Priapism
Priapism is a condition in which a male develops a permanent erection. If your erection lasts for an unusual period of time and is unrelated to sexual contact, go directly to the emergency department of the nearest hospital. Although this can be a potentially embarrassing situation, waiting to correct priapism may result in permanent damage. Penile injection therapy for impotence physical trauma to the penis or surrounding region diseases which thicken the blood like leukemia or sickle-cell anemia cancerous growth around the penis preventing outflow of blood in some cases there is no known cause. The physician will choose a different treatment depending on the severity of the priapism. Medication, or minor surgery to remove the old blood from the penis are the most common treatment options.

  • Abnormal male horniness. Priapus was the ancient Greek god of procreation. An erection that won't go down, even after four hours. Up to that long is nothing to worry about. It can be caused by supersensitivity to impotence drugs such as Viagra or prostaglandins. 1 in 500 people have this. It can be caused by overdosing on such drugs. It can also happen naturally. It can be treated in stages. Three extra strength Sudafed should help, apply ice, jog 30 minutes then take a cold shower. If that does not work go to the emergency ward of your nearest hospital. They will first try xylocaine and draining the blood out of the penis with a needle. If that does not work they will try phenylephrine. If that does not work they will try neo-synephrine. If that does not work they will use surgery. If the problem is not treated within 12 hours, permanent damage to the penis may result from lack of oxygen.

    Definition: Priapism is a persistent, unwanted erection of the penis.
    Priapism may occur in one of the following ways:
  • There may be repeated, reversible painful erections occurring over several hours (penis becomes erect then erection goes away, then becomes erect again, then erection goes away, etc.) This is also called "stuttering" priapism. There is not a problem with sexual functioning once the episode has gotten better. Repeated episodes of this type of priapism are common and occur in up to 40% of patients. This type of priapism can usually be managed at home. Patients are encouraged to take warm baths, increase fluid intake and empty their bladder (urinate) often. If the episode is not better after 3 hours, notify the doctor. Take Tylenol for pain.
  • There may be a prolonged, painful erection that does not go away for more than several hours. This can last for up to several days or weeks. This type of priapism needs attention by a doctor. Without medical treatment, severe priapism can lead to partial or complete impotence in over 80% of cases. Sometimes a blood transfusion is given during the attack.
  • Persistent penile enlargement or hardening that may last for weeks to years; this type of priapism is usually painless. It usually develops after a long episode of priapism (as described in #2 above). Sexual functioning is often impaired (for example-impotence).

    Priapism is a prolonged painful erection of the penis that commonly occurs in children and adolescents with sickle cell disease, often starting during the early morning hours. It occurs in two forms: (a) stuttering episodes which last less than 2-4 hours but are often recurrent and may precede a severe episode, and (b) severe events that last more than 2-4 hours and may eventually result in impotence. Simple maneuvers such as increasing oral fluids, taking analgesics, urination, moderate exercise, and/or taking a bath or shower may help end an episode of priapism, and no further specific intervention may be required. Patients who have frequent episodes ( 2 within one month or 4 within one year) should contact their sickle cell program for elective evaluation. For such patients, priapism prophylaxis with pseudoephedrine 30 mg/po hs ( 10 years) or 60 mg/po hs ( 10 years) should be considered. Any episode that lasts longer than 2-4 hours should be considered an emergency that requires prompt medical intervention as described below.

According to one study, the mean age at which priapism occurs is 12 years, and by the age of 20, as many as 89 percent of males with SCD will have experienced one or more episodes of priapism (1). Priapism in males with SCD is due to vaso-occlusion, which causes obstruction of the venous drainage of the penis. Priapism can be classified as prolonged if it lasts for more than three hours or stuttering if it lasts for more than a few minutes but less than three hours and resolves spontaneously; however, such stuttering episodes may recur or develop into more prolonged events. Prolonged priapism is an emergency that requires urologic consultation.

    Doctors warn of a dangerous new method of cocaine abuse: injecting the drug directly into the urinary tract. Physicians from New York Hospital-Cornell Medical Center reported the case of a 34-year-old man who suffered severe bleeding under the skin after pumping cocaine into his urethra. It led to complications that destroyed his penis, nine fingers, and parts of his legs. "They fill an eye dropper or a syringe with a cocaine solution and inject it into the penis," said Dr. Samuel Perry, a professor of clinical psychiatry. The man had injected cocaine before intercourse in an effort to enhance sexual performance. He was admitted to the hospital because his penis had remained erect for three days, resulting in a painful inability to urinate. The medical term for a prolonged erection is "priapism." On his third day in the hospital, the man's erection suddenly subsided. Over the next 12 hours, blood leaked into the tissues of his feet, hands, genitals, back and chest. Blood coagulation caused tissues to die over large areas of the patient's body, and he was transferred to the burn unit of New York Hospital-Cornell Medical Center.

Priapism is diagnosed when an erection lasts more than four hours, in the absence of any sexual stimulation.
