Pseudoxanthoma elascticum (PXE) is an inherited connective tissue disorder in which the elastic fibers present in the skin, eyes, and cardiovascular system gradually become calcified and inelastic. Description The course of PXE varies greatly between individuals. Typically it is first noticed during adolescence as yellow-orange bumps on the side of the neck. Similar bumps may appear at other places where the skin bends a lot, like the backs of the knees and the insides of the elbows. The skin in these areas tends to get thick, leathery, inelastic, and acquire extra folds. These skin problems have no serious consequences, and for some people, the disease progresses no further. Bruch's membrane, a layer of elastic fibers in front of the retina, becomes calcified in some people with PXE. Calcification causes cracks in Bruch's membrane, which can be seen through an ophthalmoscope as red, brown, or gray streaks called angioid streaks. The cracks can eventually (e.g., in 10-20 years) cause bleeding, and the usual resultant scarring leads to central vision deterioration. However, peripheral vision is unaffected. Arterial walls and heart valves contain elastic fibers that can become calcified. This leads to a greater susceptibility to the conditions that are associated with hardening of the arteries in the normal aging population-high blood pressure, heart attack, stroke, and arterial obstruction-and, similarly, mitral valve prolapse. Heart disease and hypertension associated with PXE have been reported in children as young as four to 13 years of age. Although often appearing at a younger age, the overall incidence of these conditions is only slightly higher for people with PXE than it is in the general population.

The ARA (anthracycline resistance-associated) gene that codes for a putative member of the ABC transporters has recently been cloned ( UI:97069542 ) and shown to have high sequence homology to the gene for MRP. Kuss et al. (1994, UI:94276679 ) have previously shown MRP to be deleted in a subset of inv(16) leukemic patients. The deletion of MRP was associated with an improved patient survival compared with inv(16) patients who did not have such a deletion ( UI:96430264 ). Kuss et al. (1998, UI:98389632 ) have mapped the ARA gene to 16p13.1, in the same physical interval as the inv(16) short-arm breakpoint. It is shown to be situated proximal to both MYH11 (

Associated persons: Ferdinand-Jean Darier Ester Elizabeth Grönblad Oswald Marchesani James Victor Strandberg ... Franz Wirz Description: A rare hereditary disorder with degenerated elastic fibres present in the skin, blood vessels and heart, due to calcification of elastic fibres. The skin abnormalities present with raised, soft yellowish papules, especially around the mouth, neck, axillae, elbows, groin, and periumbilical areas, sometimes involving most parts of the skin. The skin has been likened to the appearance of the skin of a plucked chicken. Ocular features usually include angioid streaks, retinal haemorrhage, loss of visual acuity, and, sometimes, macular degeneration and chorioretinitis. Cardiovascular complications generally consist of absence of the pulse in the arms and legs, intermittent claudication, hypertension, angina pectoris, peripheral periarterial calcification, and stenosis of the celiac artery. There are four genetically distinct variants - 2 dominant and 2 autosomal recessive forms. It occurs in the second decade of life or later, more often in females, and frequently in consanguineous marriages. Occasionally associated with Paget’s syndrome, Marfan’s syndrome, Herrick’s syndrome, Albers-Schönberg’s syndrome.

Pseudoxanthomas, yellow papules, are present on the neck, cubital fossa, popliteal fossa and groin. Gastrointestinal haemorrhage is frequent due to abnormal mucosal arterioles. Premature atherosclerosis of the coronary arteries is the major cardiovascular manifestation. Endocardial fibroelastosis and mitral valve prolapse are occasionally associated cardiac lesions.
