I strive to make this support group one where you are loved and treated with respect at all times. I want this to truly be a place where you loved and treated gently. Sometimes there are differences of opinion among our members. This is good and I try not to allow these differences to come between us, but try to help all to understand that we were created with these differences and they should not cause division but are truly reasons to celebrate. Diversity in our disease makes each of us respond differently to treatments and diversity in our personal lives make us unique individuals. You are "UNIQUELY YOU" and not like any other. I love having people of every color and creed in out group. I hope you will visit here often and send me suggestions for improving "OUR" page. This site is intended to be a friendly site. It is intended to be a haven in the midst of the very impersonal World Wide Web and a refuge from the cruel world. I hope you will feel at home here and want to tell others about it. I will be featuring PTC patients on this site. a.k.a. Idiopathic Intracranial Hypertension, Benign Intracranial Hypertension
The main means of treating pseudotumor is to relieve symptoms. Medications that decrease the production of spinal fluid or increase its absorption are used quite frequently. In doing this the headache and pressure on the optic nerve, which causes the vision loss, is relieved to some point. Spinal taps drain some of the fluid, but within a day that pressure starts to build again. Shunting is another means of relieving this pressure. A small-diameter tube is placed either in the head or spinal column, with a one way pressure sensitive valve in it,that drains the excess fluid to another place in the body to be absorbed. Another procedure that is done is primarily to relieve the vision problems associated with pseudotumor. It is called an Optic Nerve Sheath Fenestration, meaning they put slits into the sheath that covers the optic nerve to relieve the pressure.
There have been many theories about pseudotumor over the years. It was first believed to be only women. But there are males that have this diagnosis now to dispell that myth. It was also believed to only be in obese patients. There are now normal weight as well as underweight patients who have documented pseudotumor. Some have gone so far to attribute it to eating polar bear meat. There aren't too many that eat polar bear meat anymore so that has fallen by the way side as well. They are making correlations between the use of some medications and pseudotumor, but time will tell which of these is true medical finding or just coincidence. It was also believed that pseudotumor is "self-limiting" and would "only" last up to two years. Another myth. There are some that have had this syndrome for many years and continually have symptoms.

Purpose: Our specific aim is to assess major thrombophilic and hypofibrinolytic pathoetiologies of pseudotumor cerebri ('benign' intracranial hypertension). Our second specific aim is to determine whether treatment of the intracranial hypertension with Diamox will produce symptomatic relief. We postulate that when exogenous thrombophilic factors (estrogen-containing oral contraceptives, estrogens, corticosteroids) are superimposed on heritable thrombophilic and/or hypofibrinolytic coagulation disorders, multiple ischemic cerebral strokes occur. We postulate that as these strokes resolve, they leave a residual effect of increased intracranial pressure, either by virtue of reduced cerebral clearance of cerebrospinal fluid (CSF), or increased production of CSF, or both. We expect that most patients seen by ophthalmologists with pseudotumor cerebri can be shown to have a coagulation disorder as the pathoetiology, particularly when exogenous estrogens are superimposed on a coagulation disorder. Significance in Relationship to Human Health: Recently, it has been realized that sequelae of ischemic stroke, particularly those ischemic strokes mediated by coagulation disorders, may be pseudotumor cerebri (intracranial hypertension).

(Columbia, Missouri) 2,000 new cases of pseudotumor cerebri will be identified in the U.S. in 2001 , yet there are no prospective randomized controlled trials. Quincke (1), in 1897 , was the first to describe the condition known today as pseudotumor cerebri (PTC). The hallmark of this disorder includes: 1) symptoms and signs of increased intracranial pressure (e.g., headache, diplopia, pulsatile tinnitus, transient visual obscuration, visual acuity and visual field loss, and papilledema), but with mental alertness; 2) normal neuroradiologic imaging, apart from possible small ventricles; 3) documented elevated intracranial pressure (ICP) of greater than 250 mm of water; and 4) normal cerebrospinal fluid analysis. It is important to recall that there is a small subset of patients who have PTC with headache and elevated ICP, but with intact spontaneous venous pulsation and absent papilledema (2,3). Pseudotumor cerebri has been ascribed various terminologies including otitic hydrocephalus, toxic hydrocephalus, hypertensive meningeal hydrops, serous meningitis, menstrual edema with intracranial hypertension, increased intracranial pressure without brain tumor, and idiopathic intracranial hypertension. In 1955, Foley (4) introduced the term benign intracranial hypertension. However, the study by Corbett et al (5), which documented approximately half of their 57 patients with PTC having visual acuity or visual field loss - six of whom had bilateral blindness, indicated that this condition was certainly not benign. The term introduced in 1904 by Nonne (6) and which has withstood the test of time is, however, "pseudotumor cerebri."

Unfortunately, the term has been used to describe patients with a potpourri of clinical conditions. Accordingly, pseudotumor cerebri likely represents multiple and different disease entities. Definition: A benign process affecting the brain which appears to be, but is not a tumor. It is characterized by increased intracranial pressure and normal brain ventricle size. There is no evidence of tumor, infection, blocked drainage of the fluid surrounding the brain or any other cause.

Causes, incidence, and risk factors: The major symptoms of pseudotumor are increased pressure within the skull ( increased intracranial pressure ICP ). The cause for the condition itself is unknown, and the diagnosis is made when other health conditions are ruled out. The mechanism causing the elevated ICP is not well understood. Possible causes of pseudotumor cerebri include a defect in Cerebral Spinal Fluid (CSF) absorption, increased cerebral swelling ( edema ) or increased cerebral blood volumes. Other factors which are possibly associated with this condition are recent weight gain, menstrual irregularities, and the presence of too much Vitamin A (hypervitaminosis A). Oral retinoid drugs and some antibiotics are other possible causes as is the stopping of steroid use after an extended period. Other conditions that have been associated with pseudotumour cerebri include renal failure, sleep apnea, and some lung diseases which lead to the retention of carbon dioxide. Less frequently, Guillain-Barre syndrome has been found to be associated.

Treatment: Treatment must be directed at the condition causing the pseudotumor. An initial lumbar puncture is both diagnostic and therapeutic. Repeated lumbar punctures may be done to decrease the intracranial pressure and to help prevent progressive papilledema and visual loss. Other treatments may include:
  • Fluid and/or salt restriction. Medications: Corticosteroids, glycerol, acetazolamide, and furosemide. Shunting procedures from the lumbar subarachnoid space to the peritoneal cavity. Incision of the optic nerve sheath for relief of the papilledema, and to prevent further deterioration of vision. Weight loss is usually recommended. Close follow-up and monitoring of vision, since there is potential for progressive and sometimes permanent visual loss. Follow-up MRI or CT scan is sometimes necessary to exclude hidden (occult) malignancy (cancer).

Expectations (prognosis): The outcome varies, and sometimes the condition disappears on its own within 6 months. About 10-20% of the people diagnosed with pseudotumor cerebri experience recurrences, and there is a small percentage who become progressively worse and may eventually become blind.

Pseudotumor cerebri A syndrome of increased intracranial pressure (determined usually by lumbar puncture) with normal sized ventricles (no hydrocephalus) or mass lesion seen CT or MR imaging of the brain. The cause of pseudotumor is unknown but in some patients may be related to obesity or taking medications (including some vitamins). The increased intracranial pressure is associated with the development of papilledema which can result in injury to the optic nerve with blindness. The treatment of pseudotumor is directed to the prevention of blindness and can include fenestration of the optic nerve sheath as well as ventricular or lumbar cerebrospinal fluid shunting.
