Frensley Center for Imaging Research, Houston, USA St. Luke's Episcopal Hospital, Department of Neurology, Houston, USA University of Texas Medical Branch, Department of Ophthalmology, Galveston, USA PURPOSE: To underscore the deficiencies of standard computed imaging studies for detecting occult cerebral vascular conditions that can cause pseudotumor cerebri. METHODS: Two females with normal, unenhanced computed tomography (CT) scans of the brain were initially thought to have pseudotumor cerebri. Both patients experienced a progressive clinical course. Further imaging revealed a torcular meningioma in case one, that was documented on enhanced magnetic resonance (MR) scan. The second case had a dural arteriovenous malformation identified on cerebral angiography. CONCLUSION: A normal noncontrast CT scan is insufficient to rule out partial obstruction of the dural sinuses by a meningioma or dural arteriovenous malformations that may increase the pressure within the dural sinuses. Thus, contrast CT or MR imaging and in some cases cerebral arteriography may be required in cases that manifest atypical clinical features.

RIFAAT Neurosurgery Department, Cairo University. SHERIF A KAMEL AMER Ophthalmology Department, Cairo University. ABSTRACT Purpose To evaluate optic nerve sheath decompression (ONSD) in pseuedotumor cerebri (PTC) patients as regards visual acuity, visual field, appearance of the optic nerve head, and operative and postoperative complications. Methods This study was performed on 22 patients with PTC and progressive visual loss under maximum tolerable medical therapy. Nineteen out of 22 patients were females (86.3%) while only 3 patients (13.7%) were males. All patients had bilateral papilledema. Thirty one surgeries were performed on 30 eyes of 22 patients, 14 patients had unilateral surgery while 8 patients had bilateral surgeries. Results Papilledema resoluted in all eyes following surgery including the 14 unoperated eyes in response to surgery in the other eye. Visual field examination showed improvement in all tested eyes except 2 eyes due to surgical failure and a vascular complication of surgery.

Wisse Aim of this study was to report the follow-up of papilledema in patients with pseudotumor cerebri with a laser-tomographic method. Patients and methods: 10 eyes of five patients with bilateral papilledema and pseudotumor cerebri were examined with a Heidelberg Retina Tomograph (HRT, 2.01) in the mean course of 4,6 ± 2,8 months. The surface of the retina in the upper nasal sector, outside the edematous retina was used as a reference plane. The contour line was drawn by extrapolation on the reflectivity image and transfered to the follow-up images in order to keep the measured disc area equal. On 15º- or 20º- images the following variables were calculated: volume of the neuroretinal rim (VNR), volume of the cup (VC), mean height of the contour line (MHC) and steepness of the cup (TM) for the entire optic disc and in four sectors. Reliability of the reference plane and the volume of the neuroretinal rim was calculated by measurements on five days on five optic discs. Results: Rim volume showed the most pronounced change, decreasing in the course by 0,7mm

Patienten und Methoden: 10 Augen von fünf Patienten mit bilateralem Papillenödem bei Pseudotumor cerebri wurden im Verlauf von im Mittel 4,6 2,8 Monaten mit dem Heidelberg Retina Tomographen (HRT, Version 2.01) untersucht. Als Referenzebene für die Variablen wurde die Netzhautoberfläche nasal oben, außerhalb des ödematösen Bereichs gewählt. Die Papillenbegrenzung wurde anhand des Reflektionsbildes extrapoliert und auf die nachfolgenden Kontrollaufnahmen übertragen um die Papillenfläche konstant zu halten. An 15 - bzw. 20 -Aufnahmen wurden die Variablen Randsaumvolumen (RV), Exkavationsvolumen (EV), mittlere Höhe der Konturlinie (MHK) von der Netzhautoberfläche und die Steilheit der Exkavation (TM) für die gesamte Papille und in vier Sektoren bestimmt. Die Reliabilität der Referenzebene und des Randsaumvolumens wurde durch wiederholte Festlegung an fünf Tagen an fünf Papillen bestimmt. Ergebnisse: Das Randsaumvolumen zeigte die ausgeprägteste Änderung mit einer Abnahme um im Mittel von 0,7mm

Êàê ïðàâèëî, òàêàÿ ãîëîâíàÿ áîëü ãåíåðàëèçîâàíà, íàèáîëåå ñèëüíî âûðàæåíà ñ óòðà, óñèëèâàåòñÿ ïðè ïðîáå Âàëüñàëüâû, ïðè êàøëå èëè ÷èõàíèè (çà ñ÷åò ïîâûøåíèÿ äàâëåíèÿ âî âíóòðè÷åðåïíûõ âåíàõ). Íàðóøåíèÿ çðåíèÿ, ïî ðàçíûì äàííûì, âñòðå÷àþòñÿ â 35–70% ñëó÷àåâ. Ñèìïòîìû íàðóøåíèÿ çðåíèÿ àíàëîãè÷íû òàêîâûì ïðè ëþáûõ äðóãèõ âèäàõ âíóòðè÷åðåïíîé ãèïåðòåíçèè.
