Mens' Health: the Prostate Gland and Benign Prostatic Hypertrophy
The prostate is a walnut sized gland that lies below the male bladder. It secretes fluid during ejaculation. Statistics vary somewhat, but by age fifty, about 30 per cent of men have some symptoms of prostate enlargement known as benign prostatic hypertrophy (BPH). By age sixty the figure jumps to 50 per cent, and to 80 per cent by age seventy. Symptoms relate to the position of the prostate: surrounding the urethra, at the base of the bladder. Enlargement of the prostate can increase the frequency of urination. Having to get up at night one or more times to urinate can be an early sign. As well, decreased force of urine stream, and problems starting and stopping the flow are common symptoms. Over the age of forty, men should have a prostate exam once per year. While this can detect BPH, it is also a screening exam for early detection of prostate cancer. If you develop increased frequency of urination, or other difficulties with urination, have your prostate checked. If your health care provider finds anything unusual, they may recommend further screening, as symptoms, while typically benign, can be warning signs for other conditions such as prostate inflammation, or even prostate cancer. A number of factors influence the development of benign prostatic hypertrophy. Factors in the development of BPH may include prostaglandin deficiency, heavy metal toxicity, increased activity of 5 alpha reductase enzyme, and decreased sensitivity of prostate cells to 5 hydroxy testosterone. Nutrition is very important both in development and treatment of BPH. Caffeine, alcohol, tobacco and high fat foods should be avoided. High carbohydrate diets can also be a factor. Keep pollutants, pesticides and chemicals out of your diet as much as possible: eat organic. Keeping your sugar intake to a minimum also decreases prostate cancer risk. If you have high cholesterol, this should be treated as well, as there is a relationship to BPH. Consult your Naturopathic Physician for more information.

Subjective effectiveness was observed in the improvement of sense of residual urine (92%), retardation (86%), night frequency (85%), strain on urination (56%), protraction (53%) and forceless urinary stream (53%). The overall subjective effectiveness was 80% of patients, and the overall objective effectiveness was 54% of patients. Night frequency, residual urine volume and tidal urine volume were improved significantly. The overall effectiveness was 80%. No side effects were observed. Prostate is a gland within the male reproductive system that is located just below the bladder. The prostate surrounds the beginning of the urethra, the canal that empties the bladder. With BPH, there are benign (non-cancerous) nodules that start growing in the prostate gland. This growth causes the prostate to enlarge. Causes and Risks The actual cause of benign prostatic hyperplasia (BPH) is unknown. However, it has been noted that men who have had their testicles destroyed or removed do not develop BPH. Furthermore, after castration, BPH has been observed to regress. Since the presence of normally functioning testicles appears to be necessary for the development of BPH, it is thought that the growing tissue uses male hormones differently than normal prostate tissue does. Although this tissue growth is benign , progressive growth of the tumor may cause significant obstruction of the urethra and interfere with the normal flow of urine.
The incidence of BPH increases with advancing age. BPH is so common that it has been said, "All men will have benign prostatic hyperplasia if they live long enough!" A small amount of BPH is present in 80% of men over 40 years old and over 95% of men 80 years old. No risk factors have been identified other than having normally functioning testicles.

A sixty-four year old businessman consulted the physicians at Lancaster for the problem of prostate enlargement with urinary frequency and an increasing level of the PSA protein, correlating with the increasing volume of the gland. Prostate diseases are common in men as they age. The prostate gland is a small walnut-shaped gland that sits below the bladder. Through it passes the urethra which is a tube that carries urine from the bladder out the penis. As a man ages, the prostate gland enlarges in size and can cause symptoms of urinary obstruction. In addition, the enlarged prostate gland, as well as the prostate gland in younger men, can be involved with prostatitis which is an inflammation or infection within the prostate gland. This is usually treated with antibiotics. Symptoms of both these conditions would include frequency of urination, urgent urination, slowing of the urinary stream, urinary incontinence, retention, and frequent urination at night. The Section of Urology offers all contemporary treatment options for the treatment of BPH. Medical therapies do exist for treatment of BPH and these include alpha blockers, such as Hytrin, Cardura, and Flomax which are drugs designed to decrease the resistance at the level of the prostate by relaxing smooth muscle fibers around the prostate such that flow is improved.

  • Signs and symptoms include decreased force and caliber of urinary stream, hesitancy, retention, postmicturition dribbling, double voiding (patient voids and is able to void again in 5 to 10 minutes), and overflow urinary incontinence (on straining or coughing). Irritative symptoms such as dysuria, frequency, nocturia, urgency, hematuria, and incontinence occur frequently. Flank pain during micturition, suprapubic pain, and azotemic symptoms occur less commonly. Exam . The bladder may be distended, and the prostate is enlarged, smooth, and symmetric. The prostate gland may be soft or firm and possibly nodular. However, the nodules lack the stony-hard consistency associated with carcinoma. Laboratory findings . UA may reveal signs of infection. If the obstruction has been severe enough to impair renal function, BUN and creatinine may be elevated. PSA may be elevated.

    The prostate is a small gland present in men, located just below the bladder. The prostate encloses the urine passage (urethra) and is usually about the size of a walnut. Although men have a prostate all their lives it usually begins to enlarge at mid-life. As men age, the prostate continues to grow and may push in on the urine passage. A prostate gland that is larger than normal is said to be hypertrophic, and the condition is called benign prostatic hypertrophy (BPH). An enlarged prostate does not indicate prostate cancer. The enlargement caused by aging is inevitable, but suffering from symptoms of an enlarged prostate is not. The Department of Urology at Wayne State University has extensive experience treating men with BPH. We have been a leader in urologic research and innovative surgical treatment. Most of the diagnostic procedures for BPH can be completed in office.

    Transurethral resection of the prostate is a common surgery for BPH often nicknamed "roto-rooter" surgery. In a TURP an instrument is passed through the urine passage to the point where the prostate is blocking the flow of urine. The excess tissue is then cut away. There is no incision on the outside of the body and usually a hospital stay of only a few days is required. TURP has been used for many years and most patients notice a rapid improvement in symptoms which lasts for many years.
