Benign symptomatic prostatic hypertrophy will affect up to 25% of men. Transurethral resection of the prostate is an effective therapy, but has adverse effects and may not be required in less severe cases. Medical therapies such as alpha blockers and 5alpha reductase inhibitors may be indicated in uncomplicated cases or in patients who are unfit, unwilling or waiting to undergo surgery. Alpha1 adrenergic blocking drugs such as prazosin and terazosin are more effective than placebo at relieving obstructive urinary symptoms.
Finasteride (a 5alpha reductase inhibitor) acts by blocking the conversion of testosterone to active intracellular dihydrotestosterone and will often decrease the size of the prostate. Although it has minimal adverse effects, finasteride only slightly improves urinary flow rates. It may take 6months for the maximum benefit to occur and finasteride must be continued indefinitely, otherwise the prostate will return to its previous size and symptoms will return.

Benign prostatic hypertrophy What is benign prostatic hypertrophy? Benign prostatic hypertrophy, sometimes referred to by its initials, BPH, is an enlargement of the prostate gland. It is not cancer. The prostate gland is an organ that is about the size of a walnut and is located just below the bladder in front of the rectum. It surrounds the upper end of the urethra, the tube that carries urine from the bladder to the end of the penis. The prostate gland produces the seminal fluid that is ejaculated during orgasm. The gland begins to grow to its adult size with the onset of puberty. By the age of 20 years, it has reached its full size and weighs three-quarters of an ounce. It begins to enlarge again in most men during late middle-age. What are the causes of BPH and who is at risk? This very common condition is found in men over the age of 50 years. It is uncertain what causes it, but it may be linked to changes in hormone levels caused by the ageing process. The cells that make up the prostate begin to grow and multiply. This growth of the prostate compresses the urethra, which causes an obstruction to the flow of urine and therefore makes it more difficult both to pass urine and to empty the bladder completely. What is Your Prostate?
The prostate makes some of the milky fluid (semen) that carries sperm. The gland is the size of a walnut and is found just below the bladder, which stores urine. The prostate wraps around a tube (the urethra) that carries urine from the bladder out through the tip of the penis ( Figure 1 ). During a man's orgasm (sexual climax), muscles squeeze the prostate's fluid into the urethra. Sperm, which are made in the testicles, also go into the urethra during orgasm. The milky fluid carries the sperm through the penis during orgasm.

  • Benign (buh-nine) prostatic (prah-stah-tik) hypertrophy (hi-per-tro-fee) is when the prostate (prah-state) slows grows larger. It is also called "BPH." The prostate is a male sex gland that helps makes semen. It sits below the bladder and wraps around the urethra like a donut. The urethra (u-ree-thruh) is the tube that carries urine from the bladder into the penis. The urine is then passed out of the body.
    As the prostate grows, it can squeeze the urethra (u-ree-thruh) making it harder to urinate (pass urine). This can block urine flow and cause urinary problems. BPH is very common in men over 45 years of age but rarely causes problems before age 60. Tests will be done to learn about your prostate. You may need drug treatment or prostate surgery if your urinary problems get worse.
Causes: It is not known what causes BPH. It may be just part of getting older. Some caregivers believe it is caused by a change in hormones as men age.

Pathological Considerations
A common entity which occurs in about four out of every five men in America over the age of 50 years. It is characterized by growth of multiple fibroadenomas modules enlarging the prostate gland and compromising the urethra causing variable degrees of bladder outlet obstruction.
Physiological Considerations
Progressive urinary frequency and urgency with incomplete emptying and rapid refilling of the bladder, hesitancy and intermittency, and decreased size and force of the urinary stream are common diagnostic signs. Good nutrition over a lifetime appears to be the only safeguard against occurrence of this condition in older males. A lack of fat soluble vitamins A, D, E, wheat germ oil (because of natural hormones believed to occur in wheat germ oil), unsaturated fatty acids (vitamin F), and certain minerals, especially zinc - appear to be directly responsible for prostatic hypertrophy. Abstinence from alcohol and tobacco and use of hot sitz baths are required.

