormones. This causes changes elsewhere in the body. Disorders of the pituitary gland result from:
* Congenital and genetic problems: These usually affect the supply of the hormones. For example, a low supply of growth hormone slows the growth of a child. Dwarfism or other problems can result.
* Injury: A head injury may stop the gland from working.
* Impaired blood supply: When the blood supply is cut off by a growing tumor or aneurysm, the gland may not be able to function.

PITUITARY TUMOR PROGRAM
PITUITARY DIAGNOSES INDEX The pituitary is a small, bean-shaped gland located below the brain in the skull base, in an area called the pituitary fossa or sella turcica. Weighing less than one gram, the pituitary gland is often called the "master gland" since it controls the secretion of hormones. These substances have a dramatic and broad range of effects on metabolism, growth and maturation, sexuality and reproduction and other important bodily functions.
(above) MRI of normal Pituitary Gland Structurally, the pituitary gland is divided into a larger anterior region (adenohypophysis) and a smaller posterior region (neurohypophysis). The gland is connected to a region of the brain called the hypothalamus by the pituitary stalk. Directly above the pituitary gland and in front of the pituitary stalk are the crossing fibers of the optic nerves called the optic chiasm. On each side of the pituitary gland is the cavernous sinus. Through each cavernous sinus runs a carotid artery that carries blood to the brain, and important nerves that control eye movements. Because of the close proximity of the pituitary gland to these major intracranial nerves and blood vessels, as well as the vital hormonal control the pituitary gland provides, disorders of the pituitary can cause a wide spectrum of symptoms, both hormonal and neurological. Acromegaly (Growth Hormone Secreting Adenoma) PITUITARY TUMOR PROGRAM PITUITARY DIAGNOSES INDEX Acromegaly is a serious systemic condition caused in over 98% of cases by an adenoma of the pituitary gland that secretes excessive growth hormone (GH). The clinical features associated with acromegaly include the effects of GH oversecretion, and in some instances by the tumor compressing and injuring the normal pituitary gland, optic nerves and optic chiasm. Untreated acromegaly results in marked bony and soft tissue changes including an altered facial appearance (frontal bossing, prognathism), enlargement of the hands and feet, sleep apnea, and carpal tunnel syndrome. More serious problems may include accelerated cardiovascular disease, hypertension, diabetes mellitus and an increased risk of colon cancer. If the tumor develops before bone growth is completed in adolescence, gigantism is the result. Because of the serious systemic changes resulting from GH excess, treatment is essential, typically with transsphenoidal surgery Symptoms and signs may include:
  • Soft tissue thickening on the palms of the hand Enlargement of hands (ring size), feet (shoe size) and head (hat size)

This condition occurs when pituitary tissue is destroyed without undergoing pituitary surgery or radiotherapy, but there is no evidence of a pituitary tumour. The initial cause of empty sella syndrome may not be clear and may have occurred much earlier.

Antidiuretic hormone (ADH), which is also called vasopressin, helps the kidneys (and body) conserve the right amount of water. For example, when a person becomes dehydrated, more ADH is produced to help the body conserve the water it contains. Lack of ADH leads to too much excretion of water (diabetes insipidus), and too much ADH leads to excessive retention of water by the body (syndrome of inappropriate antidiuretic hormone secretion).

US FOOD AND DRUG ADMINISTRATION APPROVES SOMAVERT® FOR THE TREATMENT OF ACROMEGALY First in a new class of medicines treats debilitating hormone disease Peapack, N.J. (March 26, 2003)Pharmacia Corporation (NYSE:PHA) announced today that the US Food and Drug Administration (FDA) has approved SOMAVERT® (pegvisomant for injection) for the treatment of acromegaly in patients who have had an inadequate response to surgery and/or radiation therapy and/or other medical therapies, or for whom these therapies are not appropriate. The goal of treatment is to normalize serum IGF-I levels. Acromegaly is a serious, life-shortening disease triggered by over-secretion of growth hormone, most often caused by a pituitary tumor. This excess of growth hormone leads to overproduction of a second hormone, IGF-I (insulin-like growth factor-l), which contributes to the disabling symptoms and the long-term health problems associated with the disorder. Patients with acromegaly often suffer from headache, excessive sweating, soft-tissue swelling, joint disorders and, perhaps most striking, a progressive coarsening of facial features and enlargement of the hands, feet and jaw. Patients with acromegaly face a mortality rate two to four times higher than the average person, due to such serious long-term complications as heart and respiratory disease, diabetes mellitus and some forms of cancer.
