Diagnosis: Prune belly syndrome
Brief history:
33 year old man with slowly increasing creatinine. Images: Posterior images of abdomen through 20 minutes. Both rows of images are the same but the bottom row is at higher intensity to show the soft tissues. View main image(rs) in a separate image viewer View second image(rs). Post-furosemide posterior images of the abdomen, 5 minutes per frame for 30 minutes. Full history/Diagnosis is available below
Diagnosis: Prune belly syndrome
Full history:
33-year old man with known diagnosis of prune belly syndrome and history of multiple prior urinary tract infections who now presents with a slowly increasing serum creatinine. The patient has had a prior vesicostomy. Renal scintigraphy was requested to evaluate relative renal function and exclude renal obstruction.
Findings:
Renal scintigraphy shows a small right kidney with normal left renal function. The collecting systems are dilated bilaterally, worse on the left. The ureters are laterally deviated, which is not unusual in patients with prune belly syndrome. There is no evidence of urodynamically significant obstruction, as documented by prompt clearance of activity from both renal collecting systems after administration of intravenous furosemide.
Discussion:
This case simply demonstrates a renal scintigraphic appearance of an adult patient with prune belly syndrome.

The prune belly syndrome gets its name from the characteristic wrinkled appearance of the abdominal wall because of variable degrees of muscle hypoplasia. In addition, there are associated major urinary tract abnormalities and undescended testes in male patients. Rarely, female cases of prune belly syndrome are associated with major genital tract abnormalities. There is good evidence to support the view that this condition is secondary to fetal lower urinary tract obstruction. Also, the distended bladder or possibly the megaureters hinder the descent of the testes from the abdomen to the scrotum. An alternative view to the etiology of this condition is that it is secondary to abnormal mesenchymal development.
ANATOMIC FEATURES
The abdominal wall defect is variable. Above the umbilicus, the oblique muscles and the rectus abdominis are usually affected.

Description: Prune Belly Syndrome is very rare, occurring in just one in 80,000 births. Of those, approximately 20 percent are stillborn and another 30 percent die within two years of birth. The remaining 50 percent often suffer from urinary system disorders, cardiovascular problems, and/or musculoskeletal problems, including hip dislocation, scoliosis and clubfoot.

LOS ANGELES, CA (Oct. 30, 2002) About as complex as it getsthat's how pediatric urologist Andrew Freedman, M.D., director of pediatric urology at Cedars-Sinai Medical Center's Endourology Institute, describes the surgery he performed this summer on then 17- month-old Jalen Brown, born with Prune Belly Syndrome. That surgery required reconstructing the toddler's urinary system in a nearly 10-hour procedure.
Prune Belly Syndrome, also known as Eagle-Barrett Syndrome, is a very rare occurrence: just one in 80,000 births. An estimated 99 percent of those affected are boys. The cause is still unknown, though some theories suggest urinary tract obstructions and resulting distention or developmental defects in utero.

What is prune belly syndrome?
Prune belly syndrome is also known as triad syndrome or Eagle-Barrett syndrome. It is characterized by a triad of abnormalities that include the following:
  • absence of abdominal muscles
  • undescended testicles - a condition seen in newborns whereby one (or both) of the male testes has not passed down into the scrotal sac
  • an abnormal, expanded bladder and problems in the upper urinary tract, which may include the bladder, ureters, and kidneys
Because of the substantial involvement of the urinary tract, children with prune belly syndrome are usually unable to completely empty their bladders and have serious bladder, ureter, and kidney impairment. A child with prune belly syndrome may also have other birth defects. Most commonly, these defects involve the skeletal system, intestines, and heart. Girls may have defects in their external genitalia, as well. Some infants who have prune belly syndrome may be stillborn or die within a few months of birth.

The underlying causes of prune belly syndrome are unknown. There is blockage to the flow of urine from the kidney to the outside of the body during fetal life. For example, dilatation of the urinary tract may develop as a result of urethral obstruction in the womb. The incidence is approximately 1 in 30,000 to 40,000 births. Boys are almost solely affected.
Expectant mothers carrying affected infants may develop varying degrees of oligohydramnios (insufficient amniotic fluid ) that predisposes the infant to lung problems (see Potter syndrome ). The

Au cours de la troisième semaine, un troisième feuillet, le chordomésoblaste (appelé aussi mésoderme) vient se glisser entre l'ectoderme et l'endoderme.
Ce sont ces trois feuillets qui sont à l'origine des organes et de l'ensemble des systèmes qui vont permettre à l'organisme humain de fonctionner normalement (voir correspondance entre ces différents feuillets et les organes du corps). Progressivement, l'embryon est animé de flexions et de plicatures (plicature embryonnaire) dans le sens de sa longueur et de sa largeur (transversal). Le chordomésoblaste va donner un tube placé dans l'axe vertical (axe crâniocaudal : de la tête vers la queue) comportant de part et d'autre l'extrémité céphalique et l'extrémité caudale.
Autour de ce tube s'organisent d'autres tissus appelés les somites, à partir desquels se développeront les cartilages et les muscles. A la fin de la troisième semaine, les capillaires (micro-vaisseaux) sont présents : ils commencent à drainer dans le cordon ombilical par l'intermédiaire de l'artère et de la veine ombilicales.

Is it possible for a prune belly to have a successful kidney
transplant? My nephew has to start dialysis and has been told there has
never been a successful prune belly transplant. Is this true? Can you suggest anyone
he could see?
Answer:
The prune belly syndrome is a congenital abnormality that consists of deficient abdominal wall muscles, urinary tract abnormalities, and undescended testicles. The prune belly syndrome does not exclude the possibility of successful kidney transplantation. The first kidney transplant in a patient with prune belly syndrome was reported in 1977 (Schenasky and Whelchel, Journal of Urology, Jan 1976, p112). You should recommend that your nephew's parents seek a referal to a center with experience with pediatric kidney transplantation.