A major area of activity in the field of urology focuses on the prostate. The prostate gland is a male accessory sex organ comprised of 30-50 branched glands, weighs about 20 grams and measures 4 x 2 x 3 centimeters (about the size of a walnut). It is surrounded by a fibroelastic capsule that penetrates the gland to divide it into lobes. The capsule and the stroma within the gland contain smooth muscle cells capable of contracting to expel the prostatic fluid. The prostate lies in front of the rectum just under the bladder and surrounds a portion of the urethra. Ejaculatory ducts join the urethra within the prostate gland. The prostate produces an alkaline fluid rich in citric acid, spermine, cholesterol, phosphlipids, fibrinogenase, fibrinolysin, zinc and acid phosphatase, and other proteins. People usually develop early symptoms of enlarged prostate after the age of 50 and most of the men have markedly enlarged prostate by the age of 80 (often with associated cancer).
Most of the above patients develop symptoms of prostatism, now known as ‘Lower Urinary Tract Symptoms’ (LUTS). These are due to irritability of the bladder (now labelled as ‘storage’ symptoms), the symptoms being frequency of urine, nocturia, urgency and urge incontinence. If the prostate is moderately enlarged, then patients could have obstructive (now known as ‘voiding’) symptoms like hesitancy (starting trouble), poor stream, straining to void, intermittency of flow, dysuria, post-micturition dribbling or incomplete evacuation.
In my opinion (consistent with the modern world literature) diagnosing prostatism should be handled like detecting hypertension which is so common in normal population. Most of the GPs will be eager to prescribe some drug. All hypertensive patients should be immediately treated with non-pharmacological (DASH) line of treatment first. It should be tried for a few months or even longer before prescribing any drug.
In private practice, the doctors should manage prostatism problem in the same manner. There will be some part of the population which is not bothered by these symptoms and can be left alone. For the rest of them, ‘non-pharmacological treatment’ should always be advised first, before putting the patients on alpha blockers permanently (the cost of which is about Rs. 300-500 per month at least and that too for a lifetime).

The prostatic regression effect of oral administration of a new steroidal anti-androgen, osaterone acetate, was investigated in dogs with prostatic hypertrophy. To dogs with prostatic hypertrophy, 0.1-1.0 mg/kg of osaterone acetate was orally administered for one week, and the regression rate was observed. It was shown that administration of osaterone acetate at 0.2 mg/kg or higher, sharply regressed prostatic hypertrophy during the early stage. Therefore, this agent may be clinically applicable as a therapeutic agent for benign prostatic hypertrophy.

Benign Prostatic Hyperplasia - BPH
Half of all 50-year-old men have benign prostatic hyperplasia (BPH). (Formerly called "benign prostatic hypertrophy.") BPH is a major disease of the prostate. In BPH, the prostate swells and restricts the urethra, causing urinary flow problems such as less flow, with less force while urinating often followed by dribbling. Because, with continued enlargement, the bladder may not emptied fully, urination is difficult or impossible and there is an increased risk of urinary tract infection and kidney damage. BPH should be managed in consultation with a healthcare professional. for most men, successful treatment of BPH usually requires continuous use of one or a combination of herbs. In Europe, herbs are now a leading method for managing early stages of BPH. In America, conventional treatment for obstructive BPH is "roto-rooter" surgery. This is a programme run jointly by Colchester PCT and Tendring PCT. Historically men with lower urinary tract symptoms (that's what most of us would refer to as "water works problems") have been seen by their GP and then, if necessary, been referred on to the local hospital. The hospital would then give them an appointment to attend the hospital to have an assessment carried out on a specific date. However, since October 2001 specially trained and equipped nurses have assessed patients in the community. These assessments take place in designated GP's surgeries so that they are easier for people to get to and the patient can choose where and when they have their assessment. If you have any urinary symptoms then contact your GP who can refer you on directly to one of the nurses. If you have a question about symptoms then you can call one of the nurses directly on the phone numbers shown below. What is BPH? Benign prostatic hyperplasia (BPH) is the name given to enlargement of the prostate gland. This enlargement is believed to be caused by the effects of male sex hormones. Forty per cent of men aged over 70 years experience problems because of their BPH (see box opposite). Some men begin to have difficulty in passing water, because the enlarged prostate gland squashes the tube leading from the bladder. This causes the tube to narrow, obstructing urine flow from the body. Your doctor will check your symptoms and examine you to check the size of your prostate and may carry out a blood test. The prostate is a small gland found in men at the base of the bladder. It is about the size of a chestnut and sits around the tube through which urine passes from the bladder when you pass water.
The role of the prostate is not understood fully. It is believed to assist in the production of semen.
