Brain Disorders: Post-Traumatic Stress Disorder: NARSAD-Funded Research NARSAD-Funded Research on Post-Traumatic Stress Disorder Conducted over the Last Two Years Michael DeBellis, M.D. , of the University of Pittsburgh (NARSAD Young Investigator) hypothesizes that maltreated children with posttraumatic stress disorder (PTSD) and co-existing affective disorder may have structural brain changes resulting in disruption of the prefrontal cortical-limbic circuit and characteristic deficits in skills that require attention and concentration. He will use functional magnetic resonance imaging of the brain to measure structural changes. Dr. DeBellis' prior NARSAD project found that maltreated children with PTSD who had high rates of subsequent and co-existing affective disorder showed stress system dysregulation. Frederic J. Sautter, Ph.D. , of Tulane University (NARSAD Independent Investigator) is investigating the stress hormone response in psychotic and non-psychotic post-traumatic stress disorder (PTSD). Studies suggest that PTSD and psychosis frequently co-occur. Dr. Sautter proposes to identify the components of the stress hormone axis that contribute to PTSD. He will measure functioning of this hormonal system in patients with PTSD and schizoaffective disorder, patients with PTSD and no psychosis, and healthy controls. This work may lead to more effective treatments for PTSD, and elaborate the role of the stress hormones in this illness. Lisa M. Shin, Ph.D.

Note: In children, this may be expressed instead by disorganized or agitated behavior The traumatic event is persistently reexperienced in one (or more) of the following ways: recurrent and intrusive distressing recollections of the event, including images, thoughts, or perceptions. Note: In young children, repetitive play may occur in which themes or aspects of the trauma are expressed. recurrent distressing dreams of the event. Note: In children, there may be frightening dreams without recognizable content. acting or feeling as if the traumatic event were recurring (includes a sense of reliving the experience, illusions, hallucinations, and dissociative flashback episodes, including those that occur on awakening or when intoxicated). The syndrome is a disturbing psychological condition where people relive painful memories against their will. This leads to feelings of isolation and a sense of losing control - patients sometimes turn to alcohol or other drugs as they attempt to get rid of the memories. It is also known as shell shock because it first came to prominence when soldiers in the First World War were afflicted by memories of the horrors they saw in the trenches. Dr Matthew Freidman, of the US Department for Veteran Affairs, says the experience is like living through the original event a second time.

    So you focused on trauma, especially in women and children. Help us understand what Post-Traumatic Stress Disorder is. Okay. Well, I can tell you about what it says in the DSM-IV. Which is the official Bible of the Psychiatric Association. Fear is something that we're all biologically wired to experience when we're in danger. We share this with other animals. When we perceive danger, we alert, we startle, we look around and figure out, do a quick appraisal of the situation, and we either fight or flee. That's being revised now by some researchers looking more at women who say that "fight or flight" is a little bit more the male response. "Tend and befriend" there's a tendency to kind of huddle with one's kind that you observe more in females. But, okay, fight or flight: there's a whole biology of fear that's involved. This is often the case of women and children who are in domestic situations where the cycle goes on and on. I think it's true of people in any situation of coercive control, whether you're talking about a hostage situation that goes on for a long time, whether you're talking about domestic violence or sexual child abuse ... some religious cults have this same captivity kind of situation. And then, of course, the political situations of concentration camps or political prisoners.

    Post-traumatic stress disorder (PTSD) is an extremely debilitating condition that can occur after exposure to a terrifying event or ordeal in which grave physical harm occurred or was threatened. Traumatic events that can trigger PTSD include violent personal assaults such as rape or mugging, natural or human-caused disasters, accidents, or military combat. Military troops who served in Vietnam and the Gulf Wars; rescue workers involved in the aftermath of the Oklahoma City Bombing; survivors of accidents, rape, physical and sexual abuse, and other crimes; immigrants fleeing violence in their countries; survivors of the 1994 California earthquake, the 1997 South Dakota floods, and hurricanes Hugo and Andrew; and people who witness traumatic events are among the people who develop PTSD. Families of victims can also develop the disorder. Fortunately, through research supported by the National Institute of Mental Health (NIMH) and the Department of Veterans Affairs (VA), effective treatments have been developed to help people with PTSD. Research is also helping scientists better understand the condition and how it affects the brain and the rest of the body.

    ‘Flashbacks’ in post-traumatic stress disorder are influenced by culture
    Some characteristics of post-traumatic stress disorder (PTSD), notably flashbacks, appear to be culturally influenced and are not ‘universal’ across time, according to a new study.
    1 February 2003 It has been argued that PTSD is a timeless condition that existed before it was given a psychiatric diagnostic classification (in 1980), but was described by different terms such as ‘shellshock’. Others have argued that PTSD is a new condition that has resulted from a modern interaction between trauma and culture. Published in the February issue of the British Journal of Psychiatry, this study set out to test whether one core symptom of PTSD, the ‘flashback’, has altered in frequency over time in soldiers subjected to the intense stress of combat. Random selections were made of UK servicemen who had fought in wars from 1854 onwards and who had been awarded war pensions for post-combat disorders. Their files, which contained detailed medical and military reports, were studied to evaluate the incidence of flashbacks in defined, at-risk populations. The investigation was restricted to the British Army. A total of 1856 cases were included, drawn from severe conflicts representing 10 diagnoses as follows:
