Bedridden . . . Daniel Johns struggles through the Big Day Out. By Christine Sams The lead singer of silverchair, Daniel Johns, has been struck down by reactive arthritis, a condition which has left the 22-year-old bedridden in his Newcastle home. The unusual form of arthritis, where joints swell up painfully, only occurs when a person is struck down by flu or a virus. Johns has been suffering from reactive arthritis for at least two months but his illness only came to light when he was unable to leave his home. He was forced to shuffle across the stage during his performance at the Big Day Out in late January, but is now relying on a walking stick to move around. Professor Allan Sturgess, an arthritis expert from the University of NSW, said: "It is often too painful to walk with this condition and it mainly affects the knees and ankles. National Guideline on the Management of Sexually Acquired Reactive Arthritis Clinical Effectiveness Group (Association for Genitourinary Medicine and the Medical Society for the Study of Venereal Diseases) Aetiology Reactive arthritis (ReA) is a sterile inflammation of the synovial membrane, tendons and fascia triggered by an infection at a distant site, usually gastro-intestinal or genital. ReA triggered by a sexually transmitted infection (STI) is referred to as sexually acquired reactive arthritis (SARA). This includes sexually acquired Reiter’s syndrome, described as the triad of urethritis, arthritis and conjunctivitis, with or without other cutaneous or mucous membrane lesions such as, keratoderma blennorrhagica, circinate balanitis/vulvitis, uveitis, oral ulceration, cardiac or neurological involvement. Most commonly lower genital tract infections, either urethritis or cervicitis, are associated with SARA with objective features of SARA being present in 0.8-4% of cases. The place of upper genital tract infection, such as prostatitis and salpingitis, is unresolved. Previously, it was suggested that infection with the human immune deficiency virus (HIV) was directly associated with SARA but current evidence suggests this is not so.

    This condition primarily affects the spine, but it may also cause arthritis in the hips, shoulders, and knees. The tendons and ligaments around the bones and joints in the spine become inflamed. Symptoms include pain and stiffness, especially in the lower back. Ankylosing spondylitis tends to develop during late adolescence or early adulthood.     As international travel to developing countries increases, more people seek medical advice concerning food and water-borne diseases, including typhoid fever. Prevention of typhoid fever in high-risk groups (travelers to endemic areas, laboratory workers and household contacts of typhoid carriers) should rely primarily on prevention of exposure. However, immunization is an important adjunct. The decision to immunize against typhoid fever should be individualized, taking into account the benefits versus the risk of possible adverse reactions. Cases of reactive arthritis have been associated with the heat-phenol inactivated 'whole cell' parenteral vaccine, but to our knowledge reactive arthritis has not been previously reported with the oral form (Ty21a). This is a report of HLA-B27 negative reactive arthritis occurring in two travelers after the administration of oral Ty21a typhoid vaccine.

    During an outbreak of Yersinia pseudotuberculosis III, one of two HLA-B27 positive brothers developed reactive arthritis (ReA), mild at first, but later severely destructive and ultimately fatal. The reactivation of ReA was possibly triggered by an oral polio vaccine. The cause of death was severe secondary amyloidosis. The other brother was exposed to the same Y. pseudotuberculosis strain but did not develop any disease during or after the outbreak. However, he later developed ReA due to a Salmonella infection, with a benign course.

    Gout
    Gout is caused by uric acid crystals in the joint. These are irritating to the joint and cause an inflammatory reaction. This usually causes an sudden intensely painful swelling of a single joint, often the big toe. It is diagnosed by taking a small amout of fluid from the joint and examining this for crystals of uric acid. These attacks are treated with anti-inflammatory drugs, and further attacks are prevented by lowering the levels of uric acid in the blood by special diets and drugs.
    Psoriatic arthritis
    This diagnosis is suspected if the patient has psoriasis, but sometimes the arthritis can preceed the rash. It can mimic many other kinds of arthritis. Sometimes "pitting" of the nails is a clue; the nail looks a bit like the surface of a thimble with little indentations.
    Reactive arthritis
    Sometimes arthritis starts after an infection such as gastroenteritis or a sexually transmitted disease. It usually involves only one or two large joints such as knees or ankles, and may also involve the lower back.
    Viral and Post-viral Arthritis
    Some viral infections can result in a transient arthritis. Examples are mumps, German measles (rubella) or parvovirus infection. There may also be a skin rash and the arthritis usually gets better by itself, though this may take several months. Rarely these can turn into a chronic arthritis such as rheumatoid arthritis.

    Peer Review Status: Internally Peer Reviewed Enthesis is a difficult word, even cumbersome to say. It means "to insert" in Greek and refers to the bone insertion of tendon or ligament. It brings understanding to what may seem four different forms of arthritis, all "enthesopathies" or insertion diseases. The first is ankylosing (fusing) spondylitis (vertebral inflammation) also called Marie-Strumpell disease. Late, untreated cases may develop "bamboo" or "poker" spine because of the X ray appearance or totally stiff back of advanced disease. It is sometimes mistakenly called rheumatoid spine. These all affect centrally located cartilage joints of the spine and breastbone. In contrast rheumatoid arthritis affects peripheral joints with lubricated sac-like capsules, called synovial joints from "syn" joined and "oon" egg-white fluid. Reactive new bone formation is typical and shows as X ray bony spurs or bridging between vertebrae. All four have inflammation at tendons or ligament attachments which can calcify.

    • Rheumatoid arthritis Rheumatoid Arthritis (RA) not only affects the joints but also the whole body. Many people confuse RA and arthritis. Arthritis affects individual joints compared with RA which affects whole joints of body and attacks other organs of body. RA is an autoimmune disease which causes chronic inflammatory disorder that leads to the destruction of cartilage and connective tissue, resulting in destruction and deformity of the joints. Autoimmune diseases are illnesses which occur when the body tissues are mistakenly attacked by its own immune system. Patients with these diseases have antibodies in their blood which target their own body tissue, where they can be associated with inflammation. Because it can affect other organs of the body, RA is referred to as a systemic illness and is sometimes called rheumatoid disease.

    A number of people (male and female) suffer from painful joints of such severity that they take painkillers e.g. paracetamol for relief or pay a visit to their general practitioners. In some of these cases the joint pain (arthralgia) is due to inflammation of the joint (arthritis) following an infection in some other part of the body. This is known as reactive arthritis. On some occasions the infection would have been acquired following sexual activities. In sexually acquired reactive arthritis (SARA), asymmetric polyarthralgia which may be migrating is the worst common presenting symptom. Some people may have one joint involved while others have several joints involved, and the common joints are the elbow, wrist, knee and ankles. Hip joint involvement is uncommon and the condition may present acutely of sub-acutely. Reiters syndrome consists of urethritis, conjunctivitis and arthritis.
    Clinical features
    The history of men seen recently in our department are shown in table 1. More men than women are diagnosed with reactive arthritis. The male:female ratio shown in table 2 may be due to underdiagnosis in women. Inflammation of the joint usually occurs within four weeks after the infection. This may be accompanied by low back pain, muscular pains and low grade fever. Other parts of the body that may be involved are the urethra, the eyes and the heart - carditis. Skin manifestations include keratoderma blennorrhagica and erythema nodosum. In some patients there may be frequent recurrence of the joint pains while in others it may become chronic.

    What is reactive arthritis?
    Reactive arthritis, also known as Reiter's syndrome, is a type of arthritis that occurs as a reaction to an infection somewhere in the body. Most infections that cause the disease originate in the genitourinary tract (the bladder, urethra, penis, or vagina) and are spread through sexual intercourse, a form of the disease called genitourinary Reiter's syndrome, or urogenital Reiter's syndrome.     LEIBMANN: Most people would know that the Australian rock group Silver Chair has been forced to cancel sold out concerts in Sydney, New York and London, because their front man, Daniel Johns is not well enough to tour. His illness is unusual for someone in his twenties. It is Reactive Arthritis, a very painful condition leading to chronic swelling of the joints, and impaired movement, and of course, for a guitar player like Daniel Johns, it can potentially be career threatening. Joining us now, is our Health Editor, Dr Kerryn Phelps. Morning. PHELPS: Morning, Steve. LEIBMANN: What is Reactive Arthritis? PHELPS: Well, as the name suggests, the reaction to something, and it is an arthritis, which is a swelling of joints, and it is a reaction, usually to an underlying infection of some sort, that triggers of this immune response in the body. Now, that underlying infection can be a food poisoning type of problem, like Campylobacter, or Shigella or salmonella. It can also be a sexually transmitted disease, like Chlamydia. There are a range of other viruses and bacteria that can also trigger Reactive Arthritis, not all of them have been confirmed, but certainly the food poisoning ones, and they Chlamydia, are well described.
