WHAT IS PRIAPISM?
What is priapism?
Priapism is a prolonged and painful erection that can last from several hours up to a few days. The priapismic erection is NOT associated with sexual thoughts or sexual activity. What happens is that blood flows into the penis, but is unable to drain as it would in a normally flaccid penis. Considering that the penis provides little room for blood to circulate, the blood becomes stagnant after a while, acidifies, and loses oxygen. Without oxygen, the red blood cells become stiff, and even less able to squeeze their way out of the penis. Very few cases of priapism are without an obvious cause. In most cases, priapism results from one of two things: certain medications or medical conditions. Under the category of medications, there are two primary culprits. For one, penile injections used to treat some forms of impotence can cause priapism; however, this usually only happens when a man decides, on his own, to increase the dosage. Second, psychiatric medications, such as anti-depressants, seem to cause some cases of priapism; however, how these drugs affect priapism is unclear. The second major cause of priapism is certain medical conditions and diseases generally, any that cause the blood to thicken, or cause red blood cells to lose their flexibility and mobility. Sickle-cell anemia and leukemia are probably the most common conditions that cause priapism. Most men will seek treatment for this painful erection before it goes on for too long, usually within four to six hours. This is good since priapism can scar the penis if not treated early enough, and this can lead to impotence. In most cases, treatment involves draining the penis with a needle placed in the side of the penis. Medications that act on the blood vessels can also be injected to help shrink blood vessels and, thus, decrease blood flow into the penis. Sickle-cell anemia patients with priapism are usually treated by blood transfusion.

Prozac has been a lifesaver for me, but it given me uncomfortable/
painful erections. These happen only in the middle of the night.
I discover them when I get up at night to urinate.
I stopped taking prozac for awhile, and the erections very slowly
subsided after about one month. Then I started prozac again (20
mgs.) and developed the problem again after 7 or so days. (The only way to get the erections to go down is for me to stand up, and it takes maybe a minute or so for them to go away. Sometimes they come right back if I lie down, for some reason.) Questions: does this sound serious? Could this be a drug interaction? I also take bentyl and sometimes Fiorinal-type migraine meds. Should I be concerned about zoloft causing the same problems?

Special Medical Reports The following is part of " Tips From Other Journals " from the September 1, 1997 edition of American Family Physician Diagnosis and Management of Prolonged Penile Erection Priapism, a spontaneous, prolonged, usually painful penile erection, results from abnormal regulation of penile flaccidity. Harmon and Nehra review the diagnosis and management of different types of priapism. High-flow or arterial priapism results from increased arterial inflow into the cavernosal sinusoids, overwhelming venous outflow. This may cause a painless semirigid to rigid erection. Damage to cells is rare because of high oxygenation. Causes of high-flow priapism include idiopathic etiologies and groin or saddle trauma resulting in pudendal artery damage. Management of high-flow priapism is elective. In the flaccid state, vascular inflow is limited because of resistance caused by contracted cavernous arterial branches and sinuses. Neurotransmitters or vasoactive substances relax smooth muscles, with decreased compliance resulting in minimal resistance to incoming blood flow. Expansion of the sinusoidal walls inhibits venous outflow, and an erection occurs. Assessment of priapism includes a detailed history and a physical examination, observing the penile shaft for rigidity and pain. A complete blood count and a sickle cell preparation may be indicated. Medications most commonly associated with priapism include trazodone and chlorpromazine. Urologists treating erectile dysfunction with intracavernosal drug therapy using papaverine, phentolamine and prostaglandin E

    Alice, Wow. I just read your description of priapism. I never knew that there were some very serious causes. A couple of years ago, I had priapism that lasted many, many months. I didn't consider going to a doctor, and at times, it was painful. I was eventually diagnosed with leukemia. I was treated and the priapism went away. Only recently have my erections returned to "normal." I guess that was one gift-horse I should have looked into directly.

    I've been suffering from accute priapism for the past 6 years. I have undergone numerous (30 or so, and painful too)check-ups and diagnoses, with different andrologists/urologists, but to no avail. I take "Androcur" pills to get along. DESCRIPTION:
    I get very painful erections during the night, punctually towards the end of every month. The pennis is very very rigid (more than the normal sexual intercourse errection) and cold. The pain extends from the anus (inclusive) through the base of the pennis up to its tip. And the lateral muscles of the pennis. When the erections occur, I can neither urinate nor stool and I have a great urge to vomit (not nausea, though!) and fart. I get several attacks per night, ie., as soon as I fall asleep again. In order to let the erection go down (it takes 2/3 hours to go down) I have to get out of bed and WAKE up completely, ie., endure in some reasoning activity (such as write a document/report, play complex computer games, read, etc.) in the middle of the night. Musclar activities (gym, physical education, jogging, stretching, etc.) have no effect. Not even a cold bath! When I start farting it's good news: it means that the erection plus the excruciating pain is going reciding.

    Priapism and Impotence
    Priapism is a condition wherein a man experiences penile erection without sexual stimulation for an extended time. The erection may last a few hours or even days. In addition to being embarrassing and inconvenient, priapism is often painful and it can result in extensive tissue damage to the penis, as well as long-term impotence if not treated promptly. Fortunately, it is uncommon compared to other forms of sexual dysfunction, and many men can return to normal sex lives after receiving proper treatment. In general, priapism results from a disturbance to the mechanisms that control penile detumescence (the process by which the penis becomes flaccid). Blood enters the penis, but it cannot drain properly because it becomes trapped in the corpora cavernosa, a large chamber in the penis. The penis glans (head) and area around the urethra, however, remain flaccid.

    I want to learn the best way to deal with it and overcome my fears. not too many patients of sickle cell speak of this in public. the male friends of mines who suffer from this, thank me for speaking out. But, i'm only one voice and wish we all had an understanding of everything relating to sickle cell disease, even the ones we are ashamed of. But with sickle cell you can imagine our cells tend to sickle at their own leisure. All the sickled cells need is a place confined enough to get stuck and clog up.
